Epidemiology and effect of psychiatric comorbidities on survival rates in patients with systemic sclerosis: A nationwide population-based cohort study.

BACKGROUND: Systemic sclerosis (SSc) is a chronic, disabling connective tissue disease with a considerable psychological burden. While studies suggest a higher prevalence of psychiatric disorders in SSc, most have small sample sizes. METHODS: Using Taiwan's National Health Insurance Research Database, we identified 3270 SSc patients from the catastrophic illness registry and matched them 1:4 by age and sex with 13,080 controls. We compared the incidence rate ratios (IRRs) of new psychiatric disorder diagnoses between the two groups and analyzed risk factors using a Cox proportional hazards mo

Health vulnerabilities of undocumented central and eastern European migrants in Switzerland.

Destitute Central and Eastern European migrants, including homeless people, beggars, and sex workers, are in a highly vulnerable position in Switzerland. In the absence of residence permits, their access to health services and insurance is severely limited, and they suffer from institutional discrimination in Swiss medical facilities. The aim of this study is to examine the forms of health vulnerabilities of destitute mobile Eastern European citizens in Geneva and Zürich. To do this, we carried out narrative-biographical interviews with destitute migrants (n = 38) on their level of access to m

Population-attributable risk of psychiatric disorders for suicide among adolescents and young adults in Taiwan.

BACKGROUND: Youth suicide rates have increased markedly in some countries. This study aimed to estimate the population-attributable risk of psychiatric disorders associated with suicide among Taiwanese youth aged 10-24 years. METHODS: Data were obtained from the National Death Registry and National Health Insurance (NHI) claims database between 2007 and 2019. Youth who died by suicide were included, and comparisons, 1:10 matched by age and sex, were randomly selected from the Registry for NHI beneficiaries. We used multivariable logistic regression to estimate suicide odds ratios for psychiat

Trojan horse? The perils of a private health insurers' buying group for clinical independence, patient confidentiality, and the viability of private hospital care.

Private psychiatric care comprises outpatient as well as private hospital care. The three major funders of private hospital care in Australia are private health insurers, the Australian federal government and, through out-of-pocket costs not covered by the preceding, individual patients. Three big companies, Medibank, BUPA, and nib, dominate the private health insurance market in Australia, resulting in high returns on equity, while private hospitals are being progressively squeezed, resulting in falling profits and a loss in 2024-25. Nonetheless, private health insurers have sought increased

[Outpatient care for child and adolescent psychiatric disorders--data from an insuree-related epidemiological study].

The aim of the study was to examine the extent to which psychiatric disorders are documented as a diagnosis requiring treatment in children and adolescents as well as to investigate the frequency with which non-medicinal interventions are utilized. The database comprised an 18.75% random sample of insurees from the AOK statutory health insurance fund Hessen. The data allowed an examination of the care-seeking behaviour of 55,545 children and adolescents aged 0 to 18 years in the year 2006. With respect to all psychiatric diagnostic categories, a psychiatric diagnosis was documented for 19.3% o

Familial coaggregation of major psychiatric disorders and neurodevelopmental disorders among first-degree relatives of individuals with generalized anxiety disorder.

BACKGROUND: Anxiety disorders, major psychiatric disorders (e.g., schizophrenia and major affective disorders), and neurodevelopmental disorders (e.g., autism and attention-deficit/hyperactivity disorder [ADHD]) may cluster together within families. However, whether the first-degree relatives (FDRs) of individuals with generalized anxiety disorder (GAD) are at an elevated risk of neurodevelopmental or major psychiatric disorders remains unknown. METHODS: We identified 2,378,190 FDRs of patients with GAD and 9,512,760 birth year-matched and sex-matched controls from Taiwan's National Health In

[Differential diagnostic considerations using ICD-10 in chronic back pain with special regard to persistent somatoform pain disorder with somatic and psychological factors (ICD-10 F45.41)].

It is often difficult to pass an expert opinion in cases of chronic back pain. This article analyses the differential diagnostic considerations related to coding various causes in line with ICD-10. It emphasises the I importance of making a careful distinction between orthopoedic and psychiatric conditions and disorders. Simultaneous coding of orthopoedic and psychiatric illnesses and disorders based on a distinct cluster of symptoms necessitates an interdisciplinary approach that consistently applies the ICD-10 definitions of mental an behavioural disorders in order to clearly identify the ma

Working in Pairs: The Dialogic Distribution of Clinical Responsibility.

At an innovative site of psychiatric crisis care work, dialogically trained clinicians in Berlin, Germany transformed the skills learned from the Open Dialogue approach into a means for building solidarity in the face of precarious health structures. Learning to sit with unknowing and tolerate uncertainty, at first developed as techniques to support clients in crisis, became a way by which these clinicians navigated a shifting and unsteady health insurance landscape. In the face of budgetary restrictions and increased oversight by health insurance companies, which challenged the terms of their

Perception of access to health care by homeless individuals seeking services at a day shelter.

Homeless individuals experience significant physical, mental health and substance abuse issues. This study describes the prevalence of health issues and perceptions of access to care among 300 homeless individuals who use a day shelter. Approximately 43% described a serious/chronic physical health problem, 53% a serious mental health problem, and 49% a substance use disorder. Those reporting a serious problem were more likely to have insurance and to report greater perceived access to care but perceived access to care was less than expected. Having insurance was also related to longer duration
