Physical activity in advanced urological cancers undergoing immune checkpoint inhibitor treatment: a qualitative study exploring patient perspectives and experiences.

PURPOSE: Physical activity (PA) can improve health and treatment outcomes in cancer patients. However, patients with advanced cancer undergoing immune checkpoint inhibitor (ICI) therapy often experience debilitating side-effects that impact PA participation. Because PA experiences in patients with advanced cancer undergoing ICI therapy are not well known, in this study we aimed to explore their perceptions and lived experiences. METHOD: Twelve adults with advanced urological (bladder or renal) cancer undergoing ICI therapy at a tertiary cancer centre, London, UK, participated in individual se

Prioritising topics for the undergraduate ENT curriculum.

OBJECTIVE: Knowledge of ENT is important for many doctors, but undergraduate time is limited. This study aimed to identify what is thought about ENT knowledge amongst non-ENT doctors, and the key topics that the curriculum should focus on. METHODS: Doctors were interviewed about their views of ENT knowledge amongst non-ENT doctors, and asked to identify key topics. These topics were then used to devise a questionnaire, which was distributed to multiple stakeholders in order to identify the key topics. RESULTS: ENT knowledge was generally thought to be poor amongst doctors, and it was recomme

Motivational interviewing in medical care settings: a systematic review and meta-analysis of randomized controlled trials.

OBJECTIVE: Motivational Interviewing (MI) is a method for encouraging people to make behavioral changes to improve health outcomes. We used systematic review and meta-analysis to investigate MI's efficacy in medical care settings. METHODS: Database searches located randomized clinical trials that compared MI to comparison conditions and isolated the unique effect of MI within medical care settings. RESULTS: Forty-eight studies (9618 participants) were included. The overall effect showed a statistically significant, modest advantage for MI: Odd ratio=1.55 (CI: 1.40-1.71), z=8.67, p<.001. MI s

An Exploration of the Impact of Group Treatment for Aphasia on Connected Speech.

OBJECTIVE: Group treatment enables people with aphasia to practise communication skills outside the typical clinician-patient dyad. While there is evidence that this treatment format can improve participation in everyday communication, there is little evidence it impacts linguistic abilities. This project aimed to investigate the effects of 'typical' group treatment on the communication skills of people with aphasia with a focus on word retrieval in discourse. METHODS: Three people with aphasia took part in a 6-week group therapy programme. Each week focused on a different topic, and three to

Whole Exome Sequencing in South Africa: Stakeholder Views on Return of Individual Research Results and Incidental Findings.

The use of whole exome sequencing (WES) in medical research is increasing in South Africa (SA), raising important questions about whether and which individual genetic research results, particularly incidental findings, should be returned to patients. Whilst some commentaries and opinions related to the topic have been published in SA, there is no qualitative data on the views of professional stakeholders on this topic. Seventeen participants including clinicians, genomics researchers, and genetic counsellors (GCs) were recruited from the Western Cape in SA. Semi-structured interviews were cond

The role of Advanced Practice Providers in improving primary care capacity: practice perspectives on challenges and opportunities.

BACKGROUND: In Virginia and across the United States, primary care workforce shortages limit access to care and exacerbate adverse health outcomes. We conducted a statewide survey of primary care practices in 2022 and found that 42% of practices reported losing a physician or Advanced Practice Provider (APP), including Nurse Practitioners (NPs) and Physician Assistants (PAs), in the past year. Adding APPs to primary care teams is one strategy practices are using to mitigate the shrinking availability of primary care physicians. However, APPs face workforce challenges that increase their suscep

Older people living alone (OPLA) - non-kin-carers' support towards the end of life: qualitative longitudinal study protocol.

BACKGROUND: A growing number of older people, mainly women, live in single households. They represent a vulnerable group as staying at home may turn out challenging when care needs increase, particularly at the end of life. Non-kin-carers can play an essential role in supporting individuals' preferences to stay at home. In research little attention has been paid to non-kin-carers, such as friends and neighbors, yet. Thus, the Older People Living Alone (OPLA) study will evaluate whether non-kin support is robust enough to enable care dependent people to stay at home even at the end of life. Thi

Presymptomatic genetic testing in children for neurofibromatosis 2.

Genetic testing in children, when there is a question of whether or not there is a clear medical benefit that will accrue to the child, is a controversial topic within the health care community. A convenience sample of 10 parents from nine families who had made the decision whether or not to test their children for the neurofibromatosis 2 gene mutation was asked in interviews to describe why they made their choice about presymptomatic testing for this late-onset disease. Findings from a narrative analysis revealed how the nine parents who tested or intended to test their young children saw the

Upper extremity prosthesis user perspectives on unmet needs and innovative technology.

The needs of individuals with upper limb amputation and congenital limb difference are not being fully met by current prostheses, as evidenced by prosthesis rejection, non-wear, and user reports of pain and challenging activities. Emerging technologies such as dexterous sensorized robotic limbs, osseointegrated prostheses, implantable EMG electrodes, and electrical stimulation for sensory feedback have the potential to address unmet needs, but pose additional risks. We plan to assess upper limb prosthesis user needs and perspectives on these new benefits and risks using an extensive quantitati

Current guidelines for the diagnosis of testosterone deficiency.

Hypogonadism in males is a clinical syndrome complex which comprises symptoms with or without signs as well as biochemical evidence of testosterone deficiency. The diagnosis of hypogonadism thus includes both clinical history and examination as well as biochemical assessment of serum testosterone levels. Hypogonadal symptoms depend on the age at onset of hypogonadism, severity of the deficiency, its duration and sensitivity to androgen action. Prepubertal onset results in lack of virilization and pubertal development and produces features such as eunuchoid body proportions and undeveloped seco
