Extramedullary plasmacytoma of the parotid gland: report of a case and review of the literature.

A case of primary extramedullary plasmacytoma (PEMP) with local amyloid deposition arising in the left parotid gland of a 78-year-old woman is described. The present case was studied with immunologic techniques with results confirming that the neoplastic cells were of monoclonal nature, producing lambda-light chains alone. PEMPs of the salivary glands are rare, with only 8 cases reported in the literature. The salient features of these 9 cases are summarized. Problems connected with the clinical, pathological and therapeutic aspects of PEMP with regard to the rare parotid location are discusse

Pseudonodular T cell lymphoblastic lymphoma.

T cell lymphoblastic lymphoma usually presents as a rapidly growing lymphoma with histologic features of a diffuse, poorly-differentiated small cell lymphoma with a high mitotic rate. This report describes a patient who presented with an aggressive small cell lymphoma that morphologically had a nodular pattern. Repeated biopsy and use of cell surface markers resulted in reclassifying the cell type as T cell lymphoblastic and in recognizing the original pattern as pseudonodular. The routine application of these immunologic techniques in patients with lymphoma may lead to more accurate diagnoses

Host factors and infectious diseases in the elderly.

Because of declining function of their immunologic and other organ systems, elderly individuals are more susceptible to certain types of infections of their heart valves, meninges, urinary tract, and skin. They may develop serious sepsis with subtle clinical signs and symptoms, which makes accurate diagnosis difficult. The antimicrobial agents of choice for use against the organisms most commonly responsible for these septic symdromes are listed in Tables 2, 3, and 4. Some methods of preventing these infections are discussed, but further studies to verify old techniques and establish new ones

Laboratory methods for the diagnosis and confirmation of systemic mycoses.

The concomitant use of culture and nonculture techniques for the diagnosis and confirmation of systemic mycotic infections is recommended and assessed. A diagnosis based upon a single specimen and method, especially when the results are negative, is not always conclusive. In many situations, a single specimen and test method cannot be relied upon for the diagnosis of a fungal infection. The testing of multiple or serial specimens with a battery of procedures increases the chances for establishing a rapid and definitive diagnosis. Positive cultures generally provide unequivocal evidence of infe

Remarks on the diagnosis of Clonorchis sinensis infection.

A number of techniques have been developed for the diagnosis of Clonorchis sinensis infection. Although the most reliable technique for this infection are the various stool examinations in the laboratory, practical problems such as difficulties of stool collection and mass examinations need more applicable methods for epidemiological studies. During past decades some serologic and immunologic methods such as intradermal skin test, gel-diffusion technique, complement fixation test, haemagglutination test, fluorescent antibody test and enzyme-linked immunosorbent assay (ELISA) have been introduc

Blood sparing in craniosynostosis surgery.

One of the main risks of craniosynostosis surgery is the possible need for an allogenic blood transfusion (ABT). Most patients are operated on in the first months of life, when physiological conditions are particularly sensitive to even limited blood losses. Furthermore, most surgical techniques proposed in the past were based on extensive craniectomies and cranial remodeling. Because of the known infective and immunologic risks of ABT, in recent years more attention has been dedicated to factors that might help reduce the risk of ABT. We review recent preoperative (ie, erythropoietin administ

Cholera as a model for research on mucosal immunity and development of oral vaccines.

During the past year, the extensive investigational use of a recently developed oral vaccine against cholera has led to significant advances in our understanding of both immunity to cholera and related diarrhoeal diseases, and the mucosal immune response in general after oral immunization. The oral cholera vaccine has been shown to protect, through its cholera toxin B subunit component, against travellers' diarrhoea caused by enterotoxigenic Escherichia coli. The elaboration of sensitive new techniques has allowed detailed clonal analyses of the activation of specific B and T cells and immunol

Microsurgical Technique and Results of Anorectal Transplantation in the Rat.

UNLABELLED: Anorectal transplantation (ART) is an obvious therapeutic option for treating permanent colostomy and severe fecal incontinence. The rat is the best model for beginning studies of a new surgical procedure. In this article, we review ART techniques in rats. METHODS: We reviewed articles on rat and ART keywords throughout Cochrane Library, MEDLINE, and EMBASE. Five articles were found, of which 2 used autotransplantations, 1 performed only transplantation of the anal canal and extraperitoneal rectum, and another performed transplantation of the entire intestine, including the anus,

The etiologic role of infectious antigens in sarcoidosis pathogenesis.

Sarcoidosis is a disease of unknown etiology, characterized pathologically by noncaseating granulomas that most commonly involve the lung, skin, lymph nodes, and eyes. Syndromes with similar pathological and immunologic features to sarcoidosis such as chronic beryllium disease, hypersensitivity pneumonitis, and tuberculosis illustrate that granulomatous diseases may or may not have an infectious etiology. Although the etiology of sarcoidosis remains unknown, recent molecular, genetic, and immunologic studies strengthen the association of sarcoidosis with infectious antigens. Currently, the str

[Intracellular and extracellular acid mucopolysaccharides in human bronchus carcinoma and lung metastases - prognostic parameters?].

