Renal function in sick, very low-birth-weight infants.

Renal immaturity is pronounced in very low-birth-weight infants with a gestational age < or = 30 weeks. We attempted to elucidate if conditions requiring mechanical ventilation, including patent ductus arteriosus, might further compromise renal function due to decreased renal perfusion. Forty infants studied between 4 and 28 days of age were divided into four groups: Control with no patent ductus or mechanical ventilation (n = 8); PDA+MV, with both patent ductus and mechanical ventilation (n = 17); PDA, with patent ductus (n = 6); MV, with mechanical ventilation (n = 9). The groups PDA+MV and

Perinatal hip assessment in very low birth weight infants.

Fifty healthy low birth weight infants (less than 1500 g) without risk factors for congenital hip dysplasia (CHD) were examined by ultrasonography (100 hips) at 40 weeks corrected age and controlled at 3 months postnatal age. Sonographic assessment was performed using the classification of Graf. A physiological delay of hip maturation was observed in 10% of the patients (type IIa). At follow-up all of them had normal hip morphology without any pathological finding. Our results suggest that prematurity alone should not be considered a risk factor for CHD.

Nosocomial bacterial infections in very low birth weight infants.

The occurrence of congenital and nosocomial bacterial septicaemia has been documented by identifying the number of positive blood cultures by reviewing the laboratory and clinical records of 394 very low birth weight infants who were consecutively admitted to a neonatal intensive care unit over a 40-month period. The incidence of congenital septicaemia was 6% and of nosocomial septicaemia 17%. The commonest causes of congenital infection were Streptococcus agalactiae Staphylococcus epidermidis and Enterococcus faecalis (each in 18% of cases). The commonest cause of nosocomial infection was S.

Nutrition and growth analysis of very low birth weight infants.

The growth and nutrition of 220 very low birth weight infants were reviewed after comprehensive data on all infants in the hospital were entered into the Neonatal Intensive Care Unit Audit Data Base for 2 years prospectively. Fluid and energy (parenteral and oral) intakes were compared in four birth weight categories (1, less than or equal to 750 g; 2, 751 to 1000 g; 3, 1001 to 1250 g; 4, 1251 to 1500 g). Parenteral nutrition was the major source of first nutrition for the small infants, but seldom did it alone provide adequate nutrition for very low birth weight infants. The age of the first

Indomethacin-associated sepsis in very-low-birth-weight infants.

Indomethacin sodium promotes closure of the patent ductus arteriosus in premature infants. In addition to renal and gastrointestinal side effects, indomethacin may predispose to infection because of inhibition of polymorphonuclear leukocyte (PMN) function. We retrospectively assessed the incidence of sepsis in a group of 58 premature infants with patent ductus arteriosus who received either oral indomethacin, surgery, or usual medical management. A significant increase in the incidence of sepsis was observed in the indomethacin-treated group compared with patients treated with surgery or usual

Subcapsular hematoma of the liver in infants of very low birth weight.

Between January 1982 and May 1986 a large subcapsular hemorrhage of the liver (SHL) was diagnosed in six infants who weighed 1000 g or less at birth at Royal Alexandria Hospitals, Edmonton. The diagnosis of a ruptured SHL was made between 4 and 18 days of life by means of clinical and sonographic findings in four of the infants; an intact SHL was diagnosed at autopsy in the other two. None of the cases was associated with parenchymal rupture of the liver. Thrombocytopenia was present in five of the six infants and in all four infants with hemoperitoneum. Other possibly relevant antecedent even

Severe pre-eclampsia and infants of very low birth weight.

The effect of severe pre-eclampsia on the outcome of infants of very low birth weight was studied in a prospective case control study of 35 pairs of infants of comparable gestation. Significantly more infants were delivered before the onset of labour and by caesarean section in the group with pre-eclampsia. These babies tended to be smaller and had a higher incidence of hyaline membrane disease, patent ductus arteriosus, pulmonary air leak, and hypotension. They also required more intensive treatment with oxygen and mechanical ventilation. The significant difference in birth weight was still a

Low birth weight, very low birth weight rates and gestational age-specific birth weight distribution of korean newborn infants.

To obtain the low birth weight (LBW) rate, the very low birth weight (VLBW) rate, and gestational age (GA)-specific birth weight distribution based on a large population in Korea, we collected and analyzed the birth data of 108,486 live births with GA greater than 23 weeks for 1 yr from 1 January to 31 December 2001, from 75 hospitals and clinics located in Korea. These data included birth weight, GA, gender of the infants, delivery type, maternal age, and the presence of multiple pregnancy. The mean birth weight and GA of a crude population are 3,188 +/-518 g and 38.7+/-2.1 weeks, respectivel

The effect of birth hospital type on the outcome of very low birth weight infants.

