Impact of Race, Ethnicity, Primary Language, and Insurance on Reconstruction after Mastectomy for Patients with Breast Cancer at an Urban, Academic Safety-Net Hospital.

BACKGROUND: Disparities in breast reconstruction have been observed in national cohorts and single-institution studies based on race, ethnicity, insurance, and language. However, little is known regarding whether safety-net hospitals deliver more or less equitable breast reconstruction care in comparison with national cohorts. STUDY DESIGN: We performed a retrospective study of patients with either invasive breast cancer or ductal carcinoma in situ diagnosed and treated at our institution (January 1, 2009, to December 31, 2014). The rate of, timing of, and approach to breast reconstruction we

Not a New Story: Place- and Race-Based Disparities in COVID-19 and Influenza Hospitalizations among Medicaid-Insured Adults in New York City.

While SARS-CoV-2 is a novel virus, contagious respiratory illnesses are not a new problem. Limited research has examined the extent to which place- and race-based disparities in severe illness are similar across waves of the COVID-19 pandemic and historic influenza seasons. In this study, we focused on these disparities within a low-income population, those enrolled in Medicaid in New York City. We used 2015-2020 New York State Medicaid claims to compare the characteristics of patients hospitalized with COVID-19 during three separate waves of 2020 (first wave: January 1-April 30, 2020; second

Pet health insurance reduces the likelihood of pre-surgical euthanasia of dogs with gastric dilatation-volvulus in the emergency room of an Australian referral hospital.

AIMS: To determine the association between the presence of pet health insurance and the risk of euthanasia at the time of diagnosis for dogs with gastric dilatation-volvulus (GDV). METHODS: Insurance status at the time of GDV diagnosis was sought for a cohort of 147 non-referred, confirmed cases of GDV that presented to the emergency department of a university-based veterinary hospital in Australia between 2008 and 2017. Insurance status was obtained from the medical record (n=18) or after contacting the owners by phone using a standardised questionnaire (n=129). Animal, clinical and outcome

Motorcycle accident injury severity, blood alcohol levels, insurance status, and hospital costs: a 4-year study in St. Petersburg, Florida.

Clinical, demographic, and financial information for 337 patients injured in motorcycle accidents who were admitted to a community hospital over a 4-year period was reviewed for injury severity, prevalence and degree of alcohol intoxication, and effects of insurance status on hospital, patient, and community costs. Ninety percent of the patients were males (average age 31 years), and 52% had some form of insurance. Of those tested at the time of admission, 36% had blood alcohol levels of > or = 100 mg%. Uninsured patients had higher blood alcohol levels (p = 0.0001), as did older patients (p =

Hospital Surgical Volume and Regional Disparities in Congenital Heart Surgery Outcomes: Analysis of Korean National Health Insurance Claims Data, 2002-2021.

Background and Objectives: The volume-outcome relationship in congenital heart surgery (CHS) has been widely reported internationally, but systematic nationwide evidence from Korea remains limited. Given the concentration of high-volume centers in the Seoul Capital Area (SCA), we aimed to examine whether hospital surgical volume was associated with short-term mortality and to what extent regional disparities could be explained by differences in surgical volume. Materials and Methods: We conducted a nationwide retrospective cohort study of 31,150 patients who underwent CHS in 91 hospitals in Ko

Difference in outcome event coverage between insurance-based and hospital-based databases: a methodological study of diabetes drug use and cardiovascular events in Japan.

INTRODUCTION: In countries with unrestricted access to healthcare, such as Japan, patients may initiate a drug at a clinic or hospital and then may visit another hospital when outcome events occur. Theoretically, an insurance-based database can capture all outcomes, whereas a hospital-based database can only capture outcomes when patients visit that hospital. We examined the difference in outcome event coverage between insurance-based and hospital-based databases in Japan, and its impact on pharmacoepidemiology studies, using diabetes drug use and cardiovascular events as an example. METHODS:

Short-Interval Pregnancy Following Delivery in Catholic-Affiliated Versus Non-Catholic-Affiliated Hospitals Among Patients Insured Through the Medicaid Program.

BACKGROUND: As the number of U.S. Catholic-affiliated hospitals increases, more patients who receive care at Catholic hospitals may face restrictions to reproductive healthcare services, including postpartum contraception and sterilization. Little is known about how Catholic hospital affiliation affects the likelihood of a short-interval pregnancy following a delivery. A short interpregnancy interval is associated with poor obstetric outcomes in subsequent pregnancies. METHODS: In this retrospective study of Medicaid claims data, we examined if an index delivery at a Catholic-affiliated hospi

Influence of health insurance, hospital factors and physician volume on receipt of immediate post-mastectomy reconstruction in women with invasive and non-invasive breast cancer.

For women with breast cancer who undergo mastectomy, immediate breast reconstruction (IR) offers a cosmetic and psychological advantage. We evaluated the association between demographic, hospital, surgeon and insurance factors and receipt of IR. We conducted a retrospective hospital-based analysis with the Perspective database. Women who underwent a mastectomy for invasive breast cancer (IBC) and ductal carcinoma in situ (DCIS) from 2000 to 2010 were included. Logistic regression analysis was used to determine factors predictive of IR. Analyses were stratified by age (<50 vs. ≥ 50) and IBC ver

Predictive patterns of cesarean section utilisation under Indonesia's National Health Insurance following tariff reform: a machine learning analysis of hospital claims.

OBJECTIVES: To examine the predictive contributions of clinical, non-clinical and policy-related factors to cesarean section (C-section) utilisation under Indonesia's National Health Insurance (JKN) and to assess whether the predictive relationships differed between the periods before and after the 2023 Indonesia Case-Based Groups (INA-CBG) tariff reform. We developed and temporally evaluated machine learning models to determine whether non-clinical and tariff-related variables provided additional predictive information beyond clinical risk factors. DESIGN: Retrospective observational study u

Does community-based health insurance protect women from financial catastrophe after cesarean section? A prospective study from a rural hospital in Rwanda.

BACKGROUND: The implementation of community-based health insurance in (CBHI) in Rwanda has reduced out of pocket (OOP) spending for the > 79% of citizens who enroll in it but the effect for surgical patients is not well described. For all but the poorest citizens who are completely subsidized, the OOP (out of pocket) payment at time of service is 10%. However, 55.5% of the population is below the international poverty line meaning that even this copay can have a significant impact on a family's financial health. The aim of this study was to estimate the burden of OOP payments for cesarean sect
