The role of VWF for the success of immune tolerance induction.

One of the primary and most serious treatment-related complications in haemophilia A is the formation of anti-factor VIM (FVIII) antibodies, which significantly impacts patient care. For these patients, immunomodulatory therapy becomes important to induce immunological tolerance to FVIII. Immune tolerance induction (ITI) is an efficient therapeutic approach to eliminating inhibitors. Several ITI protocols are currently in use, including the Malmö-, Bonn- and van-Creveld-protocol. Successful utilisation of these protocols enables regular FVIII treatment (in the case of surgery and bleeding), pr

Immune tolerance to drugs. (II).: Long-term tolerability of nimesulide in patients with NSAID hypersensitivity.

Nimesulide is well-tolerated as an alternative to nonsteroidal antiinflammatory drugs (NSAIDs) in patients with a previous adverse reaction to other classes of NSAIDs. However, there is little information in the literature about its long-term tolerability. The study was carried out on 625 patients who had experienced adverse reactions to one or more NSAIDs. All patients received and tolerated peroral challenges with nimesulide. On the first day, patients were given an equivalent number of placebo doses to the planned number of nimesulide doses. In a successive session, the test was administere

Immune tolerance to drugs. I. Long-term tolerability of rokitamycin in patients with antibiotics hypersensitivity.

Macrolides are considered one of the safest anti-infective groups in clinical use and are well-tolerated as alternative antibiotics in patients with a previous adverse reaction to other classes of antibiotics. However there is scarce information in the literature about their long-term tolerability. The present study was performed to determine whether the results of a challenge test with rokitamycin could predict the response to ingestion of rokitamycin during illness. The study was carried out on 335 patients, who experienced adverse reactions to one or more antibiotics. All patients received

Impaired tolerance in the International Immune Tolerance Induction study due to the presence of very low-titer inhibitors.

BACKGROUND: In persons with hemophilia A (pwHA), development of an inhibitor complicates treatment with factor (F)VIII replacement therapy. Immune tolerance induction (ITI) is used to restore tolerance. However, some pwHA do not reach tolerance despite disappearance of the inhibitor detected with the Nijmegen-modified Bethesda assay (NBA), potentially due to the persistent presence of very low-titer inhibitors. OBJECTIVES: We determined the presence of very low-titer inhibitors with the Nijmegen ultrasensitive Bethesda assay (NusBA) in the International-ITI study. METHODS: This study include

Engineered Immune Constructs Alter Antigen-Specific Immune Tolerance and Confer Durable Protection in Myelin-Driven Autoimmunity.

Autoimmune diseases are broadly characterized as a failure in immune tolerance. In multiple sclerosis (MS), autoreactive immune cells attack the protective myelin sheath lining neurons in the central nervous system. Therapeutic strategies that selectively and durably restore immune tolerance without broad immunosuppression are urgently needed for MS. Our lab has developed assemblies of immune constructs built entirely from myelin antigen (MOG35-55 or PLP139-151) and regulatory innate immune cues (GpG) using layer-by-layer self-assembly. Here, we present mechanistic and translational data showi

Innovative approaches targeting innate immune cells to promote organ transplant tolerance.

Achieving long-term allograft survival while minimizing systemic immunosuppression (IS) remains a critical unmet need in transplantation. While adaptive immunity has traditionally been the primary focus of IS therapy, innate immune cells-that include neutrophils, monocytes, macrophages, dendritic cells, myeloid-derived suppressor cells, innate lymphoid cells (ILCs), natural killer (NK) cells, and gamma delta (γδ) T cells act as key upstream orchestrators of allograft rejection and tolerance. Recent advances in single-cell RNA sequencing and spatial transcriptomics have unveiled the profound he

Type 2 immune history trains lung macrophages for viral disease tolerance.

Severe respiratory viral disease varies widely among individuals and often reflects immunopathology rather than inadequate pathogen control, suggesting that previous immune history can prime the lungs towards disease tolerance. Here we show that nerve- and airway-associated macrophages (NAMs), a subset of interstitial macrophages, expand ephemerally after type 2 inflammation induced by Nippostrongylus brasiliensis. We therefore hypothesized that NAMs acquire epigenetically imprinted trained immunity and tested this using a heterologous challenge model in which mice that were previously infecte

Distinct thymic pDC populations promote tumor immune tolerance through complementary mechanisms.

Thymic central tolerance is crucial for preventing autoimmunity, but its contribution to tumor immune evasion remains poorly understood. Here, we demonstrate that plasmacytoid dendritic cells (pDCs) in the thymus have two distinct subsets, accumulating in the thymus of tumor-bearing mice, contributing to immune tolerance through clonal deletion of tumor-specific T cells and reducing newly generated T cells. Mechanistically, common dendritic cell progenitor-derived pDCs (CDP-pDCs) capture tumor antigens and migrate to the thymus in a CCR9-dependent manner, where they present these antigens to i

Post-Translational Modified Neoantigens in Autoimmune Diseases: Challenges of Immune Tolerance.

