OBJECTIVE: Managing patients on oral anticoagulant therapy (OAC) who present with traumatic intracranial hemorrhage (ICH) poses a critical challenge in balancing the prevention of thromboembolic events and the risk of progression or recurrent ICH. The objective of this systematic review was to determine the optimal timing for resuming OAC in patients with traumatic ICH, and to assess the risk of hemorrhagic and thromboembolic events, and mortality in patients for whom anticoagulation was resumed.
DESIGN: With a medical librarian, 4 databases and gray literature were searched without language

BACKGROUND: Traumatic intracranial hemorrhage (ICH) is a major driver of traumatic brain injury-related mortality. Although plasma transfusion has been associated with improved survival in these patients, the timing of transfusion influences outcomes is unclear. We sought to determine how plasma transfusion timing affects survival in patients with tICH, hypothesizing that earlier transfusion improves outcomes and that there is a threshold after which survival benefits diminish.
METHODS: We conducted a retrospective analysis of adult trauma patients (≥18 y) with tICH who received plasma-based

OBJECTIVES: Patients with mechanical heart valves require lifelong anticoagulation, exposing them to a substantially elevated risk of hemorrhagic complications following traumatic injury. Traumatic brain injury (TBI) in this population represents a particularly challenging clinical dilemma, as the coexistence of intracranial hemorrhage and the obligatory need for anticoagulation places competing physiologic imperatives in direct conflict. Despite the clinical relevance of this scenario, data on the incidence, management, and outcomes of TBI in anticoagulated mechanical valve patients remain sc

OBJECTIVE AND BACKGROUND: This study aims to better characterize the utility of CT scan imaging when seeing mild traumatic brain injury (TBI) patients managed non-operatively in a clinic. The benefit of routinely scheduling outpatient head CT for patients discharged with mild TBI and intracranial hemorrhage (ICH) remains unclear. Unselective imaging increases cost, scanner demand, and cumulative radiation exposure.
METHODS: We performed a retrospective cohort study of 100 consecutive adults with mild TBI (Glasgow coma scale (GCS) 13-15) and non‑operative ICH who were admitted between January

BACKGROUND: Plasma-based resuscitation improves outcomes in trauma patients with hemorrhagic shock, while large-animal and limited clinical data suggest that it also improves outcomes and is neuroprotective in the setting of combined hemorrhage and traumatic brain injury. However, the choice of initial resuscitation fluid, including the role of plasma, is unclear for patients after isolated traumatic brain injury.
METHODS: We reviewed adult trauma patients admitted from January 2011 to July 2015 with isolated traumatic brain injury. "Early plasma" was defined as transfusion of plasma within 4

BACKGROUND: Prediction rules for computed tomography (CT) use in mild traumatic brain injury (MTBI) often have limited generalizability due to restrictive inclusion criteria and development in high-income countries. A clinical-based prediction score was recently developed for a broader MTBI population in a low- to middle-income country. This study aims to externally validate this score in a new patient dataset.
METHODS: We prospectively collected data from adult patients with MTBI (Glasgow Coma Scale score 13-15) presenting to a hospital in Thailand between 2014 and 2016. The performance of t

OBJECTIVE: To measure the association between antithrombotic (AT) medications (anticoagulant and antiplatelet) and risk for traumatic intracranial hemorrhage (ICH) in older adults with a mild traumatic brain injury (mTBI).
METHODS: We conducted a retrospective multicenter study across 103 emergency departments affiliated with a teleradiology company dedicated to emergency imaging between 2020 and 2022. Older adults (≥65 years old) with mTBI, with a head computed tomography scan, were included. Natural language processing models were used to label-free texts of emergency physician forms and ra

The utility of 4-factor prothrombin complex concentrate (4F-PCC) for reversal in patients on factor Xa inhibitors (XaI) is unclear, specifically in mild traumatic brain injury (mTBI). This is a retrospective review over 6 years at a level 1 trauma center of patients presenting with mTBI on XaI comparing outcomes for those that received 4F-PCC to those that did not. 140 patients were included, 103 (74%) of these patients received 4F-PCC while 37 (26%) did not. There was no significant difference in neurologic decline within 48 hours of admission or need for neurosurgical intervention. Interesti

BACKGROUND: Pediatric patients with any severity of traumatic intracranial hemorrhage (tICH) are often admitted to intensive care units (ICUs) for early detection of secondary injury. We hypothesize that there is a subset of these patients with mild injury and tICH for whom ICU care is unnecessary.
OBJECTIVES: To quantify tICH frequency and describe disposition and to identify patients at low risk of inpatient critical care intervention (CCI).
METHODS: We retrospectively reviewed patients aged 0 to 17 years with tICH at a single level I trauma center from 2008 to 2013. The CCI included mecha

Endovascular therapy (EVT) is an effective treatment for large vessel occlusion, including carotid artery dissection (CAD). However, when large vessel occlusion (LVO) occurs in acute ischemic stroke (AIS) patients with concomitant intracranial hemorrhage (ICH), it can be difficult for clinicians to make a treatment decision. We report the case of a patient in their 60s who was admitted due to sudden right limb weakness. Emergency computed tomography (CT)/CTA showed a hematoma in the right parietal lobe and a possible dissection of the left internal carotid artery. The patient underwent emergen

