Importance: Despite increased initiatives and funding to improve access to evidence-based treatments for opioid use disorder (OUD), including medications for OUD (mOUD), pregnant/postpartum individuals have significant obstacles to accessing these life-saving medications. Observations: Current legislation, specifically the Comprehensive Addiction and Recovery Act (CARA), mandates that the Governor of each state has systems in place to identify and address the needs of substance-exposed infants. However, this legislation removed the word "illegal" when defining substance use and left other impo

BACKGROUND: Multiple health professional associations have expressed concern with policies that require clinician reporting of pregnant people's substance use to child welfare, including that reporting negatively affects patient outcomes and the patient-provider relationship. However, research has shown that clinicians continue to report pregnant and birthing patients at high rates.
OBJECTIVE: This study aimed to explore clinician views on reporting pregnant and birthing patients who use alcohol or drugs during pregnancy to child welfare and whether there are patterns in the types of decision

Maternal health in low-income countries has received increasing attention over the last 15-20 years. Maternal mortality ratios in these countries are mainly still modelled estimates and one cannot discern trends. The introduction of registration systems-giving reliable causes of death-is essential both for monitoring maternal health and fuelling action. Countries with documented success in reducing maternal mortality have used systematic, incremental approaches, often tied to multi-sectoral efforts, including roads, communication links, education, water and sanitation. Improving maternal healt

All patients <13 years of age referred to the open-access paediatric echocardiography clinic at the Sultan Qaboos University Hospital, Muscat, Oman, during the five years from 1994 to 1998 were analysed. Among the 2633 patients studied, 1543 (58.6%) were normal, 845 (32.1%) had congenital heart disease, and 245 (9.3%) had acquired heart disease. The major congenital heart diseases identified were secundum atrial septal defect (22.5%), ventricular septal defect (22.5%), patent ductus arteriosus (15.7%), mitral valve prolapse (10.7%), pulmonary stenosis (9.7%) and atrioventricular septal defect

UNLABELLED: The French decision-making processes for recommendation and introduction of infant vaccination with Prevenar reflect a public health-based dialogue between the Direction Générale de la Santé and Wyeth (Paris-La Défense, France) nurtured by open and ongoing exchanges. Three surveillance programmes are being discussed: (1) to ascertain the future impact of large-scale Prevenar vaccination on invasive pneumococcal disease incidence, (2) to follow the evolution and carriage of pneumococcus and (3) to look at the possibilities to establish an epidemiological surveillance and active "vac

Congenital toxoplasmosis is caused by Toxoplasma gondii. The incidence of death due to congenital toxoplasmosis in Japan from 1974 to 2007 was calculated using the autopsy database of the Japanese Society of Pathology and vital statistics from the Ministry of Health, Labour and Welfare. Two neonatal deaths due to congenital toxoplasmosis were reported during that time. As there were 161,195 neonatal deaths during this period and 32,465 autopsies were performed, the yearly neonatal death from congenital toxoplasmosis was calculated as 2 × 161,195/32,465/34 = 0.29 and the autopsy rate as 32,465/

Treating patients who physically abuse their children creates in the therapist moral and ethical dilemmas which challenge his or her ability to maintain a professional conduct and attitude. In such cases the therapist usually finds himself or herself confronted with the following dilemmas: (a) treatment of a patient who experiences therapy as a no-choice situation; (b) treatment of a patient whose conduct is in conflict with the moral values of the therapist; (c) the obligation of the therapist to report the abusive acts to the authorities versus his or her duty to protect the patient's right

OBJECTIVES: To investigate whether adding an artificial intelligence-assisted hip ultrasound (CHC-US) to the selective screening for developmental dysplasia of the hip (DDH) at Child Health Care (CHC) centers can reduce the number of referrals for hip ultrasound without an increase of missed cases.
METHODS: We conducted a cross-sectional diagnostic study at four CHC centers in the Netherlands between May 2022 and December 2022. All participating infants received both a CHC-US and a Hospital-US. The results of both ultrasounds (US) were analysed to evaluate the diagnostic performance of the CH

AIM: The primary aim was to investigate the incidence, birth characteristics, and timing of diagnosis of cephalhaematoma (CH). The secondary aim was to explore associations between CH and complications during the neonatal period and infancy.
METHODS: This is a nationwide population-based register study of infants (< 12 months) born in Sweden from 1997 to 2018. Information was retrieved from health registers of the Swedish National Board of Health and Welfare. Cases were infants with a diagnosis of birth-related CH.
RESULTS: The study comprises 2930 infants with CH. The incidence was 2.2 per

