Removing environmental barriers to independent living: A feasibility randomised controlled trial targeting people ageing with long-term physical disabilities.

INTRODUCTION: People ageing with long-term physical disabilities face unique challenges to maintaining independence. We developed a novel, evidence-based intervention - Removing Environmental Barriers to Independent Living - to support participation and reduce falls. METHODS: To examine Removing environmental barriers to independent living's feasibility and estimate its magnitude of efficacy, we conducted a single-blinded randomised controlled trial. Fifty participants who were aged 45-65 years with a physical disability for at least 5 years were randomised to intervention (n = 24) or control

Degree of Housing Instability Shows Independent "Dose-Response" With Virologic Suppression Rates Among People Living With Human Immunodeficiency Virus.

Housing instability negatively impacts outcomes in people [living] with human immunodeficiency virus (PLHIV), yet the effect of diverse living arrangements has not previously been evaluated. Using 6 dwelling types to measure housing status, we found a strong inverse association between housing instability and viral suppression across a spectrum of unstable housing arrangements.

Muscle Measures and Nutritional Status at Hospital Admission Predict Survival and Independent Living of Older Patients - the EMPOWER Study.

OBJECTIVES: Older adults with sarcopenia and malnutrition are at risk for co-morbidities, hospitalization, institutionalization, and mortality. In case of hospitalization, risks may be further increased, especially in case of suboptimal dietary intake. The aim of our study was to assess whether muscle mass, muscle strength, functional performance, and nutritional status at hospital admission were associated with survival and independent living among older patients three months after discharge. DESIGN, SETTING, PARTICIPANTS: The EMPOWER study was an observational, prospective and longitudinal

A phenomenological study of routine, independence, and help-seeking in home hospice patients living alone with terminal illness.

BACKGROUND: Patients living alone with terminal illness in home hospice face challenges distinct from those living with caregivers. While prevailing research describes patterns of service use, less is known about the lived meanings that shape how these patients organize their days and decide when to seek assistance. This study describes the lived experience of solo-dwelling adults receiving home hospice care and the conditions under which help is sought or refused. This addresses a gap that limits person-centred planning for patients who wish to remain at home. METHODS: Twelve patients of a h

The Impact of Trunk Function and Lower Limb Paralysis on Independence in Activities of Daily Living Among Stroke Patients.

BACKGROUND: Stroke is a major global health issue, and many patients experience motor paralysis and sensory impairments that affect their independence in the activities of daily living (ADLs). Trunk and lower limb functions are crucial in poststroke ADL independence. Although these two functions are closely related, a few studies have evaluated them in combination, and the importance of assessment methods that consider their mutual relationship has not been thoroughly examined. In this study, we aimed to clarify the degree to which trunk function and lower limb paralysis impact ADL independenc

Participation and autonomy, independence in activities of daily living and upper extremity functioning in individuals with spinal cord injury.

Improvements in care and rehabilitation have resulted in a higher proportion of people living with spinal cord injury (SCI), which calls for an increased focus on participation and autonomy. This observational cross-sectional study investigated the impact of SCI on autonomy and how it correlates to activity performance and upper extremity functioning. A total of 25 adults (mean age 58 years) with chronic cervical or thoracic SCI were included. Self-perceived autonomy was measured with Impact on Participation and Autonomy questionnaire, independence in activities of daily living (ADL) with Spin

Infection with HIV-1 subtype D adversely affects the live expectancy independently of antiretroviral drug use.

INTRODUCTION: HIV-1 subtypes have been associated with less favourable clinical profiles, differences in disease progression and higher risk of neurocognitive deficit. In this study we aimed to analyse the long term survival disparities between patients infected with the most common HIV-1 variants observed in Poland. METHODS: For the study data from 518 Caucasian non-immigrant patients of Polish origin infected with divergent HIV subtypes and variants [subtype A (n = 35, 6.8%), subtype B (n = 386, 74.5%), subtype C (n = 13, 2.5%), subtype D (n = 58, 11.19%) or other non-A,B,C,D (n = 26, 5.01%

The Effectiveness of a PRoactive Multicomponent Intervention Program on Disability in Independently Living Older People: A Randomized Controlled Trial.

BACKGROUND: There is an increase in functional limitations and a decline in physical and mental well-being with age. Very few effective lifestyle interventions are available to prevent adverse outcomes such as disability in (pre-) frail older people. The effectiveness of an interdisciplinary multicomponent intervention program to prevent disability in older people in the community was tested. METHOD: A randomized controlled trial (RCT) with a one-year follow-up was conducted in the Netherlands. Community-dwelling pre-frail older people aged 65 years and over were invited to participate. Protective efficacy afforded by live Pasteurella multocida vaccines in chickens is independent of lipopolysaccharide outer core structure.

