INTRODUCTION: Internal hernias are rare causes of small bowel obstruction and are often difficult to diagnose preoperatively. Retro-appendicular internal hernias are exceedingly rare, with very few reports in the literature. This case is noteworthy for involving a patient with no prior history of abdominal surgery, highlighting an unusual presentation of a retro-appendicular internal hernia.
CASE PRESENTATION: A man in his mid-60s, with no history of abdominal surgery, presented to the emergency department with acute right lower quadrant abdominal pain. Physical examination revealed localized

BACKGROUND: Chylous ascites is a rare medical disorder in which chylous fluid leaks from the lymphatic system into the abdominal cavity. When a patient has a history of abdominal surgery, especially subtotal gastrectomy, it is crucial for the physician to remain vigilant regarding the risk of internal hernia, which may result in lymphatic blockage and subsequent chylous ascites.
CASE PRESENTATION: A 70-year-old male admitted to our hospital complained of abdominal pain for 9 h. The patient had no obvious cause of abdominal pain, and the abdominal pain was not severe. The surgical history indi

INTRODUCTION: Intestinal obstruction is one of causes of acute abdomen leading to laparotomy. Even though there are different causes of small bowel obstruction (SBO), internal hernia is a rare one. Though different types of internal hernias occur, herniation through the mesoappendix is reported only four times in the literature.
CASE PRESENTATION: We describe here such a type of hernia on a 50 yrs old male presented with symptoms and signs of SBO whose cause was diagnosed to be internal hernia through mesoappendix intraoperatively. He was managed with appendectomy and release of the hernia.

Roux-en-Y gastric bypass, a procedure proven effective for treating morbid obesity and metabolic disorders, carries the risk of complications such as the formation of internal hernias. These hernias are often difficult to diagnose and can be potentially fatal because they can cause structural obstruction. Most internal hernias occur in the jejunojejunostomy mesentery space, followed by Petersen's space hernias, although herniation at other locations can also occur. Our case report presents an example of a rare internal hernia after laparoscopic Roux-en-Y gastric bypass. A 36-year-old woman pre

Among the advantages of the One Anastomosis Gastric Bypass (OAGB) are the lack of jejuno-jejunal anastomosis and a supposed lower incidence of internal hernia (IH), with only a few cases reported until now. However, the incidence of IH after OAGB is not null. We present a video of the laparoscopic management of an IH that occurred after an OAGB. The patient was a 49-year-old female who had undergone a laparoscopic revisional OAGB 2 years previously after a failed laparoscopic adjustable gastric banding. She was referred to our Unit for recurrent postprandial colicky pain. She lost a total of 5

Appendiceal mucinous neoplasm is a rare form of appendix disorder of all appendectomies. Patients may present with many non-special symptoms which the right low quadrant pain is the mostly reported. We reported a case of 70-year-old man who was admitted to emergency for abdominal pain and progressive aggravation of peritoneal irritation sign. Computed tomography (CT) revealed incomplete intestinal obstruction with volvulus of partial small intestine in right lower quadrant, and internal hernia was suspected and simple appendectomy was performed followed. The postoperative pathologic diagnosis

Internal hernias involve herniation of viscera into an abdominal compartment through a defect in the mesentery or peritoneum. Herniation may occur through normal anatomic structures or through pathologic defects secondary to congenital abnormality, inflammation, trauma, or surgery. Patients with an internal hernia most commonly present with acute bowel obstruction. While internal hernia is an uncommon cause of bowel obstruction, making up approximately 0.2-0.9% of cases (Choi, 2017), the incidence is increasing due to greater use of techniques such as Roux-en-Y for liver transplant and gastric

INTRODUCTION: Internal hernia due to a Meckel diverticulum is a common presentation of bowel obstruction mostly seen in pediatric population. However, it has been stated that among 5% of the patients had a giant Meckel diverticulum (defined as a Meckel diverticulum with increased dimensions than the ones commonly found), being this condition very unusual.
PRESENTATION OF CASE: We presented a 19 year old male with acute abdominal pain suggestive of appendicitis. During appendectomy we discovered ischemic and necrotic signs in a bowel segment, leading us to perform a laparotomy that revealed a

