Extra-uterine placental transfusion and intact-cord stabilisation of infants in caesarean sections: an intervention development and pilot-study (INTACT-1).

BACKGROUND: Keeping the umbilical cord intact the first minutes after delivery is beneficial for both term and preterm infants. However, this may be challenging in caesarean sections (CS) due to lack of mobile resuscitation equipment, maintenance of sterility or concern for excessive maternal blood loss. The objective of this study was to develop and pilot-test extra-uterine placental transfusion and intact-cord stabilisation of infants in CS. METHODS: The intervention development process (phase 1) covered: (A) placenta delivery without cord clamping, (B) intact-cord stabilisation of the infa

Can We Stop the Current Epidemic of Blindness From Retinopathy of Prematurity?

Retinopathy of Prematurity (ROP) is a retinal vascular disease of the premature infant. There have been significant advances in the understanding of the pathogenesis, prevention, detection and treatment of severe ROP. In developed countries, all premature infants at risk for ROP are screened routinely based on national guidelines and treatment is prompt, resulting in good visual outcomes. Blindness from ROP is a rare event in these countries. However, there is an ongoing epidemic of ROP- induced blindness in India. The reasons for this epidemic of ROP blindness are many and include increasing

Transabdominal Ultrasound-Guided Transcervical Submucosal Uterine Leiomyoma Resection: A Case Report.

Hysteroscopic resection is considered the gold standard treatment for the removal of submucous uterine leiomyoma (SMLs). However, when hysteroscopic equipment is unavailable, alternative treatment approaches must be explored. In this case report, we present a video demonstrating the transcervical resection of an SML performed under transabdominal ultrasonography guidance on a patient with persistent abnormal uterine bleeding. A 37 mm Type 0 leiomyoma was successfully removed in 9 min without any complications. Six months postoperatively, the patient conceived spontaneously and delivered a heal

Preparing families to care for ventilated infants at home.

Advances in neonatal care have led to increased survival of infants with complex medical needs and technology dependence. Transition of the ventilator-dependent infant from hospital to home is a complex process that requires extensive coordination between the medical team and family. Home caregivers must be prepared to provide routine care for the ventilator-dependent child and respond to life-threatening emergencies. Families should be counseled on the need for home nursing, medical equipment and an adequate home environment to ensure a safe transition to home. Throughout the process, the fam

Advances in craniofacial surgery.

The past 10 years have witnessed many advances in craniofacial surgery. Advances in surgical techniques, such as distraction osteogenesis and endoscopic procedures, combined with refinements in surgical equipment, such as resorbable plating and distractors, have improved surgical outcomes, while minimizing morbidity. Technological advances in 3-dimensional imaging, computer simulation, and intraoperative navigation facilitate diagnosis, preoperative planning, and surgical execution. Rising cases of deformational plagiocephaly owing to increased supine infant sleep positioning necessitated the

Reducing blood donor exposures in low birth weight infants by the use of older, unwashed packed red blood cells.

We performed a prospective, randomized masked trial to determine whether the use of dedicated units of packed red blood cells equipped with seven satellite bags would reduce donor exposures in infants with birth weights < 1500 gm. We also examined the use of unwashed and older red blood cells. Red blood cells given to the study group were used without washing and until their expiration date (35 to 42 days). Changes in blood pH, potassium, ionized calcium, and hemoglobin were determined with each transfusion and compared with data collected from a control group that received washed, younger red

Retinopathy of Prematurity (ROP): An Overview of Biomarkers in Various Samples for Prediction, Diagnosis, and Prognosis.

Retinopathy of Prematurity (ROP) is a proliferative retinal vascular disease marked by abnormal development of retinal vessels in low birth weight preterm infants. It is one of the leading causes of blindness in preterm infants. Current ROP screening methods impose high demands on both the equipment and the expertise of ophthalmologists, which limits their widespread application, particularly in secondary hospitals and remote areas. Thus, the identification of relevant biomarkers and the development of simpler detection methods are important and promising. Non-invasive or minimally invasive sa

Sustaining Breastmilk Expression and Direct Feeding at Breast for Very Low Birth Infants: A Qualitative Exploration of Parental Perspectives.

