Prophylaxis of postoperative endophthalmitis after cataract surgery: results of the 2007 ASCRS member survey.

An online survey of members of the American Society of Cataract and Refractive Surgery indicated a strong preference for preoperative and postoperative topical antibiotic prophylaxis, with most surgeons favoring latest generation topical fluoroquinolones. A significant percentage of surgeons reported being concerned about the risks of homemade intracameral antibiotic preparations, and there was a strong desire to have a commercially available antibiotic approved for intracameral injection. This is reflected in the fact that 77% of respondents were still not injecting intracameral antibiotics,

Induction of anterior chamber-associated immune deviation (ACAID) by allogeneic intraocular tumors does not require splenic metastases.

Anterior chamber-associated immune deviation (ACAID), induced by intracameral injection of allogeneic tumor cells, is expressed in three distinct ways: 1) progressive growth of intraocular tumors, 2) specific suppression of systemic allograft immunity, and 3) transient growth of allogeneic tumors injected subcutaneously. Induction of ACAID requires that alloantigen presentation occur via the anterior chamber; injection by other routes failed to elicit this phenomenon. Antigenic material must remain in the anatomically intact eye for at least 10 days; removal of the injected dye before this tim

Descemet's Membrane Detachment Following Inadvertent Intrastromal Injection of an Ophthalmic Viscoelastic Device.

Hypotony is a well-documented postoperative complication of glaucoma filtration surgery. Management may include the injection of an ophthalmic viscoelastic device (OVD) into the anterior chamber. This case report describes a 45-year-old woman with juvenile-onset open-angle glaucoma who developed clinical hypotony after trabeculectomy surgery augmented with mitomycin-C. An intracameral injection of Healon5® (Johnson & Johnson Vision, USA) was attempted to restore physiological intraocular pressure (IOP); however, Healon5® was inadvertently introduced into the corneal stroma, causing a localised

ESCRS guideline for cataract surgery 2024: executive summary.

Cataract is a leading cause of visual impairment globally, with surgery as the only effective treatment. Management involves a care pathway, including screening, patient selection, preoperative assessment, surgery, and postoperative care. The ESCRS developed evidence-based guidelines to aid decision-making for patients, clinicians, and stakeholders. A multidisciplinary panel, supported by methodologists using the Grading of Recommendations, Assessment, Development, and Evaluations approach, addressed 32 key clinical questions. Recommendations include intracameral antibiotics to prevent endopht

Magnusiomyces capitatus: a new and emerging pathogen linked to keratomycosis.

We describe our experience managing a case of fungal keratitis in a 71-year-old immunocompetent woman. Intracameral cultures isolated Magnusiomyces capitatus, a dimorphic commensal microbe with limited pathogenicity, in an immune competent patient. Repeated corneal cultures for bacteria and fungus were negative. Initial broad-spectrum topical fortified antibiotics were eventually escalated to include topical natamycin and voriconazole and oral voriconazole. The patient underwent emergent corneal transplantation, but recurrent infection eventually required anterior chamber washout and intracame

Treatment of acute corneal hydrops with intracameral C3F8 in a patient of pellucid marginal degeneration with keratoglobus.

We report the use of perfluoropropane (C(3)F(8)) gas for the treatment of acute hydrops with intrastromal cleft in a case of pellucid marginal degeneration with keratoglobus. Acute hydrops in a 36-year-old man with pellucid marginal degeneration with keratoglobus was treated with intracameral injection of 0.3 mL isoexpansile C(3)F(8) (14%). Reduction in corneal haze was seen beginning from the third postoperative day. Significant clearing had taken place by the end of the second week with the closure of intrastromal cleft. Best corrected visual acuity improved from finger counting at 1 foot pr

Use of recombinant tissue plasminogen activator for treatment of recalcitrant anterior uveitis: A case series.