BACKGROUND: The incidence of internal hernia after gastrectomy can increase with the increasing use of laparoscopic surgery, although this trend has not been elucidated.
METHODS: Clinical information was collected from medical records and by questionnaire for 18 patients who underwent surgical treatment for internal hernia after gastrectomy for gastric cancer in 24 hospitals from January 2005 to December 2009.
RESULTS: Gastrectomy for gastric cancer was open/distal gastrectomy (DG) in five (28%) patients, open/total gastrectomy (TG) in seven (39%), laparoscopy-assisted/DG in three (17%), and

A redundant loop of transverse colon was herniated symptomatically behind the right liver to a suprahepatic, subdiaphragmatic position in an 11-year-old girl (Chilaiditi Syndrome). Constipation from birth, partial obstruction, and recurrent severe right upper quadrant pain unresponsive to nonoperative management warranted transverse colectomy.

After colonoscopy with polypectomy, a patient developed a surgically acute abdomen. Although abdominal radiology did not show free air, a presumptive diagnosis of bowel perforation was made and laparotomy performed. At the time of surgery the colon was normal, and there was no peritoneal contamination. A loop of ileum was discovered incarcerated into an internal paracecal hernia. The bowel was freed, and the operation was completed without need for resection. Several cases of incarcerated inguinal hernia resulting from endoscopy are described in the medical literature; this is the first report

Internal herniation with volvulus of the small intestine is an uncommon, but potentially fatal, complication after liver transplantation. We present here four cases in which the herniation occurred around the Roux-en-Y loop used for the biliary reconstruction. One patient died due to intestinal and liver allograft necrosis; another lost almost the entire small intestine and has since undergone successful intestinal transplantation. Two patients survived following surgery that involved reduction of the hernia and closure of the mesenteric defect. Clinical diagnostic implications emphasize early

Persistent bile-stained vomiting in a neonate led to a radiologic workup that revealed the existence of a situs inversus and a partial duodenal obstruction. A duodenal diaphragm with a central aperture was diagnosed. Surgery confirmed the preoperative diagnosis. In addition, an annular pancreas with an anterior gap and a right parietocolic internal hernia containing all the small bowel were discovered. Embryologic aspects, therapeutic options, and the literature were discussed.

Surgical hernia repair in an apparently normal man leaded to the diagnosis of internal male pseudohermaphroditism after removal of a cystic structure which proved to be a remnant of the Müllerian duct on microscopic examination. The diagnosis was confirmed at explorative laparotomy in search of the second testicle. The terminology of the condition is reviewed and the therapeutic attitude is discussed.

Internal hernias are particularly difficult to diagnosis and should be considered in patients presenting with sudden, severe abdominal pain and symptoms of intestinal obstruction. Internal hernias carry a high risk of morbidity and mortality. Here, we present an unusual case of internal hernia through the foramen of Winslow of an ileoileal anastomosis created 25 years prior that was successfully diagnosed and operatively reduced via single-port laparoscopy with an optimal clinical outcome. Uniquely, the defect was closed using a ligamentum teres buttress to avoid undue tension at the foramen.

BACKGROUND: Meckel's diverticulum is a common congenital anomaly of the gastrointestinal tract, with a higher incidence rate in children under 7 years old. The condition is characteristically asymptomatic but may become a clinical concern when complications such as intestinal obstruction, bleeding, perforation, or diverticulitis precipitate acute abdominal presentations.
CASE SUMMARY: This report describes the case of a middle-aged man initially suspected of having acute appendicitis, which rapidly progressed to acute peritonitis with concomitant intestinal obstruction observed during preoper

A 63-year-old woman was admitted with abdominal pain two months after laparoscopic abdominoperineal resection for rectal cancer. Computed tomography revealed dilated small intestine had passed through a defect between the lifted sigmoid colon and abdominal wall. She was diagnosed with small bowel obstruction without strangulation due to internal hernia and managed nonoperatively based on her wish. Recurrence of intestinal obstruction occurred for which curative surgery was performed laparoscopically. The herniated intestine was restored to the normal position, and the hernia orifice was closed

BACKGROUND: Chylous ascites is often reported in cases with lymphatic obstruction or after lymphatic injuries such as intraabdominal malignancies or lymphadenectomies. However, chylous ascites is also frequently encountered in operations for internal hernias. We sought to characterize the frequency and conditions when chylous ascites is encountered in general surgery patients.
METHODS: Data from patients who underwent operations for CPT codes related to open and laparoscopic abdominal and gastrointestinal surgery in our tertiary hospital from 2010 to 2019 were reviewed. Patients with the post
