Systemic lupus erythematosus is a systemic autoimmune pathology that generally presents in young people and manifests acutely, while its late presentation in people over 50 years of age is rare and insidious. Vasculitis is a pathology that affects any vessel producing fibrinoid necrosis, and presents with a positive antineutrophil cytoplasmic antibody. The concomitance of these two entities is rare and leads to worse clinical outcomes. We present a 73-year-old female patient who presented with rapidly progressive glomerulonephritis requiring renal replacement therapy, pulmonary tuberculosis, l

Pediatric critical care medicine : a journal of the Society of Critical Care Medicine and the World Federation of Pediatric Intensive and Critical Care Societies2011eng
OBJECTIVE: To assess the outcome of pediatric patients supported by concomitant extracorporeal membrane oxygenation (ECMO) and continuous renal replacement therapy (CRRT).
DESIGN, SETTING, AND PATIENTS: Acute kidney injury is associated with mortality in ECMO patients. CRRT in patients on ECMO provides an efficient and potentially beneficial method of acute kidney injury management. Concern that concomitant CRRT use increases the risk of developing anuria and chronic renal failure limits its use in some centers. We hypothesized that development of chronic renal failure is rare with concurrent

Continuous renal replacement therapies (CRRT) are used to manage fluid overload and/or renal failure. The continuous nature of the fluid and solute removal has less impact on hemodynamic variables in critically ill patients, making CRRT preferred over intermittent hemodialysis for some patients in the intensive care arena. The impact of CRRT on drug removal is variable depending on the CRRT modality, the ultrafiltrate and dialysate flow rates, the filter, and the patient's residual renal function; all of these may change from patient to patient or even in the same patient depending on the clin

Multiple wasp stings may cause fatal complications, such as anaphylactic reactions, intravascular hemolysis, rhabdomyolysis, acute kidney injury (AKI), increased levels of liver enzymes, clotting abnormalities, or even death. AKI-related mortality due to multiple wasp stings may reach 25%, occurring within the early onset of disease; therefore, renal function should be continuously monitored within the first few days following the stings. Herein, we report 2 cases of AKI due to multiple stings of wasp (Vespa affinis). In both cases, delayed hospital admissions and gradual loss of kidney functi

In this second installment of the proceedings of the University of Alabama at Birmingham Continuous Renal Replacement Therapy (CRRT) Academy, we focus on the topic of transitions of care in acute renal replacement therapy (RRT). Although we have accumulated data from thousands of critically ill patients with AKI randomized to different strategies for RRT initiation, no trial data exist to guide de-escalation of RRT in the intensive care unit. However, for survivors of severe AKI whose kidney function does not recovery rapidly enough to allow for liberation directly from CRRT, successful de-esc

Reliable models of renal failure in large animals are critical to the successful translation of the next generation of renal replacement therapies (RRT) into humans. While models exist for the induction of renal failure, none are optimized for the implantation of devices to the retroperitoneal vasculature. We successfully piloted an embolization-to-implantation protocol enabling the first implant of a silicon nanopore membrane hemodialyzer (SNMHD) in a swine renal failure model. Renal arterial embolization is a non-invasive approach to near-total nephrectomy that preserves retroperitoneal anat

Continuous renal replacement therapy (CRRT) was first described in 1977 for the treatment of diuretic-unresponsive fluid overload in the intensive care unit (ICU). Since that time this treatment has undergone a remarkable technical and conceptual evolution. It is now available in most tertiary ICUs around the world and has almost completely replaced intermittent haemodialysis (IHD) in some countries. Specially made machines are now available, and venovenous therapies that use blood pumps have replaced simpler techniques. Although, it remains controversial whether CRRT decreases mortality when

Methotrexate (MTX) is widely used in oncology but carries a risk of severe nephrotoxicity, especially at high doses. We report a case of a man in his 70s with diffuse large B-cell lymphoma who developed acute kidney injury (AKI) and refractory hypotension 48 hours after receiving high-dose methotrexate (HD-MTX). Despite standard interventions including leucovorin rescue and urine alkalinisation, MTX clearance was impaired due to AKI. In the absence of glucarpidase, the patient was successfully managed with intermittent high-flux haemodialysis followed by continuous renal replacement therapy. T

Three patients reaching end-stage renal failure were referred to an internist-nephrologist. A 53-year old woman was referred late, with several late complications of chronic renal failure such as secondary hyperparathyroidism and signs of left ventricular hypertrophy. In addition she suffered from complications of a temporary vascular access required for intermittent haemodialysis prior to commencing peritoneal dialysis. A 52-year old man was referred early, resulting in reduced late complications of chronic renal failure and the timely start of peritoneal dialysis. An 84-year old man was refe

