Predicting nonroutine discharge after elective spine surgery: external validation of machine learning algorithms.

OBJECTIVE: Nonroutine discharge after elective spine surgery increases healthcare costs, negatively impacts patient satisfaction, and exposes patients to additional hospital-acquired complications. Therefore, prediction of nonroutine discharge in this population may improve clinical management. The authors previously developed a machine learning algorithm from national data that predicts risk of nonhome discharge for patients undergoing surgery for lumbar disc disorders. In this paper the authors externally validate their algorithm in an independent institutional population of neurosurgical sp

Rethinking hand fracture management in frail patients: the perspectives of hand surgeons and therapists in Ireland.

INTRODUCTION: Frailty is a well-recognized predictor of adverse surgical outcomes. Utilization of frailty indices has become more common across surgical specialties, yet variability persists in how frail patients with hand fractures are assessed and managed by hand surgeons and therapists, and in the extent of their engagement with frailty services. AIM: To evaluate the awareness and institutional management of frailty among consultant and fellow hand surgeons (HS) and hand therapists (HT) treating patients over 65 years old with hand fractures. METHODS: A 16-item web-based questionnaire was

Distress management. Clinical practice guidelines.

The evaluation and treatment model expressed in the NCCN Distress Management Guidelines recommends that each new patient be rapidly assessed in the office or clinic waiting room for evidence of distress using a brief screening tool (the Distress Thermometer and Problem List) presented in Figure 1 (see page 369). A score of 5 or greater on the thermometer should trigger further evaluation and referral to a psychosocial service. The choice of which service should be determined by the problem areas specified on the Problem List. Patients with practical and psychosocial problems are referred to so

Birk-Barel Intellectual Disability Dimorphism and KCNK9 Imprinting Syndrome: Craniofacial Surgery Considerations for an Exceedingly Rare Syndrome.

Birk-Barel intellectual disability dimorphism syndrome, also referred to as KCNK9 imprinting syndrome, is an exceedingly rare condition described in under 20 cases that presents with intellectual disability, hypotonia, scoliosis, dysphonia, dysphagia, and craniofacial dysmorphic features. The condition follows an autosomal dominant pattern of inheritance in the maternally expressed KCNK9 gene on chromosome 8. Due to the complexity of presentation, patients with Birk-Barel syndrome are optimally managed by a multidisciplinary team including a craniofacial surgeon. Previously described craniofac

From the Operating Room to the Dental Chair: Collaborative Prosthodontics in Craniofacial Rehabilitation.

Craniofacial rehabilitation demands a multidisciplinary approach where prosthodontists play a pivotal yet often underutilized role in the continuum of care. Traditionally, prosthetic rehabilitation is considered a postsurgical step, leading to delays in functional restoration and compromised outcomes. This editorial emphasizes the need to redefine prosthodontics as an integral component of craniofacial surgical planning, from the operating room to the dental chair. By fostering collaboration between surgeons, prosthodontists, and digital design teams at the preoperative stage, patient-specific

Transvenous phrenic nerve stimulation: setting up a clinical program.

BACKGROUND: Central sleep apnea (CSA) is a form of sleep-disordered breathing caused by a lack of the drive to breathe during sleep. Phrenic nerve stimulation (PNS) was approved in 2017 for treatment of moderate to severe CSA. However, information on setting up a successful PNS program is lacking. We describe our institution's program to provide a framework to bridge the gap between clinical research and clinical application for PNS therapy. METHODS: The PNS program was created as a joint program between cardiology and sleep medicine. The program team included cardiologists, sleep medicine sp

The association between team-based model of care characteristics and patient outcomes in community health centres: a systematic review.

BACKGROUND: Community Health Centres (CHCs) are community-governed organizations often located in communities facing significant challenges with social determinants of health. To date, no review has summarized the evidence on the effects of team-based models of care on patient outcomes. The objective of this systematic review was to synthesize the evidence on the characteristics of interprofessional teams practicing in CHCs and their associations with patient outcomes. METHODS: A systematic review was conducted following the guidelines of the Cochrane Handbook for Systematic Reviews of Interv

An Antibiotic Stewardship Program Blueprint for Optimizing Verigene BC-GN within an Institution: a Tale of Two Cities.

Rapid diagnostic tests (RDTs) have revolutionized the management of Gram-negative bacteremia by allowing antimicrobial stewardship teams the ability to escalate therapy and improve patient outcomes through timely organism identification and detection of certain resistance determinants. However, given the complex nature of Gram-negative resistance, stewardship teams are left without clear direction for how to respond when resistance determinants are absent, as the safety of de-escalation in this setting is unknown. The primary purpose of this analysis was to determine the negative predictive va

Development of a Hematology-General Medicine Hybrid Team to Improve Care of Patients With Sickle Cell Disease.

PROBLEM: Approximately 100,000 individuals in the United States have sickle cell disease (SCD). These individuals face multiple barriers to equitable care. At Brigham and Women's Hospital, existing health inequities for these patients were compounded by admitting, rounding, and team structures that assigned patients with SCD to multiple medicine teams with a hematologist attending, leading to delays in patient care and gaps in residents' hematology knowledge. APPROACH: A hematology-general medicine hybrid team was created in September 2021 to enhance trainee knowledge, skill, and confidence i

Existential suffering at the end of life in long-term care: A critical essay on recognition, dignity, and witness.

Existential suffering is widely acknowledged in palliative care, but it remains inconsistently recognized and unevenly addressed in long-term care. This critical essay argues that the gap is not caused by clinician indifference, but by a persistent mismatch between whole-person ideals and care systems organized around measurable symptoms, risk management, task completion, and professional uncertainty. Drawing on literature on suffering, existential loneliness, dignity, spiritual care, and recent critiques of impersonal care systems, the essay identifies three interrelated shortcomings: the red

Necessary Harmony Between Anesthesia and Neurosurgery During Extracranial-Intracranial Bypass: A Review of Neuroanesthesia Strategies and Perioperative Insights.

The extracranial-intracranial (EC-IC) bypass is a complex neurosurgical procedure performed for cerebral flow augmentation or flow replacement. Anesthetic management of these patients poses significant challenges due to the delicate balance required to maintain cerebral perfusion, often complicated by extensive cardiovascular comorbidities. Despite the complexity of these cases, current literature offers limited guidance on optimal anesthetic strategies. At our high-volume academic institution, we developed a standardized multimodal anesthetic protocol aimed at achieving intraoperative hemodyn

Organizational innovation under constraints: The case of covid patients' flow management in Parisian hospitals.

The COVID crisis has put hospitals under great stress over the past 2 years and some institutions came close to their breaking points. This has often forced decision makers and the entire institutions to change their practices and the organization of the hospitals in order to continue operating despite limited resources. It has also led some hospitals to develop and implement organizational innovations. This article is based on a qualitative case study analyzing the case of a crisis unit that has implemented various innovative medical and organizational actions in order to manage the flow of r
