Single-Round Infectious Particle Antiviral Screening Assays for the Japanese Encephalitis Virus.

Japanese Encephalitis virus (JEV) is a mosquito-borne flavivirus with a positive-sense single-stranded RNA genome that contains a big open reading frame (ORF) flanked by 5'- and 3'- untranslated regions (UTRs). Nearly 30,000 JE cases with 10,000 deaths are still annually reported in East Asia. Although the JEV genotype III vaccine has been licensed, it elicits a lower protection against other genotypes. Moreover, no effective treatment for a JE case is developed. This study constructed a pBR322-based and cytomegaloviruses (CMV) promoter-driven JEV replicon for the production of JEV single-roun

[Encephalopathy associated with severe bacterial infections: "presuppurative encephalitis" or "toxi-infectious disorder"?].

Three children are reported, exhibiting encephalopathic symptoms in the course of a severe bacterial infection, though they had no sign of intracranial purulent collection nor metabolic disturbances: the first was associated with a appendicular peritonitis, the second with a purulent knee arthritis, the third with a pneumoccus septicemia. The elements are reviewed that favor either a true septic encephalitis, i.e. microscopic presuppurative foci, versus a toxiinfectious process, either by direct action of toxins or through intracerebral activation of pre-inflammatory cytokines. Whatever the me

[Para-infectious aseptic meningo-encephalitis following SARS-CoV-2 infection: two case reports].

Neurological manifestations following COVID-19 have been increasingly reported, yet the pathophysiology of para-infectious processes remains unclear. We present two patients who developed headaches, gait instability, cognitive impairments, and inflammatory cerebrospinal fluid (CSF) changes after SARS-CoV-2 infection. Imaging revealed meningitis and encephalitis, respectively, but there was no direct pathogen detection. After excluding other causes and initiating corticosteroid therapy, both patients showed significant improvement. These cases highlight the relevance of autoimmune mechanisms in

Immune checkpoint inhibitor-induced encephalitis in endometrial carcinoma: Distinguishing immune from infectious causes.

BACKGROUND: Immune checkpoint inhibitors (ICIs) have expanded therapeutic options for advanced gynecologic cancers but can occasionally cause neurologic immune-mediated adverse events such as encephalitis. These presentations can overlap with infectious or metabolic conditions necessitating timely recognition. CASE: A 68-year-old woman with stage IIIC1 endometrioid endometrial carcinoma developed progressive encephalopathy 15 days after her fourth cycle of pembrolizumab-based therapy. Initial findings of fever, leukocytosis, and abnormal urinalysis supported urosepsis, and a positive West Nil

Japanese Encephalitis Virus Genotype 5 Infectious Clone and Reporter System for Antiviral Evaluation.

The recent emergence of Japanese encephalitis virus genotype 5 (JEV5) has raised concerns regarding limited vaccine efficacy. Here, we report the construction of a full-length infectious cDNA clone of JEV5 using a bacterial artificial chromosome (BAC) system, based on a South Korea isolate, K15P38 strain. The resulting clone enabled recovery of replication-competent virus and was genetically distinguishable from the parental strain via engineered silent mutations. To extend its application, we introduced nanoluciferase and enhanced green fluorescent protein into the viral genome, generating re

Epidemiology of infectious encephalitis, differences between a prospective study and hospital discharge data.

The French epidemiology of infectious encephalitis has been described in a 2007 prospective study. We compared these results with available data (demographic features, causative agents, case-fatality ratio) obtained through the French national hospital discharge 2007 database (PMSI), in order to evaluate it as a surveillance tool for encephalitis. Causative agents were identified in 52% of cases in the study, and 38% in PMSI (P < 0·001). The incidence of encephalitis in France in 2007 was estimated as 2·6 cases/100 000 inhabitants. HSV and VZV were the most frequent aetiological agents in both

Infectious transcripts of tick-borne encephalitis virus, generated in days by RT-PCR.

Construction of infectious clones of flaviviruses can be problematic owing to instability, toxicity, and recombination events occurring while cloning cDNA in the bacterial vectors. To overcome these difficulties we have devised a rapid and simple method for producing an infectious genetically engineered tick-borne encephalitis virus in less than 10 days using viral RNA from an unpurified virus suspension. The experimental protocol utilized the high fidelity reverse transcription-polymerase chain reaction to produce two long (5.7 and 5.2 kb) overlapping cDNA segments. To produce full-length cDN

Serial magnetic resonance imaging in post-infectious focal encephalitis due to influenza virus.

Two cases of a 13-year-old girl and a 14-year-old boy with postinfectious focal encephalitis due to influenza are reported. The clinical and magnetic resonance imaging (MRI) findings included: (1) partial motor seizures as the initial central nervous system manifestation, appearing more than 20 days after the influenzal infection, (2) no change in the level of consciousness although a boy demonstrated apraxia, and (3) high signal intensity lesions noticed with T2-weighted MRI located mainly in the cortex. The girl's lesion appeared to resolve within 10 days on MRI, while that of the boy (demon

Diagnostic Yield of Anti-Neuronal Antibody Testing in Patients Suspected of an Infectious Encephalitis.

BACKGROUND AND OBJECTIVES: Recognition of autoimmune encephalitis (AIE) can be difficult as typical radiological or cerebrospinal fluid abnormalities may be lacking. We investigated the yield of comprehensive diagnostic testing for anti-neuronal antibodies in patients suspected of encephalitis in an acute setting. METHODS: In a prospective multicenter cohort, we included patients suspected of encephalitis in whom a lumbar puncture was performed. We retrospectively selected patients from this cohort in whom no infectious cause was identified and an autoimmune CNS disease was considered. Immuno

Autoimmune and infectious encephalitis: development of a discriminative tool for early diagnosis and initiation of therapy.

BACKGROUND: Encephalitis originates from diverse autoimmune and infectious etiologies. Diagnostic challenges arise due to the spectrum of presentation and the frequent absence of specific biomarkers. This study aimed to comprehensively characterize and differentiate autoimmune encephalitis (AE) from infectious encephalitis (IE) in adults, and disentangle clinical, paraclinical, and therapeutic differences. METHODS: A cohort study spanning 10 years was conducted across three Austrian tertiary care hospitals. Inclusion criteria comprised adults with probable or definite encephalitis. Demographi

Infectious Causes of Acute Encephalitis Syndrome in India - Decadal Change and the Way Forward.

The diagnosis and management of encephalitis were previously largely based on clinical grounds and minimal laboratory investigations. Japanese encephalitis (JE) gets considered as the probable diagnosis in most encephalitis cases. However, reports of JE in adults and the elderly are increasing after the JE vaccine introduction among children in 2006. The Nipah virus (NiV) emerged in 2002 and continues to afflict humans in new geographic areas. Many other infections cause encephalitis, including Chandipura, chikungunya, dengue, and West Nile. Significant advances in diagnostic testing like mult

[An acute meningo-encephalitis with a favourable prognosis and occasional severe electrical and clinical changes; infectious mononucleosis meningo-encephalitis (author's transl)].

Four cases of severe meningo-encephalitis in patients with infectious mononucleosis (IMN) are reported. In three cases the clinical context suggested a mononucleosis. In the other case there was fever but only isolated neurological signs, which demonstrates the importance of serological diagnostic tests because of the benign prognosis of this affection. In fact, neurological complications of IMN are considered to be reversible even though deaths have occurred from respiratory failure, from polyradiculoneuritis, or bulbar involvement. In the cases reported, the onset of a deep coma with loss of