Fontan palliation depends on low pulmonary vascular resistance in order to maintain pulmonary blood flow and adequate oxygenation. This physiology results in higher central venous pressures with limited renal perfusion pressure and cardiac output. Positive pressure ventilation with mechanical ventilation increases intrathoracic pressure and raises central venous pressure and can further limit pulmonary and renal perfusion. Fluid removal with intermittent hemodialysis can be challenging in Fontan patients and can cause intolerable hypotension, however the increased abdominal filling pressures d

AIMS: Our objectives were to review the characteristics of patients who developed atheroembolic renal disease requiring dialysis as well as their renal function recovery and survival rates.
METHODS: All cases of atheroembolic disease with renal failure severe enough to require dialysis were reviewed from January 1984 to December 2000 in two centers. The diagnosis of atheroemboli was based on clinical presentation and/or biopsy. Acute renal failure was defined as a serum creatinine >200 micromol/l if normal at baseline or doubling from baseline if chronic renal failure, whereas renal function

Tandem extracorporeal blood purification and support therapies integrate established therapies such as continuous renal replacement therapies (CRRT), extracorporeal membrane oxygenation (ECMO), intermittent or prolonged renal replacement therapies with emerging adsorptive, immunomodulatory, and organ-support technologies. These approaches are increasingly applied in critically ill children with complex multi-organ dysfunction, particularly in the settings of severe sepsis, hyperinflammatory syndromes, acute kidney injury, liver failure, and refractory cardiorespiratory compromise. Continuous arteriovenous hemofiltration is a form of renal replacement therapy whereby small molecular weight solutes and water are removed from the blood via convection, alleviating fluid overload and, to a degree, azotemia. It has been used in many adults and several children. However, in patients with multisystem organ dysfunction and acute renal failure, continuous arteriovenous hemofiltration alone may not be sufficient for control of azotemia; intermittent hemodialysis or peritoneal dialysis may be undesirable in such unstable patients. Recently, the technique of continuous arteriovenous

Rhabdomyolysis is a serious clinical condition often associated with serious complications and can be life-threatening in nature. The following is a case report of a healthy 17-year-old male adolescent who presented with rhabdomyolysis due to a dual hit of Influenza A and excessive physical exertion. Acute onset of myalgia, weakness in the lower extremities, and dark urine warranted his hospitalization in the intensive care unit for critical monitoring and investigation. The laboratory findings indicated a high creatine kinase level of more than 60,000 U/L, severe metabolic acidosis, hyperkale

BACKGROUND: Despite the general recommendation of AUC 0-24h /MIC ratio of 400-600 mg·h/L for vancomycin (VAN) effectiveness, there is no strong evidence of this index for patients undergoing renal replacement therapy (RRT). The aim of this scoping review was to summarize the scientific literature to assess the current evidence on VAN monitoring in hospitalized patients receiving intermittent hemodialysis (HD), identify gaps in knowledge, and guide future research.
METHODS: A systematic search was performed using PubMed, Web of Science, Embase, LILACS, and the Cochrane Library databases based

UNLABELLED: Septic shock is often complicated by severe metabolic acidosis, for which renal replacement therapy may be considered. However, little is known about the use of intermittent hemodialysis to manage this condition. The aim of this study was to compare physiologic and biochemical variables and vasopressor requirements before and after intermittent hemodialysis among patients who received intermittent hemodialysis to manage metabolic acidosis during resuscitation of septic shock.
DESIGN: This retrospective, cross-sectional study was conducted between April 2014 and September 2015.

Pulmonary arterial hypertension (PAH) is a progressively life-threatening disease that causes right heart failure (RHF). Renal dysfunction frequently complicates PAH with RHF and is associated with a worse prognosis. Renal replacement therapy (RRT) may be a therapeutic option, although its efficacy and safety are unclear. We describe a 30-year-old male with severe PAH who developed renal insufficiency and diuretic-refractory volume overload complicated with RHF but was successfully managed with intermittent RRT via a subcutaneously fixed superficial artery for 4 years. RRT led to haemodynamic

INTRODUCTION: Acute kidney injury (AKI) is a common occurrence in the intensive care unit (ICU). Studies have looked at outcomes of renal replacement therapy using intermittent haemodialysis (IHD) in ICUs with varying results. Little is known about the outcomes of using IHD in resource-limited settings where continuous renal replacement therapy (CRRT) is limited. We sought to determine outcomes of IHD among critically ill patients admitted to a low-income country ICU.
METHODS: A retrospective review of patient records was conducted. Patients admitted to the ICU who underwent IHD for AKI were