Pasteurella multocida is a major animal pathogen that causes a range of diseases including fowl cholera. P. multocida infections result in considerable losses to layer and breeder flocks in poultry industries worldwide. Both killed whole-cell and live-attenuated vaccines are available; these vaccines vary in their protective efficacy, particularly against heterologous strains. Moreover, until recently there was no knowledge of P. multocida LPS genetics and structure to determine precisely how LPS structure affects the protective capacity of these vaccines. In this study we show that defined li

Socio-economic factors related with the subjective well-being of the rural elderly people living independently in China.

BACKGROUND: Many Chinese elderly increasingly face the serious problem of the "empty nest" phenomenon. The elderly living independently, also called empty-nest elderly, refers to elderly people living alone whose children left home. However few studies concerned about the subjective well-being (SWB) of the elderly living independently. METHODS: This study employs The Memorial University of Newfoundland Scale of Happiness (MUNSH) to explore the SWB of the elderly living independently in rural areas of Wenzhou, a relatively developed region in China. 536 sampled are randomly selected. RESULTS:

Food access and diet quality independently predict nutritional status among people living with HIV in Uganda.

OBJECTIVE: Although undernutrition is recognized as a risk factor for mortality among people living with HIV (PLWHIV), even among those initiating antiretroviral therapy, few studies have explored the underlying determinants of undernutrition. The objectives of the present study were to: (i) examine the independent association between household food security, individual diet quality and nutritional status; and (ii) determine if any association between food security and nutritional status is mediated through diet quality. DESIGN: Cross-sectional baseline survey. SETTING: Gulu and Soroti distr

Independence in Activities of Daily Living Among Autistic Toddlers: A Pilot Study Using Ecological Momentary Assessment.

Background: The acquisition of adaptive skills is critical for independence and participation in activities of daily living (ADL). While caregiver perceptions provide valuable insights, most studies on autistic participation have focused on older children and relied on one-time clinic-based assessments. As a result, little is known about how autistic toddlers function in their natural environments across time. Ecological momentary assessment (EMA) is a real-time, context-sensitive method in which parents can report behaviors at multiple time points in the child's natural environment. This pilo

Impact of gait problems and falls on functioning in independent living persons of 55 years and over: a community survey.

In a cross-sectional, population-based study among community-dwelling persons of 55 years and over the incidence of falls, risk indicators for falls, specifically age, and the impact of gait problems, falls and other risk factors on functioning was determined. A randomly age-stratified sample (n = 655) was taken from all independent living persons of 55 years and over (n = 2269) and registered in a primary health care centre. They received a mail questionnaire concerning demographic data, history of falls and injuries due to falls, physical and mental health status, gait problems, functional s

The "ELDERDES" model: how older adults' adoption of digital environment solutions improves residential experiences, health outcomes, and enables independent living.

Global increases in longevity have led to a rapidly growing population of older adults entering advanced old age, facing increased risks of chronic illness, mobility limitations, sensory impairments, cognitive decline, poor emotional health, and social losses. These vulnerabilities make it more difficult for them to maintain independent living arrangements and to age optimally in their current homes, neighborhoods, and communities. Routine tasks like climbing stairs, reaching high shelves, or avoiding slippery floors become unsafe. Older people struggle to reach inhospitable community destinat

Worsening renal function during hospitalization in elderly patients with heart failure: an independent factor of activities of daily living decline.

This study aimed to clarify the effects of worsening renal function (WRF) during hospitalization on activities of daily living (ADL) at discharge of elderly heart failure (HF) patients. We included 323 consecutive patients hospitalized for HF who were prescribed phase I cardiac rehabilitation (CR) from November 2017 to April 2019. WRF was defined as a relative increase from baseline in serum creatinine of 25% or that in serum creatinine ≥ 0.3 mg/dL during hospitalization. The indices of ADL and physical function were the functional independence measure (FIM), short physical performance battery

Providing instruction in independent living and vocational rehabilitation for individuals with head injuries within an entry-level occupational therapy curriculum.

With the goal of occupational therapy being to assist persons in living independently, few occupational therapists (OTs) work in independent living and vocational settings or are heavily engaged in independent living and vocational interventions. Ultimately, clients suffer when attempting to reintegrate into the community without adequate resources, especially persons with head injuries who present with numerous disabilities. Perhaps OTs are not well-trained to provide these services. This paper describes the incorporation of information about service provision within the independent living an
