Initial experiences of an in-reach service providing iron infusions in residential aged care facilities.

OBJECTIVE: To examine the feasibility of developing an in-reach parenteral iron infusion service to residents of residential aged care facilities (RACFs). METHODS: An audit comparing the use of iron infusions in RACFs prior to and following the introduction of an in-reach iron infusion service. RESULTS: Of the 738 inpatient iron infusions administered to inpatients ≥65 years in the 12 months prior to the in-reach service, 52 (7%) lived in an RACF, with no significant adverse events reported. After implementation of an in-reach service, a total of 37 RACF residents received parenteral iron in

Influence of Glucose Dosage in Parenteral Nutrition on Body Thiamine Levels in Rats.

The objective of this study was to determine the relationship between glucose dosage in parenteral nutrition and reductions in levels of body thiamine in rats. Vitamin-free infusions with differing amounts of glucose were administered to normal or thiamine-deficient rats for 5 days, after which urinary thiamine excretion and the amounts of thiamine in the blood, liver, brain, and skeletal muscles were measured. The total energy dosage was set at three levels (98, 140, and 196 kcal/kg), and the dose of amino acids was constant among all groups. Urinary thiamine excretions on Day 5 decreased wit

Parenteral morphine and oxycodone mixtures administered in specialised palliative and hospice care units: A retrospective medical record review.

BACKGROUND: Continuous subcutaneous infusions combining several drugs are routinely used in palliative care, particularly for severe pain management. Parenteral administration routes, however, have been found to be prone to errors. According to previous studies, standardising the compositions of parenteral mixtures can improve medication safety. OBJECTIVES: To evaluate the potential for standardisation of parenteral drug mixtures used in palliative and hospice care by identifying the most commonly administered compositions of mixtures containing morphine or oxycodone. DESIGN: A retrospective

Hazards of parenteral treatment: do particles count?

After prolonged parenteral nutrition a 12 month old infant died with pulmonary hypertension and granulomatous pulmonary arteritis. A review of necropsy findings in 41 infants who had been fed parenterally showed that two of these also had pulmonary artery granulomata, while none of 32 control patients who died from sudden infant death syndrome had similar findings. Particulate contaminants have been implicated in the pathogenesis of such lesions and these were quantified in amino acid/dextrose solutions and fat emulsions using automated particle counting and optical microscope counting respect

Modern infusion pumps: are they accurate?

This study examined the volumetric accuracy of six infusion pumps currently available in Canada. Volumes delivered by all pumps over a range of 20-200 ml an hour were within five per cent of their indicated values. The pumps proved to be consistent and unaffected by the height of the infusion bag above the pump or by differing cannulae sizes. A discussion of some of the infusion methods for obtaining clinical accuracy is included.

Administration of peripheral parenteral nutrition: a prospective study comparing rotation of venous access sites with ultrafine cannulas.

Peripheral veins have been used successfully for patients requiring short- to medium-term total parenteral nutrition. This study prospectively compares two methods of peripheral parenteral nutrition (PPN). Forty-six patients requiring parenteral nutrition (PN) were identified prospectively. Fifty courses of PPN were prescribed using a standardized PPN formula of 9.3 g nitrogen, 1400 kCal, 2500 ml (KABI II, Pharmacia). Patients were randomized to receive PPN via 23G, 15 cm flexane catheters (Nutriline) inserted into an antecubital vein which remained in-situ with a continuous infusion over 24 h

Peripheral vein complications in patients receiving amino acid/dextrose solutions.

The incidence and severity of peripheral vein phlebitis was evaluated in patients receiving infusions of amino acids alone (1.4 g/kg/day) (group I), amino acids (1.4 g/kg/day) plus 5% dextrose (group II) and 5% dextrose alone (Group III). Ninety-three patients with carcinoma of the bladder were randomized to receive one of three peripheral parenteral nutrition solutions which were infused predominantly via scalp-vein needles. Peripheral parenteral nutrition solutions contained sodium and potassium chloride as necessary to maintain normal serum electrolyte levels. The osmolality of each solutio

A pilot study of protein sparing in healthy dogs using peripheral parenteral nutrition.

Total parenteral nutrition is the standard nutritional support of dogs when the enteral route is contraindicated, but it can be difficult because of cost, technical difficulties, and potential complications. Peripheral parenteral nutrition (PPN) may be a feasible option for short-term support in some cases. The objectives of this study were to determine the effect of PPN on nitrogen balance (as an indicator of the effect on protein sparing), serum folate concentrations and serum insulin-like growth factor-I (IGF - I) concentrations in fasting dogs. The effect of PPN on these parameters has not

Management of Hyperglycemia in Hospitalized Patients Receiving Parenteral Nutrition.

Almost half of inpatients on parenteral nutrition experience hyperglycemia, which increases the risk of complications and mortality. The blood glucose target for hospitalized patients on parenteral nutrition is 7.8 to 10.0 mmol/L (140 to 180 mg/dL). For patients with diabetes, the same parenteral nutrition formulae as for patients without diabetes can be used, as long as blood glucose levels can be adequately controlled using insulin. Insulin can be delivered via the subcutaneous or intravenous route or, alternatively, added to parenteral nutrition admixtures. Combining parenteral with enteral

Central, high flow subclavian artery-internal jugular vein fistula for venous access.

A subclavian artery--internal jugular vein shunt was created for intermittent parenteral infusions in six patients who had undergone extensive intestinal resection because of Crohn's disease or ulcerative colitis. The flow rates through the shunt were initially around 1000 ml/min. In three patients the flow later increased, giving rise to mild cardiac symptoms. Slight disturbance from arm ischemia during work was experienced by two patients. Some patients were disturbed by murmurs from the shunt when the flow increased. Narrowing of the vein at the end-to-side anastomosis diminished excessive

Prehepatic hyperalimentation.

A technique for prehepatic infusion of parenteral nutrients is described. Portal vein hyperalimentation allows hepatic modification and control of the infused nutrients before delivery of these substances into the general circulation and theoretically should reduce the incidence of metabolic complications of hyperalimentation. The clinical experience with prehepatic infusions is reported and the metabolic investigations are described. Transumbilical catheters provided prehepatic delivery of parenteral nutrients for 1 month after esophagogastrectomy for esophageal malignancy without serious inf

Ferric carboxymaltose: A practical guide on the administration of iron infusions in general practice.

BACKGROUND: Despite having strong knowledge on iron deficiency, there is ongoing hesitancy among general practitioners (GPs) in providing iron infusions due to reduced confidence in the administration technique and concerns about adverse events. OBJECTIVE: This article aims to prepare GPs, general practice registrars and other health providers with the knowledge to confidently and safely perform ferric carboxymaltose (FCM) infusions. It provides education around the use of parenteral iron, including indications, storage, administration, PBS (Pharmaceutical Benefits Scheme) subsidy, side effec

Ferric carboxymaltose in patients with iron-deficiency anemia and impaired renal function: the REPAIR-IDA trial.

BACKGROUND: Iron-deficiency anemia in non-dialysis-dependent chronic kidney disease (NDD-CKD) frequently requires parenteral iron replacement, but existing therapies often require multiple administrations. We evaluated the efficacy and cardiovascular safety of ferric carboxymaltose (FCM), a non-dextran parenteral iron permitting large single-dose infusions, versus iron sucrose in patients with iron-deficiency anemia and NDD-CKD. METHODS: A total of 2584 participants were randomized to two doses of FCM 750 mg in one week, or iron sucrose 200 mg administered in up to five infusions in 14 days.