Autoimmune diseases, like spondyloarthritis (SpA) and rheumatoid arthritis (RA), have a high incidence and disability rate and are major diseases that seriously endanger human health. The pathogenesis of autoimmune diseases involves the interaction among genetic factors, environmental factors, and immune disorders. The post-translational modified neoantigens are the key nodal of these three factors. Under the intervention of the external environment, changes in metabolic status in vivo induce metabolite-related post-translational modifications (PTMs). These PTM proteins and peptides, after bei

Methicillin-resistant and susceptible Staphylococcus aureus: tolerance, immune evasion and treatment.

Staphylococcus aureus is capable of infecting every organ system in the body and developing resistance to every available antibiotic used to treat it, such as methicillin-resistant S. aureus (commonly referred to as MRSA). This pathogen is characterized by the sudden emergence of virulent new clones and an array of mechanisms to circumvent the immune system and antibiotics. Furthermore, despite the development of new antibiotics and a growing body of high-quality data to inform their use, S. aureus continues to be a leading bacterial cause of death worldwide. In this Review, we provide an over

Sustained exposure to Helicobacter pylori induces immune tolerance by desensitizing TLR6.

Helicobacter pylori (H. pylori, Hp) has been designated a class I carcinogen and is closely associated with severe gastric diseases. During colonization in the gastric mucosa, H. pylori develops immune escape by inducing host immune tolerance. The gastric epithelium acts as the first line of defense against H. pylori, with Toll-like receptors (TLRs) in gastric epithelial cells being sensitive to H. pylori components and subsequently activating the innate immune system. However, the mechanism of immune tolerance induced by H. pylori through the TLR signalling pathway has not been fully elucidat

The Role of Exosomes in Central Immune Tolerance and Myasthenia Gravis.

BACKGROUND: Immune homeostasis plays a crucial role in immunology andis dependent on both central and peripheral tolerance. Centraltolerance and peripheral tolerance occur in the thymus and thesecondary lymphoid tissues, respectively. Tolerance breakdown andimmune regulation defects can lead to autoimmune disorders. In thisreview article, we aimed to describe the role of exosomes inregulating central tolerance and provide a summary of their effectson the pathogenesis, diagnosis, and therapeutic potential inmyasthenia gravis (MG). METHODS: Articles for this review wereidentified using the PubM

Cellular strategies to induce immune tolerance after liver transplantation: Clinical perspectives.

Liver transplantation (LT) has become the most efficient treatment for pediatric and adult end-stage liver disease and the survival time after transplantation is becoming longer due to the development of surgical techniques and perioperative management. However, long-term side-effects of immunosuppressants, like infection, metabolic disorders and malignant tumor are gaining more attention. Immune tolerance is the status in which LT recipients no longer need to take any immunosuppressants, but the liver function and intrahepatic histology maintain normal. The approaches to achieve immune tolera

Platelet-targeted gene therapy induces immune tolerance in hemophilia and beyond.

Blood platelets have unique storage and delivery capabilities. Platelets play fundamental roles in hemostasis, inflammatory reactions, and immune responses. Beyond their functions, platelets have been used as a target for gene therapy. Platelet-targeted gene therapy aims to deliver a sustained expression of neo-protein in vivo by genetically modifying the target cells, resulting in a cure for the disease. Even though there has been substantial progress in the field of gene therapy, the potential development of immune responses to transgene products or vectors remains a significant concern. Of

Th1-like Treg in vitiligo: An incompetent regulator in immune tolerance.

Vitiligo is an autoimmune skin disease resulting from epidermal melanocyte destruction mediated by CD8+T cells that breach the self-tolerance. Regulatory T cells (Tregs) are critical for keeping the CD8+T cells in check, but the deficiency of Tregs leading to the immune disequilibrium in vitiligo remains undefined. In the present study, we used RNA-sequencing (RNA-seq) to acquire the transcriptome data of Tregs from vitiligo patients and healthy controls, respectively. Further flow cytometry analysis and immunofluorescence assays substantiated the phenotype of Th1-like Tregs in vitiligo. CD8+T

Immune tolerance therapies for autoimmune diseases: Shifting the goalpost to cure.

Autoimmune rheumatic diseases are characterised by an autoimmune inflammatory response to antigens of synovial tissue, muscles, and other organs. While the prognosis of these disorders has improved remarkably over recent years with the advent of biological therapeutics, prolonged drug-free remission is still rare. Advances in the understanding of the immunopathogenesis and response to immunotherapy of seropositive autoimmune rheumatic diseases, such as rheumatoid arthritis systemic lupus erythematosus, type 1 diabetes and the autoimmune-like celiac disease have revealed novel therapeutic oppor

Erythropoeitin Signaling in Macrophages Promotes Dying Cell Clearance and Immune Tolerance.

The failure of apoptotic cell clearance is linked to autoimmune diseases, nonresolving inflammation, and developmental abnormalities; however, pathways that regulate phagocytes for efficient apoptotic cell clearance remain poorly known. Apoptotic cells release find-me signals to recruit phagocytes to initiate their clearance. Here we found that find-me signal sphingosine 1-phosphate (S1P) activated macrophage erythropoietin (EPO) signaling promoted apoptotic cell clearance and immune tolerance.