OBJECTIVE: To test the hypothesis that the likelihood of death or major morbidity is reduced for very low birth weight (VLBW; <1500 g) infants who are born at hospitals with subspecialty perinatal and neonatal care compared with other available birth sites. METHODS: A population-based cohort study was conducted of all live births of 500 to 1499 g at the 19 hospitals in the greater Cincinnati region from September 1, 1995, through December 31, 1997 (N = 848). Primary outcome was the risk-adjusted, predischarge mortality or morbidity, including bronchopulmonary dysplasia, severe intracranial he

Radiation exposure in 212 very low and extremely low birth weight infants.

OBJECTIVE: We determined the frequency and estimated effective radiation dose (E) from conventional diagnostic radiographs for infants who had birth weight of <or=1500 g (very low birth weight [VLBW] infants) and were treated in a NICU. METHODS: Entrance skin doses were experimentally measured for all standard weight-dependent exposure settings. For each radiograph in the radiologic file, the exposed area on the film was measured manually. Together with clinical data obtained from the Vermont Oxford Network, medical charts, and radiologic files, we estimated E. E values per radiograph and per

Anaesthetic considerations for the management of very low and extremely low birth weight infants.

The opportunities for very low birth weight infants (birth weight < 1500 g) and extremely low birth weight infants (birth weight < 1000 g) to undergo surgery are increasing. These infants are prone to prematurity-related morbidities including respiratory distress syndrome, intraventricular haemorrhage, periventricular leukomalacia, retinopathy of prematurity, patent ductus arteriosus and necrotising enterocolitis. Evidence is accumulating that preterm infants are also sensitive to pain and stress. The pharmacokinetics of drugs in preterm infants is not fully understood but smaller doses of ana

Sutureless Loop Enterostomy in Very Low and Extremely Low Birth Weight Infants.

Introduction Sutureless enterostomy is used as an effective technique for constructing an enterostomy in very low and extremely low birth weight infants in Japan. Sutureless enterostomy is a separate type of enterostomy procedure for low birth weight infants. We adapted this technique and developed sutureless loop enterostomy (SLE), an approach without a skin bridge in which the intestinal wall is not sutured to the abdominal wall. This study aimed to compare SLE outcomes with those of sutureless enterostomy (SE) and the conventional procedure (C). Methods We retrospectively reviewed clinical

[Metabolic therapy of premature infants with very low and extremely low birth weight].

The efficiency of a metabolic therapy for premature newborns has been assessed. It is established that early metabolic therapy leads to improvement of tissue perfusion, normalization of and alkalipenia indices, and rapid decrease in requirement for high oxygen concentration in carrying out respiratory support, thus producing fast cardiocytoprotective effect.

Taiwanese parents' perceptions of their very low-birth-weight infant with developmental disabilities.

This study explores the perceptions and experiences of Taiwanese parents in coping with the unfolding evidence of a disability, their response to the official diagnosis, and their views about their child's developmental disability. This descriptive qualitative study is a partial analysis of data from a larger study including 19 Taiwanese parents of very low-birth-weight infants with developmental disability at 6 and 12 months of corrected age. Four themes were generated: uncertainty and worry about developmental progress, search for meaning and supernatural will, desire for normality and attit

[Iatrogenic pleuropneumonia complicating central venous cannulation in a very low birth weight infant].

BACKGROUND: Central venous cannulation is necessary for long-term parenteral nutrition in premature infants. Peripherally inserted long catheters are commonly used in these patients but even this relatively simple technique can end in serious complications. We present a case in which perforation of the vena cava and migration of the catheter to the intrapleural space resulted in multiple organ failure and death. CASE REPORT: A 700 g bw. infant, born at 28 weeks of gestation, was referred to our centre because of suspected bowel perforation. In the referring hospital, the infant had a central

Transient leukoerythroblastosis in a very low birth weight infant with parvovirus B19 infection.

BACKGROUND: Leukoerythroblastosis is characterized by the presence of leukocytosis and erythroid and myeloid blast cells in the peripheral blood. The most common etiological factors of leukoerythroblastosis occurring during early childhood are viral infections, juvenile myelomonocytic leukemia, and osteopetrosis. To our knowledge, an association with parvovirus B19 infection has only been reported in a preterm infant. Human parvovirus B19 has been associated with red cell aplasia, leukopenia, and thrombocytopenia. CASE REPORT: The case of a very low birth weight preterm infant with transient
