Biomarkers to inform the management of polycystic ovary syndrome: A review of systematic reviews.

INTRODUCTION: Polycystic ovarian syndrome (PCOS) is the commonest endocrine condition affecting reproductive age women. Many biomarkers may aid assessment and management, however evidence is limited on their utility in clinical practice. We conducted a review of systematic reviews to identify the most useful biomarkers in the clinical management of PCOS. METHODS: We searched MEDLINE, EMBASE, CENTRAL and HTA until August 2023 for reviews evaluating biomarkers in PCOS women compared to healthy controls. Methodological quality was assessed using the AMSTAR2 tool. We reported pooled evidence for

Understanding Behavioural Development of Calves in Natural Settings to Inform Calf Management.

One important type of animal welfare concern is "natural living" (i.e., that animals are able to express natural behaviours that are important to them, and to engage with aspects of the natural world that they find important). The aims of this narrative review were to describe the behavioural development of calves (Bos taurus) in natural settings and use this to identify characteristics of natural systems that may be important to consider relative to this natural living conception of animal welfare. At birth, calves are licked by their mothers and soon stand to suckle for colostrum, and during

Scabies: Application of the Novel Identify-Isolate-Inform Tool for Detection and Management.

Scabies is a highly contagious, globally prevalent, parasitic skin infestation caused by Sarcoptes scabiei var. hominis, also known as the itch mite. There have been outbreaks not only in the developing world, but also in the developed world among refugees and asylum seekers. Once infested with scabies mites, symptomatic patients, as well as asymptomatic carriers, quickly spread the disease through direct skin-to-skin contact. Typically, symptoms of scabies are characterized by an erythematous, papular, pruritic rash associated with burrows. Treatment of scabies involves using topical or syste

Is There a Role for Informal Caregivers in the Management of Diabetic Foot Ulcers? A Narrative Review.

Successful management of diabetic foot ulceration (DFU) is crucial for preventing long-term morbidity and lowering risk of amputations. This can be achieved with a multifaceted approach involving a multidisciplinary team, with the patient at the centre. However, not all healthcare setups enable this, and the rate of lower limb amputations continues to rise. It is therefore time to consider new approaches to diabetic foot care, capitalising on engagement from patients in self-management while supported by their informal caregivers (ICGs) to help improve outcome. The role of ICGs in DFU care has

Informed appropriate imaging for low back pain management: A narrative review.

Most patients with acute low back pain (LBP), with or without radiculopathy, have substantial improvements in pain and function in the first 4 weeks, and they do not require routine imaging. Imaging is considered in those patients who have had up to 6 weeks of medical management and physical therapy that resulted in little or no improvement in their LBP. It is also considered for those patients presenting with suspicion for serious underlying conditions, such as cauda equina syndrome, malignancy, fracture and infection. In western country primary care settings, the prevalence has been suggeste

Pharmacodynamic studies of voriconazole: informing the clinical management of invasive fungal infections.

INTRODUCTION: Voriconazole is a broad-spectrum antifungal agent commonly used to treat invasive fungal infections (IFI), including aspergillosis, candidiasis, Scedosporium infection, and Fusarium infection. IFI often occur in immunocompromised patients, leading to increased morbidity and mortality. AREAS COVERED: The objective of this review is to summarize the pharmacodynamic properties of voriconazole and to provide considerations for potential optimal dosing strategies. Studies have demonstrated superior clinical response when an AUC/MIC >25 or Cmin/MIC >1 is attained in adult patients, co

Managing urinary incontinence at the end of life: an examination of the evidence that informs practice.

To achieve a 'good' death, management of urinary incontinence at the end of life arguably should be as much a priority for nursing attention as managing symptoms such as pain and nausea. To understand how contemporary best practice is described and the nature of interventions prescribed for managing this issue, this article reviews the content of 16 seminal palliative care textbooks and 10 journal articles (retrieved through systematic search techniques) that discuss the management of urinary incontinence for patients at the end of life. The findings depict an area of nursing that has been gov

Miltefosine in the treatment of leishmaniasis: Clinical evidence for informed clinical risk management.

Visceral leishmaniasis (VL) is a life-threatening disease. Traditional treatment with pentavalent antimony injections has become ineffective in the area with the world's highest prevalence of disease (North Bihar, India) and is becoming less effective elsewhere as well. A replacement is needed, best if it can be given to more patients outside the hospital. Miltefosine is the first oral drug registered for VL. Given daily under medical supervision for 4 weeks, it cures 94% of patients (both children and adults) and is reasonably safe. Miltefosine has great potential for improving access to trea

[Surgical management of incidental Meckel's diverticulum: the necessity to obtain the informed consent].

