Exploratory ultrasonographic evaluation of parasternal intercostal muscle asymmetry in hemiparesis following stroke: a case-control ultrasonographic study.

PURPOSE: Diaphragmatic weakness is a known cause of respiratory dysfunction in stroke. However, the role of extra-diaphragmatic muscles in this regard remains unclear. The aim of this study was to evaluate parasternal intercostal muscle thickness and asymmetry between the two sides in hemiparetic patients after stroke using ultrasonography and to provide exploratory and hypothesis-generating data regarding inspiratory muscle involvement and thoracic mechanical changes. METHODS: This prospective case-control study included 30 stroke patients and 30 healthy volunteers. Demographic data, paraste

Pressure-dependent coupling between arterial pressure and intercostal muscle blood-flow index during acute hypotension in humans.

Regulation of respiratory muscle blood flow during acute systemic hypotension remains poorly understood in humans because continuous assessment of microvascular perfusion during rapid haemodynamic transients has been technically challenging. Using diffuse correlation spectroscopy, we investigated second-by-second coupling between mean arterial pressure (MAP) and intercostal muscle blood-flow index (BFI) during an abrupt hypotensive stimulus in healthy humans. Fifteen young men underwent 5 min of suprasystolic bilateral thigh occlusion followed by rapid cuff release. The MAP and intercostal BFI

Breath-by-breath parasternal intercostal muscle stiffening tracks inspiratory effort during graded inspiratory threshold loading in humans.

Inspiratory effort is associated with the recruitment of extra-diaphragmatic muscles, including parasternal intercostal muscles (PIMs). The mechanical behavior of PIM and its relationship to indices of inspiratory effort in humans remains poorly characterized. We investigated whether PIM stiffening, assessed using ultrafast ultrasound shear wave elastography (SWE), tracks inspiratory effort breath-by-breath during graded inspiratory loading. Fourteen healthy adults (9 men and 5 women) were studied during quiet breathing and inspiratory threshold loading at 10%, 20%, 30%, and 40% of maximal ins

Parasternal intercostal muscle thickening fraction predicts weaning failure in invasively ventilated patients with COPD: a prospective observational study.

BACKGROUND: Weaning from invasive mechanical ventilation (IMV) is a critical phase of intensive care in patients with chronic obstructive pulmonary disease (COPD), in whom weaning failure is associated with increased morbidity and prolonged intensive care unit stay. Although diaphragm ultrasonography has been extensively studied, the contribution of accessory respiratory muscles during the weaning process remains insufficiently explored. We aimed to investigate whether parasternal intercostal muscle thickening fraction (IMTF), assessed by ultrasound, is associated with weaning outcomes in inva

Prolonged loss of intercostal muscle mass and its predictors in COVID-19 patients: A retrospective study from tertiary hospital.

Sarcopenia is a contributing factor in the development of long-COVID syndrome. We aimed to investigate how intercostal muscle mass changes over 3 months compared to other chest wall muscles following COVID-19 infection, along with identifying factors contributing to intercostal muscle loss during follow-up. We retrospectively studied 110 COVID-19 patients, analyzing muscle masses in the intercostal, pectoralis, and thoracic 12th vertebra level (T12) on initial and follow-up CT scans. Muscle mass was quantitatively assessed using density histogram analysis. We calculated the muscle difference r

Nerve transfer to deltoid muscle using the intercostal nerves through the posterior approach: an anatomic study and two case reports.

PURPOSE: To evaluate the feasibility of restoring the deltoid function in patients with C5 through C7 root avulsion injuries by transferring 2 intercostal nerves to the anterior branch of the axillary nerve through a posterior approach. The preliminary results of the clinical application of this procedure also are reported. METHODS: The study was performed on 10 fresh cadavers. The lengths of the third, fourth, and fifth intercostal nerves from the costochondral junction to the midaxillary line were recorded. The distance from the pivot point at the midaxillary line to the anterior branch of

Acute reconstruction of traumatic injuries of median and ulnar nerves by grafting with intercostal nerves from the rectus muscle: case reports.

In two cases of complicated hand and wrist injuries for which rectus flaps were used as soft-tissue coverage, intercostal nerves from the rectus muscles were harvested simultaneously to graft significant nerve defects. In one patient, a 7-cm ulnar-nerve defect was bridged with four intercostal nerves. The other patient had a 5-cm median-nerve defect repaired with three intercostal nerves. Both patients recovered either protective or normal sensation; and the patient with the median nerve injury recovered thenar muscle function. Intercostal nerves harvested with rectus muscle flaps can be the b

Restoration of elbow flexion and wrist extension in brachial plexus paralyses by means of free muscle transplantation innervated by intercostal nerve.