Management of inflammation after surgery for recalcitrant anterior uveitis is challenging. Herein, we report successful treatment using intracameral injection of recombinant tissue plasminogen activator (rtPA) in two patients with recalcitrant anterior uveitis, due to infective uveitis and Vogt-Koyanagi-Harada disease, respectively. A 40-year-old woman presented with bilateral redness and vision reduction that had persisted 2 weeks. She also had bilateral anterior uveitis, vasculitis, retinitis, and optic disc swelling. Serology was positive for Bartonella henselae and Toxoplasma gondii. She w

Intracameral antibiotics during cataract surgery: efficacy, safety, and cost-benefit considerations.

PURPOSE OF REVIEW: We summarize evidence-based considerations regarding the use of intracameral antibiotics during cataract surgery. RECENT FINDINGS: The use of intraoperative intracameral antibiotics reduced the incidence of postcataract surgery endophthalmitis 3.5-fold, with an odds ratio ranging from 0.14 to 0.19. A survey of the American Society of Cataract and Refractive Surgery showed usage of intracameral injections of antibiotics increased by 16% in the United States between 2014 and 2021. The frequency of vancomycin usage has sharply dropped to 6%, while moxifloxacin is now the domin

Effects of prostaglandin E2 and cholinergic drugs on intraocular pressure.

Effects of prostaglandin E2 (PGE2) and cholinergic drugs on intraocular pressure (IOP) have been studied in the rabbit. PGE2 injected intracamerally in a dose of 5 micrograms produced a long-lasting increase in IOP. Topical pretreatment with carbachol, physostigmine and pilocarpine abolished the rise of IOP after the intracameral injection of PGE2. When the IOP had already been increased by PGE2 topical application of pilocarpine and physostigmine did not change it. Carbachol even potentiated the pressor effect of PGE2. Topical application of cholinergic drugs only slightly increased protein c

Effect of anterior chamber air tamponade after phacoemulsification cataract surgery: a systematic review and meta-analysis.

BACKGROUND: This study aimed to evaluate the safety of anterior chamber air tamponade post-phacoemulsification on the cornea compared to balanced salt solution or Ringer's lactate filling, and summarize its potential influence on the aspects like the risk of postoperative endophthalmitis and Descemet's membrane detachment (DMD). METHODS: This meta-analysis was conducted using Review Manager 5.4 and included studies that evaluated post-phacoemulsification air tamponade compared with no air tamponade before October 2025. Methodological quality was assessed using the Cochrane Risk of Bias tool.

[Complications of intracameral cefuroxime in cataract surgery].

Intracameral injection of cefuroxime during phacoemulsification is considered effective in preventing endophthalmitis. Its widespread usage has led to the accumulation of data on local and systemic adverse reactions. The analysis of complications of cataract surgery with cefuroxime injections described in literature has allowed identifying a few different clusters of toxic and allergic reactions to intracameral cefuroxime for the first time. These clusters included toxic anterior segment syndrome, serous retinal detachment with macular edema, retinal hemorrhagic infarction and anaphylactic rea

Three cases of Descemet's membrane detachment after cataract surgery.

Descemet's membrane detachment (DMD) is an uncommon condition with a wide range of etiologies. More than likely, the most common cause is a localized detachment occurring after cataract surgery. We report three cases of Descemet's membrane detachment that occurred after uncomplicated phacoemulsification cataract surgeries. The first patient was managed without surgical intervention, the second patient was treated using an intracameral air injection, and the last patient was treated with an intracameral perfluoropropane (C3F8) gas injection. All three patients recovered their vision following t

Cohort study of intracameral moxifloxacin in postoperative endophthalmitis prophylaxis.

We conducted a cohort study to evaluate post-cataract surgery endophthalmitis rates in relation to prophylactic intracameral moxifloxacin administration. A total of 2332 patients (2674 eyes) who underwent phacoemulsification by a single surgeon from January 2007 through December 2012 were included in the study. A total of 1056 eyes did not receive intracameral prophylactic moxifloxacin and the antibiotic was injected in 1618 eyes. The incidence of presumed postoperative endophthalmitis in the 2 groups was calculated. The rate of presumed infectious endophthalmitis after cataract surgery betwee
