[Diversity in antibiotic use in intensive care units in Germany].

INTRODUCTION: The aim of the study was to present data from the SARI study (Surveillance of Antibiotic Use and Bacterial Resistance in Intensive Care Units) on antibiotic use in Intensive care Units (ICU) in Germany and to determine parameters responsible for involved in the diversity of antibiotic usage. METHODS: Antibiotic administration in 43 intensive care units in Germany was recorded. Antibiotic usage density (AD) was measured: it describes the use of antibiotics and is expressed as defined daily doses (DDD) and is normalized per 1000 patient-days (pd). Prescribing parameters were the n

Variations in mortality rates among Canadian neonatal intensive care units.

BACKGROUND: Most previous reports of variations in mortality rates for infants admitted to neonatal intensive care units (NICUs) have involved small groups of subpopulations, such as infants with very low birth weight. Our aim was to examine the incidence and causes of death and the risk-adjusted variation in mortality rates for a large group of infants of all birth weights admitted to Canadian NICUs. METHODS: We examined the deaths that occurred among all 19 265 infants admitted to 17 tertiary-level Canadian NICUs from January 1996 to October 1997. We used multivariate analysis to examine th

[Admissions of patients with AIDS to pediatric intensive care units].

OBJECTIVE: To describe the most significant clinical features of children with acquired immunodeficiency syndrome who required admission to a pediatric intensive care unit (PICU). METHODS: Retrospective study of 12 patients with AIDS who required 13 admissions, between January 1988 and December 1997. RESULTS: Mean age at admission was 15 months (1 month-6 years). Seven patients were under 1 year of age; four were diagnosed during their stay in the unit. The most common reason for admission was respiratory failure (six patients), followed by cardiac failure. Six patients needed mechanical ven

External sources of vancomycin-resistant enterococci for intensive care units.

OBJECTIVE: The incidence of colonization and infection with vancomycin-resistant enterococci (VRE) has increased dramatically in the last 5 yrs, especially in intensive care units (ICUs). We studied VRE-colonization in patients on admission to a medical ICU (MICU) where VRE colonization is endemic. DESIGN: Prospective, descriptive analysis. SETTING: An MICU of a public hospital. PATIENTS: Three hundred and one consecutively admitted patients. MEASUREMENTS AND MAIN RESULTS: Rectal swabs were obtained on admission from all patients. VRE isolates from all colonized patients were genetically f

[Cardiorespiratory management of severe injured patients in intensive care units].

Late mortality of severely injured patients could be prevented by the quality of early cardiorespiratory management. Indeed traumatized patients with high risk of multiple organ failure (according to age, ISS, amount of blood transfused, and/or metabolic acidosis) need a pulmonary artery catheterization as soon as possible (postdefinitive phase: during the surgical period or at the admission in the ICU). Such a procedure allows the intensivist to determine therapeutic goals in term of O2 delivery (DO2) and O2 uptake (VO2) in front of a frequent increased peripheral O2 demand, These goals (usua

[Microbiologic flora in two different periods in intensive care units].

Bacterial examinations performed in two periods (Nov. 1985-Feb. 1986) and Nov. 1988-Feb. 1989) on materials taken from patients admitted into two similar Intensive Care Units (ICU) of the same hospital were evaluated. Excluding oropharyngeal swab cultures, which were performed only in the second period, Gram positive bacteria were 45.3% in 1985-1986 and 62.3% in 1988-1989 and Gram negative fell from 41.9% to 29%. The percentage of fungi remained unchanged. Microorganisms did not grow in 33.3% of tracheal aspirate specimens in 1985-1986 and in 54.3% in 1988-1989, probably depending on the large

Clinical Frailty in Intensive Care Units: Prevalence and Associated Factors.

BACKGROUND: Frailty is a syndrome characterised by increased sensitivity to stress alongside a reduction in an individual's biopsychosocial reserves. In intensive care patients, frailty is a significant condition that affects mortality and morbidity. However, routine frailty screening and its integration into care plans in intensive care units (ICUs) have not been standardised. AIM: The study aimed to determine the prevalence of frailty in patients hospitalised in an ICU in Türkiye and to examine the factors associated with frailty. METHODS: This prospective, observational study was conducte

AI-guided precision parenteral nutrition for neonatal intensive care units.

