HIV-1 subtype F integrase polymorphisms external to the catalytic core domain contribute to severe loss of replication capacity in context of the integrase inhibitor resistance mutation Q148H.

BACKGROUND: In prior studies, HIV-1 BF recombinants with subtype F integrases failed to develop resistance to raltegravir through the Q148H mutational pathway. We aimed to determine the role of subtype-specific polymorphisms in integrase on drug susceptibility, viral replication and integration. METHODS: Integrase sequences were retrieved from the Los Alamos Database or obtained from the Garrahan HIV cohort. HIV-1 infectious molecular clones with or without Q148H (+ G140S) resistance mutations were constructed using integrases of subtype B (NL4-3) or F1(BF) ARMA159 and URTR23. Integrase chime

Targeted integration of extrachromosomal arrays in C. elegans using PhiC31 integrase.

Extrachromosomal arrays are unique chromosome-like structures created from DNA injected into the Caenorhabditis elegans germline. However, they are unstable unless integrated into a chromosome. Current methods for integration using x-rays or CRISPR can damage DNA and exhibit low efficiency. We demonstrate that the viral integrase PhiC31, which mediates nonmutagenic recombination between short attB and attP sequences, can be used for extremely efficient and targeted integration of arrays. Arrays were integrated by PhIAT (PhiC31-mediated integration of arrays of transgenes) at attB sites on thre

Protein-Protein Interaction as an Efficient Target to Inhibit HIV-1 Integrase.

Human immunodeficiency virus (HIV) infection remains a major global health concern, affecting nearly 40 million people worldwide. Therefore, discovering new therapeutic targets against HIV is still a continued demand. HIV integrase, an essential enzyme that mediates the integration of viral DNA into the host genome, represents a key target for antiretroviral therapy. HIV integrase inhibitors are mainly divided into two classes: integrase strand transfer inhibitors (INSTIs), which act on the enzyme's active site, and allosteric integrase inhibitors (ALLINIs), which bind to a distinct site forme

Targeting HIV-1 integrase: A review of options, challenges and future directions.

HIV-1 infects CD4+ T-cells, causing immunodeficiency and, if untreated, progression to AIDS. Global antiretroviral therapy scale-up has reduced deaths and new infections by nearly half over the past decade, yet lifelong treatment and emerging drug resistance remain critical challenges. This review provides an overview of the HIV-1 lifecycle, with emphasis on the structure, function, and multiple roles of IN in viral replication. We discuss the development, mechanisms of action, clinical utility, and resistance profiles of current integrase inhibitors, underscoring the need for innovative appro

Host proteins interacting with integrase of human immunodeficiency virus type 1.

Integrase (IN) is a key enzyme that catalyzes the integration of Human Immunodeficiency Virus (HIV) viral DNA into the human genome. It consists of three domains: the N-terminal domain, the catalytic core domain, and the C-terminal domain. In the cytoplasm, IN, viral DNA, and multiple host factors assemble into a pre-integration complex (PIC). The translocation of PIC from the cytoplasm to the nucleus is a prerequisite for integration. Numerous host proteins modulate IN activity during PIC translocation and the integration process. Interactions between IN and host factors represent promising t

Impact of integrase strand transfer inhibitors on cardiovascular disease in people with HIV.

PURPOSE: Integrase Strand Transfer Inhibitors (INSTIs) are effective and well-tolerated in HIV treatment, but their cardiovascular impact remains uncertain. This study evaluated the association between INSTI-based antiretroviral therapy (ART) and cardiovascular disease (CVD) risk in people with HIV (PWH). METHODS: We conducted a retrospective cohort study using electronic health records and survey data from the All of Us research program. Adults with HIV on continuous ART for ≥ 1 year, free of CVD at baseline and within one year of ART initiation, were included. INSTI exposure was categorized

Anti-HIV-1 Activity of the Integrase Strand Transfer Inhibitor ACC017.

HIV-1 integrase strand transfer inhibitors (INSTIs) are pivotal to antiretroviral therapy. However, the emergence of drug-resistant mutations necessitates the development of new agents. Here, we present ACC017 as a novel INSTI candidate. ACC017 demonstrated potent activity against the laboratory-adapted HIV-1IIIB strain (EC50 = 0.59 nM; SI > 34,525) and maintained efficacy against a panel of drug-resistant strains (EC50 range from 0.34 to 9.12 nM) and clinical isolated strains (EC50 range from 0.11 to 1.78 nM). Mechanism of action studies confirmed its ability to inhibit the integrase enzyme (

HIV-1 3΄ polypurine tract mutations and integrase inhibitor resistance.

The WHO recommends the use of the integrase strand transfer inhibitor (INSTI) dolutegravir for first-line antiretroviral therapy (ART) in all adults living with HIV infection. Although dolutegravir-based ART is well tolerated and effective, mutations in the integrase-encoding region of HIV-1 pol that confer resistance to dolutegravir can undermine treatment efficacy. An alternative pathway to dolutegravir resistance has also been described involving mutations in the 3΄ polypurine tract (3΄PPT), an RNA sequence element known to be important for retroviral reverse transcription and integration.

A Conformational Escape Reaction of HIV-1 against an Allosteric Integrase Inhibitor.

HIV-1 often acquires drug-resistant mutations in spite of the benefits of antiretroviral therapy (ART). HIV-1 integrase (IN) is essential for the concerted integration of HIV-1 DNA into the host genome. IN further contributes to HIV-1 RNA binding, which is required for HIV-1 maturation. Non-catalytic-site integrase inhibitors (NCINIs) have been developed as allosteric IN inhibitors, which perform anti-HIV-1 activity by a multimodal mode of action such as inhibition of the IN-lens epithelium-derived growth factor (LEDGF)/p75 interaction in the early stage and disruption of functional IN multime

An Isoquinoline Scaffold as a Novel Class of Allosteric HIV-1 Integrase Inhibitors.