OBJECTIVE: Current guidelines do not address the disposition of patients with mild traumatic brain injury (TBI) and resultant intracranial hemorrhage (ICH). Emergency medicine clinicians working in hospitals without neurosurgery coverage typically transfer patients with both to a trauma center with neurosurgery capability. Evidence is accruing which demonstrates that the risk of neurologic decompensation depends on the type of ICH and as a result, not every patient may need to be transferred. The purpose of this study was to identify risk factors for admission among patients with mild TBI and

BACKGROUND: Dysglycemia is frequently observed in patients with non-traumatic intracranial hemorrhage (ICH) and has been linked to adverse clinical outcomes. However, the prognostic impact of longitudinal blood glucose trajectories in critically ill non-traumatic intracranial hemorrhage remains incompletely understood.
METHODS: We conducted a retrospective cohort study using the MIMIC-IV database (United States, 2008-2019), including 2,260 critically ill non-traumatic ICH patients. Latent class mixed modeling (LCMM) was employed to identify distinct longitudinal blood glucose trajectory subty

Trauma patients with traumatic intracranial hemorrhage (tICH) who require extracorporeal membrane oxygenation (ECMO) present a difficult therapeutic dilemma. Early anticoagulation may worsen intracranial bleeding, whereas prolonged anticoagulation-free ECMO may increase thrombotic complications. This narrative review examines the time-dependent balance between these competing risks and proposes a conceptual time-dependent risk framework for anticoagulation initiation. A focused search of PubMed, Embase, and the Cochrane Library identified studies on the temporal profile of hemorrhagic progress

OBJECTIVE: Acute kidney injury (AKI) is a common and serious complication in traumatic intracranial hemorrhage (tICH), leading to worse outcomes. Blood pressure variability (BPV), beyond mean blood pressure levels, may impair end-organ perfusion and predispose to AKI. However, the relationship between BPV and AKI in tICH patients remains poorly defined. We investigated the association between BPV and AKI in patients with tICH.
METHODS: We conducted a retrospective cohort study of tICH patients from the Medical Information Mart for Intensive Care IV (MIMIC-IV) database. Inclusion required ≥24

BACKGROUND: The role of bridging anticoagulation with unfractionated heparin (UFH) in traumatic brain injury (TBI) patients with mechanical heart valves (MHV) remains uncertain. While bridging has traditionally been used to reduce thromboembolic risk, emerging evidence suggests it may increase bleeding complications without clear benefit. This study evaluates the impact of bridging and anticoagulation resumption timing on clinical outcomes. METHODS: A retrospective observational study was conducted at Memorial Hermann–Texas Medical Center, including 44 adult patients with MHV and acute TBI pre

BACKGROUND. Antithrombotic medications may increase the risk and severity of traumatic intracranial hemorrhage (tICH) after minor head trauma. OBJECTIVE. The purpose of this study was to determine the frequency, distribution, and clinical course of tICH in patients receiving antithrombotic therapy who present with good neurologic status after a ground-level fall. METHODS. This retrospective study included 1630 patients (693 women and 937 men; mean age, 80.2 ± 12.7 [SD] years) who underwent head CT after presenting to the emergency department after a ground-level fall between January 1 and Dece

BACKGROUND: Mild traumatic brain injury (TBI) is a common event and antiplatelet therapy might represent a risk factor for bleeding.
OBJECTIVE: The aim of this study was to evaluate the risk of intracranial hemorrhage (ICH) after mild TBI in patients on antiplatelet therapy through a systematic review and meta-analysis.
METHODS: We conducted a systematic review and meta-analysis of prospective and retrospective observational studies on patients with mild TBI on antiplatelet therapy vs. those not on any antithrombotic therapy. The primary outcome was the risk of ICH in patients with mild TBI

BACKGROUND: Assessing the risk of intracranial hemorrhage (ICH) in patients with a mild traumatic brain injury (MTBI) who are taking direct oral anticoagulants (DOACs) is challenging. Currently, extensive use of computed tomography (CT) is routine in the emergency department (ED).
OBJECTIVE: This study aims to investigate whether the clinical and laboratory characteristics presented at the ED evaluation can also estimate the risk of post-traumatic ICH in DOAC-treated patients with MTBI.
METHODS: A retrospective observational study was conducted in three EDs in Italy from January 1, 2016 to M

BACKGROUND: Traumatic intracranial hemorrhaging associated with revascularization surgery for moyamoya vasculopathy is a potentially devastating problem that requires meticulous management, including surgery. However, only a few studies on this subject have been reported, and the clinical characteristics are poorly understood. We report a case of successful surgical management for a patient with traumatic intracranial hematoma managed with encephalo-duro-arterio-myo-synangiosis (EDAMS). The purpose of this article is to clarify the specific features of clinical scenarios, hemorrhagic sites, an