Although asthma is probably a heterogeneous disease or syndrome, three factors and/or events consistently emerge for their ability to significantly influence asthma inception in the first decade of life: immune response aberrations, which appear to be defined best by the concept of cytokine dysregulation; lower respiratory tract infections, in particular respiratory syncytial virus (RSV); and some form of gene-environment interaction that needs to occur at a critical time-period in the development of the immune system or the lung. It remains to be firmly established, however, how any one or al

Turkey has a young population as a result of high fertility and growth rates in the recent past. Thirty-five percent of the population is less than 15 years of age, and 25 percent of the population comprises reproductive-age woman. The latest estimate of the population growth rate was 19 per thousand for the 1990-1995 period. In recent decades dramatic declines in fertility rates have been noted. In the early 1970s, the overall fertility rate was approximately five children per woman, declining to 2.7 in 1993. The crude birth rate is currently estimated to be approximately 23 per thousand. BACKGROUND: Gestational diabetes mellitus (GDM) is a form of diabetes that occurs in pregnancy. Although GDM usually resolves following birth, it is associated with significant morbidities for mothers and their infants in the short and long term. There is strong evidence to support treatment for GDM. However, there is uncertainty as to whether or not screening all pregnant women for GDM will improve maternal and infant health and if so, the most appropriate setting for screening. This review updates a Cochrane Review, first published in 2010, and subsequently updated in 2014.

Congenital heart block (CHB), defined as an atrioventricular block diagnosed in utero, at birth, or within the neonatal period (0-27 d after birth), is a rare disorder closely linked to transplacental transport of maternal antibodies anti-Ro/SSA and anti-La/SSB. These antibodies may induce a myocarditis, or interact directly with calcium channel proteins with disturbance of transmembrane signaling at the level of the conduction tissue, or interfere with apoptosis. Depending on the severity of the process, the fetus may die in utero or a few days after birth or survive to the perinatal period a

BACKGROUND: Stunting is a major public health problem in low- and middle-income countries as in Democratic Republic of the Congo (DRC). Many factors, such as environmental enteric dysfunction (EED), are incriminated in the pathogenesis of stunting. EED, characterized by intestinal inflammation, can be evaluated by a non-invasive marker, the fecal calprotectin (FC). The aim of this study was to determine the concentration of FC, to compare the FC of exclusively breastfed infants with that of mixed-fed infants, and to assess the association between the stunting and the FC in infants aged 4 to 7

In recent years, the need for infant organs for transplantation has increased. There is a growing recognition of the potential use of anencephalics as sources of organs. Prevalent arguments defending the use of live anencephalics for organ sources are identified and criticized. I argue that attempts to deny the applicability of the "dead-donor rule" are either question-begging or based on false premises and that attempts to skirt the Kantian dictum against treating others as a means only are not successful. I contend that the apparent utilitarian justification for live anencephalics as organ s

The high societal and personal costs of child maltreatment make identification of effective early prevention programs a high research priority. Early Head Start (EHS), a dual generational program serving low-income families with children prenatally through age three years, is one of the largest federally funded programs for infants and toddlers in the United States. A national randomized trial found EHS to be effective in improving parent and child outcomes, but its effectiveness in reducing child maltreatment was not assessed. The current study used administrative data from state child welfar

Viral lower respiratory tract illnesses occurring during the first years of life are associated with increased risk of subsequent asthma, but the mechanisms involved have not been completely elucidated. The available evidence suggests that the factors that explain this connection are heterogeneous. Children who start life with lower levels of airway function appear to be more prone to transient forms of wheezing in the first years of life. "Intrinsic" bronchial hyperresponsiveness, that is, that measured shortly after birth and unrelated to markers of atopy, has been reported to predict both e

AIM: This population-based study assessed the incidence of rickets in infants up to age of one born in Sweden from 1997 to 2014. We also examined maternal and perinatal factors and co-morbidity.
METHODS: We used Swedish National Board of Health and Welfare registers and data from Statistics Sweden. The outcome measure was an International Classification of Diseases, Tenth Revision, code for rickets.
RESULTS: There were 273 cases of rickets, with an incidence of 14.7 per 100 000 and a 10-fold incidence increase between 1997 and 2014. The majority (78.4%) were born preterm, half were small-for