Allosteric HIV-1 integrase inhibitors (ALLINIs) are a new class of potential antiretroviral therapies with a unique mechanism of action and drug resistance profile. To further extend this class of inhibitors via a scaffold hopping approach, we have synthesized a series of analogues possessing an isoquinoline ring system. Lead compound 6l binds in the v-shaped pocket at the IN dimer interface and is highly selective for promoting higher-order multimerization of inactive IN over inhibiting IN-LEDGF/p75 binding. Importantly, 6l potently inhibited HIV-1NL4-3 (A128T IN), which confers marked resist

Inhibitors of the integrase-transportin-SR2 interaction block HIV nuclear import.

BACKGROUND: Combination antiretroviral therapy efficiently suppresses HIV replication in infected patients, transforming HIV/AIDS into a chronic disease. Viral resistance does develop however, especially under suboptimal treatment conditions such as poor adherence. As a consequence, continued exploration of novel targets is paramount to identify novel antivirals that do not suffer from cross-resistance with existing drugs. One new promising class of targets are HIV protein-cofactor interactions. Transportin-SR2 (TRN-SR2) is a β-karyopherin that was recently identified as an HIV-1 cofactor. It

Effect of a High-Fat Meal on the Pharmacokinetics of the HIV Integrase Inhibitor Cabotegravir.

Cabotegravir is an integrase inhibitor in clinical development for the treatment and prevention of HIV infection using oral tablets for short-term, lead-in use before subsequent administration of a long-acting injectable formulation. This phase 1, single-center, randomized, 2 × 2 crossover study evaluated the effect of a high-fat meal on the pharmacokinetics (PK) of oral cabotegravir. Healthy adults received oral cabotegravir 30 mg as a single dose on 2 separate occasions, either after fasting or following a high-fat meal (∼53% fat, ∼870 kcal). Safety evaluations and serial PK samples were col

HIV-1 integrase strand-transfer inhibitor resistance in southern Taiwan.

The use of antiretroviral therapy has reduced rates of mortality and morbidity in patients with human immunodeficiency virus/acquired immune deficiency syndrome(HIV/AIDS). However, transmission of drug-resistant strains poses a challenge to control the spread of HIV-1. Primary resistance to integrase strand-transfer inhibitors (INSTIs) is rare despite their increased use. The prevalence of transmitted drug resistance (TDR) to INSTIs was 0.9% in northern Taiwan. This study was to analyse the prevalence and risk factors of TDR to INSTIs in southern Taiwan. In this study, we enrolled antiretrovir

Successful Prevention of Transmission of Integrase Resistance in the Swiss HIV Cohort Study.

The prevalence of integrase strand transfer inhibitor (INSTI)-transmitted drug resistance (TDR) may increase with the increasing use of INSTIs. We analyzed the prevalence of INSTI TDR in the Swiss HIV Cohort Study (2008-2014). In 1 of 1316 drug-naive samples (0.1%), a major INSTI TDR mutation was detected. Prevalence was stable, although INSTIs were increasingly used. We showed that this is in contrast to the introduction of previous drug classes, in which more treatment failures with resistant strains occurred and TDR was observed more rapidly. We demonstrated on a population-level that it is

Prevalence of Integrase Strand Transfer Inhibitors (INSTI) Resistance Mutations in Taiwan.

Antiretroviral therapy containing an integrase strand transfer inhibitor (INSTI) plus two NRTIs has become the recommended treatment for antiretroviral-naive HIV-1-infected patients in the updated guidelines. We aimed to determine the prevalence of INSTI-related mutations in Taiwan. Genotypic resistance assays were performed on plasma from ARV-naïve patients (N = 948), ARV-experienced but INSTI-naive patients (N = 359), and raltegravir-experienced patients (N = 63) from 2006 to 2015. Major INSTI mutations were defined according to the IAS-USA list and other substitutions with a Stanford HIVdb

Resistance against Integrase Strand Transfer Inhibitors and Relevance to HIV Persistence.

Drug resistance prevents the successful treatment of HIV-positive individuals by decreasing viral sensitivity to a drug or a class of drugs. In addition to transmitted resistant viruses, treatment-naïve individuals can be confronted with the problem of drug resistance through de novo emergence of such variants. Resistant viruses have been reported for every antiretroviral drug tested so far, including the integrase strand transfer inhibitors raltegravir, elvitegravir and dolutegravir. However, de novo resistant variants against dolutegravir have been found in treatment-experienced but not in t

New approaches for inhibiting HIV integrase: a journey beyond the active site.

Integrase (IN) inhibitors have proven to be highly beneficial in the treatment of HIV infection. The recently approved inhibitor, raltegravir, the phase III clinical trials compound elvitegravir, and all other agents reported to be in clinical trials bind to the active site of IN and share the same mechanism of action. There is a high probability for the development of cross resistance among these compounds. Therefore, drugs targeting alternative mechanisms and binding sites on IN are desired. Two potential sites for inhibiting the enzyme have been disclosed recently. The first site is located

[HIV-1 integrase inhibition by modified oligonucleotides: optimization of the inhibitor structure].

Integration of human immunodeficiency virus type 1 DNA into the infected cell genome is one of the key steps of the viral replication cycle. Therefore viral enzyme integrase, which realizes the integration, is of interest as a target for new antiviral drugs. Conjugates of 11-mer single stranded oligonucleotides with hydrophobic molecules are shown to be efficient integrase inhibitors since they induce dissociation of the integrase-viral DNA complex. The effect of the oligonucleotide length and structure as well as the structure of hydrophobic molecules on the conjugate inhibitory activity has