In Ireland, while most women express milk for their preterm very low birthweight (VLBW) infants, only a few are discharged home exclusively receiving mothers' own milk, and even fewer are engaged in direct feeding at breast (DFAB). This is in the context of Ireland having one of the lowest breastfeeding initiation and sustenance rates in the world. This qualitative descriptive study aimed to explore parents' experiences and perspectives on sustaining breastmilk expression and DFAB for VLBW infants. Semi-structured interviews were conducted with nine mothers and three partners, and the data wer

Genotyping and Source Tracking of Cronobacter sakazakii and C. malonaticus Isolates from Powdered Infant Formula and an Infant Formula Production Factory in China.

Cronobacter spp. (formerly defined as Enterobacter sakazakii) are opportunistic bacterial pathogens of both infants and adults. In this study, we analyzed 70 Cronobacter isolates from powdered infant formula (PIF) and an infant formula production facility in China to determine possible contamination routes. The strains were profiled by multilocus sequence typing (MLST), pulsed-field gel electrophoresis (PFGE), PCR-based O-antigen serotyping, and ompA and rpoB sequence analyses. The isolates were primarily Cronobacter sakazakii (66/70) or Cronobacter malonaticus (4/70). The strains were divided

Accuracy of ultrasonic detection of the uterus in normal newborn infants: implications for infants with ambiguous genitalia.

Every year approximately one in 5000 babies is born with ambiguous genitalia, a condition that requires immediate sex assignment if possible. We investigated whether transabdominal ultrasonography with the use of a linear array high-resolution 7.5-MHz transducer would enhance the accuracy of detecting the uterus in the newborn. One hundred mothers who delivered normal, term infants (> 36 weeks and > 2500 g) gave their informed consent for the investigation. Each infant was placed in an unmarked bassinet, external genitalia were covered, and ultrasound examinations were performed by the same ex

Pumping and hygiene practices are associated with bacterial load and microbial composition in human milk expressed at home.

BACKGROUND: Human milk (HM) harbors a unique microbiome that contributes to the development of the infant gastrointestinal microbiome and influences long-term health outcomes. While pumping and bottle-feeding HM are increasingly common, doing so may introduce exogenous bacteria, altering this microbial community. The extent to which real-world pumping and hygiene practices alter the HM microbial community remains inadequately characterized. METHODS: We conducted a secondary analysis of 104 paired milk samples from 52 healthy women to investigate the associations between at-home pump hygiene p

Integrating Neonatal Intensive Care Into a Family Birth Center: Describing the Integrated NICU (I-NIC).

Parent-infant separation resulting from admission to a neonatal intensive care unit (NICU) is often reported as the most challenging and distressing experience for parents. Aiming to mitigate the stress of parent-infant separation, a new neonatal care model was designed to integrate NIC with delivery and postpartum care. Yet, little is known about the model and its implementation. Therefore, using a qualitative descriptive design with field observations, we describe the characteristics of an integrated neonatal intensive care (I-NIC) model and examined perceptions of clinical staff ( n = 8) an

Delivery room emergencies: Respiratory emergencies in the DR.

The majority of newborns transition to extra uterine life without support. However, respiratory emergencies in the delivery room are a common occurrence. Whilst some situations are predictable e.g. the anticipated birth of an extremely preterm infant, others are less so. In this chapter we address the most frequent scenarios that result in delivery room respiratory emergencies and discuss the latest recommendations for their management. We outline the need for a trained resuscitation team and appropriate equipment to provide respiratory support at every birth. We address the basic care that al

Influences of Cheek Support on Sucking Pressure During Bottle Feeding in Infants: A Pilot Study.

Effective sucking is important for shortening hospital stays in newborn infants. Several studies have demonstrated that oral support can improve sucking efficiency. However, the effects of oral support on sucking pressure have not been thoroughly investigated. In our previous study, we reported the equipment configuration and analysis method of sucking pressure. That method of measuring sucking pressure was applied to cheek support. In this study, four newborn infants with low birth weight, congenital diaphragmatic hernia, or multicystic encephalomalacia were enrolled. Their sucking pressures