Surgical approach to the incidentally found Meckel's diverticulum (MD) is still source of debate: some support the systematic search and the surgical resection, others suggest to leave in situ the asymptomatic diverticulum. A wide literature review and some claims for malpractice referred to resection of diverticular formations (2 cases are described herein) allow us to deem that the removal of an asymptomatic MD may be an excess of surgical indication regarding an entity that cannot be defined as pathological for itself; anyway, the informed consent must be obtained before the incidental rese

Informed choices for long-term benefits. The management of catheters in continence care.

1. Non-invasive methods of managing urinary dysfunction should be considered as alternatives to long-term catheterisation and reviewed on a regular basis. 2. Nurses must match choice of catheter and drainage equipment with the individual patient, to enhance self-care and independence. 3. Monitoring of catheter life can establish a pattern of recurrent blockage and allow pro-active, planned recatheterisation. 4. Good handwashing practice is important in catheter management to reduce the risk of cross-infection.

Body Composition in Heart Failure: A Phenotype-Informed Framework for Assessment and Management.

Body mass index (BMI) and waist-based anthropometric indices are widely used to estimate adiposity and cardiometabolic risk. Yet in established heart failure (HF), modestly higher BMI is associated with improved survival, while waist-based measures show a linear association with worse outcomes. This apparent discrepancy suggests that the biological interpretation of BMI and body composition changes once HF develops. In HF, BMI reflects the composite of adipose tissue, fluid accumulation, and skeletal muscle wasting, whereas waist-based indices primarily reflect central adiposity. In this narra

Management of tropical urban parks informed by divergent stomatal regulation of trees.

As climate change intensifies urban environmental challenges, cities increasingly rely on urban green spaces as climate adaptation infrastructure. Urban trees provide critical ecosystem services through root water uptake and leaf water loss, yet how their stomata respond to atmospheric drivers in tropical urban environments remains poorly understood. This research examines how ten dominant tree species in Chulalongkorn University Centenary Park, Bangkok regulate stomatal conductance (gs) in response to vapor pressure deficit (VPD) and photosynthetically active radiation (PAR) across wet and dr

Frailty-Informed Pain Management: A Clinical Imperative Beyond Chronological Age.

BACKGROUND: Chronic pain is a prevalent and disabling condition whose management becomes increasingly complex with aging and frailty. While chronological age often guides clinical decisions, frailty offers a more biologically grounded approach. AIMS: We sought to examine the independent associations of chronological age, frailty, and sex with pain management variables in a community-based adult population. METHODS: A cross-sectional study was conducted in 455 adults with chronic non-cancer pain. Frailty was assessed using a 31-item frailty index (FI) on the basis of the deficit accumulation

Salmonella environmental persistence informs management relevant to avian and public health.

Salmonellosis is a significant public health threat responsible for millions of human cases annually but is also of significance to domestic and wild animals around the globe. While human infections are often foodborne, an increasing number of cases arise from environmental sources and contact with animals, including wild birds. Understanding the persistence of Salmonella in shared environments is critical for mitigating zoonotic transmission risks and understanding transmission dynamics for pets and free-living wildlife. Two experiments were conducted to investigate Salmonella persistence on

Ruminating on soil carbon: Applying current understanding to inform grazing management.

Among options for atmospheric CO2 removal, sequestering soil organic carbon (SOC) via improved grazing management is a rare opportunity because it is scalable across millions of globally grazed acres, low cost, and has high technical potential. Decades of scientific research on grazing and SOC has failed to form a cohesive understanding of how grazing management affects SOC stocks and their distribution between particulate (POM) and mineral-associated organic matter (MAOM)-characterized by different formation and stabilization pathways-across different climatic contexts. As we increasingly loo

Pain management in trauma: the need for trauma-informed opioid prescribing guidelines.

BACKGROUND/OBJECTIVES: Surgical populations and particularly injury survivors often present with complex trauma that elevates their risk for prolonged opioid use and misuse. Changes in opioid prescribing guidelines during the past several years have yielded mixed results for pain management after trauma, with a limiting factor being the heterogeneity of clinical populations and treatment needs in individuals receiving opioids. The present analysis illuminates this gap between clinical guidelines and clinical practice through qualitative feedback from hospital trauma providers and unit staff me