Traumatic lesions of the brachial plexus with nerve root avulsion pose extremely complex therapeutic problems. Since 1965, the authors have performed neurotization of the musculocutaneous nerve by intercostal nerves in order to restore elbow flexion. The results of this operation are considerably poorer when the interval between the trauma and the neurotization exceeds 6 months. In these late cases, the authors have used, since 1978, a free muscle transfer reinnervated by a transposed intercostal nerve. 17 patients have been operated by means of this technique; 8 of the 11 patients with a foll

Exercise training decreases intercostal and transversus abdominis muscle blood flows in heart failure rats during submaximal exercise.

Diaphragm muscle blood flow (BF) and vascular conductance (VC) are elevated with chronic heart failure (HF) during exercise. Exercise training (ExT) elicits beneficial respiratory muscle and pulmonary system adaptations in HF. We hypothesized that diaphragm BF and VC would be lower in HF rats following ExT than their sedentary counterparts (Sed). Respiratory muscle BFs and mean arterial pressure were measured via radiolabeled microspheres and carotid artery catheter, respectively, during submaximal treadmill exercise (20 m/min, 5 % grade). During exercise, no differences were present between H

Intercostal nerve transfer for restoration of the diaphragm muscle function after phrenic nerve transfer in total brachial plexus avulsion.

OBJECT: To determine the possibility of innervation of the diaphragm muscle using intercostal nerve after ipsilateral phrenic nerve transfer in total brachial plexus avulsion. METHODS: Bilateral phrenic nerves and the 9th intercostal nerves were observed inside the thorax. The point where the phrenic nerve entered the diaphragm muscle (point A), the point where the 9th intercostal nerve gave rise to the cutaneous branch (point B) and crossed the posterior axillary line (point C) and the point where the posterior axillary line met the insertion of the diaphragm muscle (point D) were identified

Free muscle transplantation combined with intercostal nerve crossing for reconstruction of elbow flexion and wrist extension in brachial plexus injuries.

Complete paralysis due to traumatic brachial plexus injury is extremely difficult to treat when the injury affects whole nerve roots and when motor function fails to show any signs of recovery. Seddon has suggested that arthrodesis of the shoulder and amputation at the humerus, combined with the use of a functional upper extremity prosthesis, was the most practical procedure available. Since 1965, in cases of irreparable lesions such as complete root avulsion type injuries, we have performed direct intercostal nerve crossing to the musculocutaneous nerve without free nerve graft to achieve elb

Muscle oxygenation and local blood volume difference between intercostal and deltoid during dry static apnea in breath-hold divers.

During either a static or dynamic apnea, oxygen is delivered to vital organs, i.e., the brain and heart, and there is a compensatory reduction of oxygen consumption in peripheral tissues. Additionally, maximal apnea is characterised by the initial easy-going phase and subsequent struggle phase in which involuntary breathing movements appear. The aim of this study was to compare the oxygenation and local blood volume of one active (external intercostal) and one non-active (deltoid) muscle during maximal dry static apneas in breath-hold divers while supine. Thirteen breath-hold divers performed

Free composite latissimus dorsi muscle-rib flap not containing the intercostal artery and vein for reconstruction of bone and soft-tissue defects.

Reconstruction of bone and soft-tissue defects of the forehead and leg was done with use of a free latissimus dorsi muscle-rib flap. By utilizing the curve characteristic of the rib and by filling the cavity with latissimus dorsi muscle, the forehead could be restored in a good shape. In our application of this flap to the leg, it is considered in retrospect that reconstruction with the use of two ribs should have been done. Furthermore, it was concluded that in comparison with the heretofore employed vascularized rib graft, the scope of operative invasion is small in the use of this flap with

Bochdalek Hernia and Partial Diaphragmatic Agenesis: Pedicled Intercostal Muscle Flap and Mesh Repair in a Young Adult with Sickle Cell Disease.

Congenital Bochdalek hernia (BH) in an adult is rare and has an unusual presentation. They are confined to the pediatric age group with an incidence of 1:3,000 live births. It rarely persists asymptomatic until adulthood. Surgical repair by thoracic, abdominal, or thoraco-abdominal approach is the treatment of choice with diaphragmatic reconstruction in associated diaphragmatic agenesis. With only 10 cases of BH with partial diaphragmatic agenesis reported to date, we discuss the rarity, unusual presentation, and management of BH in a young adult with sickle cell disease that has not been repo

Spinal Accessory Nerve Versus Intercostal Nerves in Gracilis Free-Functioning Muscle Transfer for Elbow Flexion in Adult Brachial Plexus Patients.

PURPOSE: Traumatic brachial plexus injury may cause either partial or complete loss of arm function. Surgical reconstruction of elbow flexion using a gracilis free-functioning muscle transfer (FFMT) may be necessary. The donor nerve, which could be the spinal accessory nerve (SAN) or intercostal nerves (ICN), can affect the final muscle strength, but little is known about how they affect voluntary neuromuscular control. This study aimed to examine the differences in voluntary neuromuscular control of the FFMT gracilis reinnervated by either SAN or ICN using biomechanical measures. METHODS: Pa
