Goat-associated Q fever: a new disease in Newfoundland.

In the spring of 1999 in rural Newfoundland, abortions in goats were associated with illness in goat workers. An epidemiologic investigation and a serologic survey were conducted in April 1999 to determine the number of infections, nature of illness, and risk factors for infection. Thirty-seven percent of the outbreak cohort had antibody titers to phase II Coxiella burnetii antigen >1:64, suggesting recent infection. The predominant clinical manifestation of Q fever was an acute febrile illness. Independent risk factors for infection included contact with goat placenta, smoking tobacco, and ea

From Qi Flow to Air Leak: Bilateral Apical Pneumothoraces After Acupuncture.

Acupuncture is a popular form of traditional Chinese medicine used for management of pain, stress, and other chronic conditions. In recent years, it has been increasingly incorporated as an adjunctive therapy in Western medicine for acute and chronic pain control. While it is usually safe when performed by trained practitioners, complications including bleeding, infection, and injury to surrounding structures near acupoints can occur. We herein report a case of a patient who developed bilateral small apical pneumothoraces following acupuncture at thoracic acupoints, which was managed conservat

Intestinal Carriage of Carbapenemase-Producing Organisms: Current Status of Surveillance Methods.

Carbapenemases have become a significant mechanism for broad-spectrum β-lactam resistance in Enterobacteriaceae and other Gram-negative bacteria such as Pseudomonas and Acinetobacter spp. Intestinal carriage of carbapenemase-producing organisms (CPOs) is an important source of transmission. Isolation of carriers is one strategy that can be used to limit the spread of these bacteria. In this review, we critically examine the clinical performance, advantages, and disadvantages of methods available for the detection of intestinal carriage of CPOs. Culture-based methods (Centers for Disease Contro

Does physician bias affect the quality of care they deliver? Evidence in the care of sexually transmitted infections.

BACKGROUND: Primary care providers are well placed to control the spread of sexually transmitted infections (STI); however, care is likely to be influenced by their attitudes and beliefs. The present study investigates the relationship between general practitioner's (GP) self-reported level of comfort in dealing with patients with STI and the care they deliver. METHODS: A postal survey was conducted using a stratified random sample of 15% of GPs practising in New South Wales, Australia, to assess practitioners' management of STI. A total of 409 GPs participated in the study yielding a respons