One in ten neonates are admitted to neonatal intensive care units, highlighting the need for precise interventions. However, the application of artificial intelligence (AI) in guiding neonatal care remains underexplored. Total parenteral nutrition (TPN) is a life-saving treatment for preterm neonates; however, implementation of the therapy in its current form is subjective, error-prone and resource-consuming. Here, we developed TPN2.0-a data-driven approach that optimizes and standardizes TPN using information collected routinely in electronic health records. We assembled a decade of TPN compo

[Postextubation dysphagia : Challenges regarding interdisciplinary collaboration in intensive care units].

BACKGROUND: Postextubation dysphagia (PED) represents a significant morbidity and mortality factor even in nonneurological intensive care units (ICU), potentially prolonging both ICU and overall hospital stays. METHODS: A literature review was conducted to access the relevance, diagnostic approaches, and therapeutic options for PED, which were then placed in context using a clinical case study. RESULTS: Although evidence-based guidelines for PED are lacking in the literature, several articles and recommendations addressing multiprofessional treatment strategies were identified. Antimicrobial susceptibility at intensive care units in Sudan, antibiogram development.

Antibiograms are statistics on bacterial spectrum and resistance rates. It is of importance to regularly monitor the trends in antimicrobial resistance within the institution through the periodic cumulative resistance to provide the effective empiric antibiotic selection, enhanced infection control interventions, and containment strategies. Antimicrobial stewardship interventions improve quality and quantity of prescribing antimicrobial in ICUs without compromising any patient outcomes. The aim of this study was to develop a local unit-specific antibiogram for the Intensive Care Units based on

Impact of sleep disturbances on outcomes in intensive care units.

BACKGROUND: Sleep deprivation is common in intensive care units (ICUs) and may alter respiratory performance. Few studies have assessed the role of sleep disturbances on outcomes in critically ill patients. OBJECTIVES: We hypothesized that sleep disturbances may be associated with poor outcomes in ICUs. METHODS: Post-hoc analysis pooling three observational studies assessing sleep by complete polysomnography in 131 conscious and non-sedated patients included at different times of their ICU stay. Sleep was assessed early in a group of patients admitted for acute respiratory failure while brea

Emergence of Candida auris in intensive care units in Algeria.

BACKGROUND: Currently, Candida auris is among the most serious emerging pathogens that can be associated with nosocomial infections and outbreaks in intensive care units. Clinicians must be able to identify and manage it quickly. OBJECTIVE: Here, we report for the first time in Algeria seven cases of C. auris infection or colonisation. METHODS AND RESULTS: The strains were isolated from clinical sites including bronchial aspirates (n = 4), wound swabs (n = 1), urine sample (n = 1) and peritoneal fluid (n = 1), in patients admitted to the intensive care unit. Candida auris was identified both

Surveillance of Antibiotic Prescribing in Intensive Care Units in Poland.

Antibiotic use and microbial resistance in health care-associated infections are increasing globally and causing health care problems. Intensive Care Units (ICUs) represent the heaviest antibiotic burden within hospitals, and sepsis is the second noncardiac cause of mortality in ICUs. Optimizing appropriate antibiotic treatment in the management of the critically ill in ICUs became a major challenge for intensivists. We performed a surveillance study on the antibiotic consumption in 108 Polish ICUs. We determined which classes of antibiotics were most commonly consumed and whether they affecte

Incorporating speech-language pathology within Australian neonatal intensive care units.

Working in neonatal intensive care units (NICU) is considered to be advanced practice for speech-language pathologists (SLPs) requiring post-graduate specialised education and ongoing clinical mentoring, not easily accessible within some countries such as Australia. This review investigated whether there was evidence to support SLPs as core NICU team members, rather than fleeting therapists. The review found that the roles and responsibilities of SLPs in NICUs included assessment and intervention of preterm infant communication, feeding and swallowing using a family-centred approach.