Local cellular and extracellular (acid mucopolysaccharide) reactions in human lung cancers and metastases - prognostic parameters? The role of immunologic reactions in human malignant tumors is still unsettled. In the work presented, 72 primary lung cancers and 17 pulmonary metastases of 8 different organs of origin were studied with histochemical techniques for cellular infiltrates (eosinophils and macrophages) and extracellular acid mucopolysaccharides (AMPS). The results showed a positive correlation between eosinophils and macrophages and a negative correlation between cellular reactions a

Kidney transplantation procedures in rats: assessments, complications, and management.

Kidney transplantation in rats is an experimental model often used for the development of general microsurgical or transplantation techniques, for immunologic studies, and for analyzing transplant-associated long-term arterial blood-pressure changes. The aim of the present study was to analyze different surgical techniques of kidney transplantation in rats, with emphasis on minimizing surgical complications and establishing guidelines for their prevention and management. Complications were categorized into general (e.g., core body temperature drop, ischemic time) and surgically related vascula

Options for immunologic support of renal transplantation through the HLA and immunology laboratories.

HLA and immunology laboratories are an integral part of clinical kidney transplant programs. They assist transplant clinicians with evaluating the immunological suitability of potential recipients for transplantation and selecting donor-recipient combinations with a low risk of immunological failure. With sophisticated new techniques becoming available for posttransplant immunological monitoring, laboratories play an increasing supporting role during posttransplant follow up. The level of precision at which immunological testing predicts clinical outcome, however, leaves room for improvement.

Abdominal Wall Closure in Intestinal and Multivisceral Transplantation: A State-Of-The-Art Review of Vascularized Abdominal Wall and Nonvascularized Rectus Fascia Transplantation.

Failure to close the abdomen after intestinal or multivisceral transplantation (Tx) remains a frequently occurring problem. Two attractive reconstruction methods, especially in large abdominal wall defects, are full-thickness abdominal wall vascularized composite allograft (AW-VCA) and nonvascularized rectus fascia (NVRF) Tx. This review compares surgical technique, immunology, integration, clinical experience, and indications of both techniques. In AW-VCA Tx, vascular anastomosis is required and the graft undergoes hypotrophy post-Tx. Furthermore, it has immunologic benefits and good clinical

Cross-over kidney transplantation with simultaneous laparoscopic living donor nephrectomy: initial experience.

With cross-over living donor kidney transplantation, immunologic incompatibilities within the original donor/recipient pair can be overcome. As minimal invasive techniques for organ recovery are increasingly applied, this should also be performed in a cross-over kidney transplantation. We present the first report of a successful simultaneous laparoscopic kidney recovery for cross-over kidney transplantation as well as a review of the international practice of cross-over kidney transplantation in the context of national laws. Cross-over kidney transplantation should be encouraged. A databank on

Update on immunofluorescent testing in bullous diseases and lupus erythematosus.

One of the earliest methods for investigating the immunologic basis of disease was the use of immunofluorescence (IF) on skin biopsy specimens and serum. IF can be used to detect immunoglobulins, complement components, and fibrin. In some diseases, the IF findings are disease specific and diagnostic, especially in certain bullous diseases. The use of IF testing has increased with the recent development of the salt split skin technique. The newer IF findings in some well-established immunodermatologic diseases and several recently described diseases are discussed. A brief description of the tec

Megakaryoblastic transformation of a myeloproliferative disorder.

A case of megakaryoblastic transformation of a myeloproliferative disorder presenting initially as chronic granulocytic leukaemia with an intermediate phase of essential thrombocythaemia in a Chinese woman of 65 years is reported. The diagnosis of megakaryoblastic crisis was based on morphological, cytochemical and immunocytochemical features present in blast cells in the blood, together with cytochemical and ultrastructural features of micromegakaryocytes and megakaryoblasts which were predominant in the marrow. A literature review showed newer refinements in diagnosis such as ultrastructural

The laboratory diagnosis of Legionnaires' disease.

Legionnaires' disease can be diagnosed by culture, by direct detection of the bacterium or its products by using immunologic means, with a DNA probe, and by serologic means. Culture diagnosis is the most specific and sensitive test. Legionellae can be isolated from sputum samples by using selective techniques. Antibody detection is more suited for epidemiologic purposes than for use in individual cases. Immunofluorescent microscopy is a useful and rapid means of diagnosis. Alternative rapid and specific tests are urinary antigen detection and the use of a DNA probe. Culture must always be perf

Erythroadsorption and enzyme-linked immunoassays (EAIA and ELISA) for specific circulating antibodies and antigens in schistosomiasis.

We developed a method of immunologic titration by erythroadsorption in order to assay anti-Schistosoma mansoni soluble cercarial antigen IgM antibodies and anti-S. mansoni soluble worm and egg antigen IgG antibodies. This method was compared to the ELISA technique. Results obtained using the two techniques were similar, although the erythroadsorption technique was less sensitive for identifying IgM during the acute phase of the disease. Both assay systems revealed a correlation between the level of circulating soluble antigens contained in the eggs and the number of eggs found in the stools of
