OBJECTIVE: Microsurgery is the preferred treatment for large vestibular schwannomas (VSs). However, anatomical and functional preservation of the facial nerve (FN) remains a challenge during this surgery. The aim of this study was to determine whether it is beneficial to the preservation rates of the FN during microsurgical treatment of large VSs using intraoperative neurophysiological monitoring (IONM).
METHODS: We retrospectively reviewed 53 patients with large VSs that underwent microsurgical resection via the retrosigmoid approach in our department during April 2009 to March 2016. IONM wa

Intraoperative neurophysiological monitoring (IONM) is becoming the standard of care for many spinal surgeries, especially those with deformity correction and instrumentation. We reviewed 2069 spine cases with multimodality IONM including somatosensory evoked potentials (SSEP), transcranial electrical motor evoked potentials (TCeMEP), and spontaneous and triggered electromyography (s-EMG and t-EMG) in a University setting over a period of four years to examine perioperative clinical findings when an IONM event was noted and to ascertain how IONM has affected our ability to avoid potential neur

Serious vascular injury is a rare, but potentially devastating complication during anterior lumbar spinal surgery. The authors describe the first reported case where vascular injury was detected by multimodality neurophysiological monitoring during an L3-S1 anterior lumbar interbody fusion. The case demonstrates the need for multi-modality monitoring and the combined use of somatosensory-evoked potentials and motor-evoked potentials.

This paper deals with the first 45 consecutive patients undergoing microvascular decompression (MVD) surgery for trigeminal neuralgia, studied with perioperative brainstem auditory evoked potentials (BAEPs) and electromyography (EMG). We observed a good correlation between the intraoperative BAEP modifications and postoperative hearing function. BAEP monitoring was useful in identifying the manoeuvres that may compromise cochlear nerve function. This improved the surgical technique in the subsequent cases and reduced the incidence of iatrogenic hearing deficits after the learning period. There

Electromyography (EMG) is used to assess the functional status of peripheral motor axons during surgical procedures and can help protect patients' neurological integrity intraoperatively. This is a position statement on intraoperative EMG monitoring from the American Society of Neurophysiological Monitoring (ASNM) and updates prior ASNM position statement on EMG from 2005. This position statement is endorsed by ASNM and serves as an educational service to the neurophysiological community on the recommended use of EMG as a neurophysiological monitoring tool. It presents the rationale for EMG ut

BACKGROUND: Remimazolam is a novel short-acting benzodiazepine. We investigated the effects of low doses of volatile anesthetic agents on motor evoked potentials (MEPs) and somatosensory evoked potentials (SSEPs) during remimazolam-remifentanil anesthesia.
METHODS: Thirty-nine patients undergoing cervical spine surgery were randomly assigned to either the sevoflurane (n = 20) or desflurane (n = 19) groups. Volatile anesthetic agents were administered at 0.3, 0.6, and 0.8 minimum alveolar concentrations (MACs) during remimazolam-remifentanil anesthesia. Significant changes were considered as m

Intraoperative neurophysiological monitoring during endoscopic, endonasal approaches to the skull base is both feasible and safe. Numerous reports have recently emerged from the literature evaluating the efficacy of different neuromonitoring tests during endonasal procedures, making them relatively well-studied. The authors report on a comprehensive, multimodality approach to monitoring the functional integrity of at risk nervous system structures, including the cerebral cortex, brainstem, cranial nerves, corticospinal tract, corticobulbar tract, and the thalamocortical somatosensory system du

There are very few reports on the effects of benzodiazepines such as midazolam and diazepam on intraoperative visual-evoked potential (VEP), and there is no report on the effect of remimazolam at all. Five patients underwent neurosurgery using VEP monitoring for avoiding surgical injury to the optic nerve. In all cases, drug administration was based on actual body weight. General anesthesia was induced with propofol and remifentanil, and then maintained with propofol at target concentrations of 2.7-3.5 µg/ml for maintaining bispectral index (BIS) between 40 and 60. After resection of the tumor

Intraoperative spinal cord monitoring consists in a subcontinuous evaluation of spinal cord sensory-motor functions and allows the reduction the incidence of neurological complications resulting from spinal surgery. A combination of techniques is used: somatosensory evoked potentials (SSEP), motor evoked potentials (MEP), neurogenic motor evoked potentials (NMEP), D waves, and pedicular screw testing. In absence of intraoperative neurophysiological testing, the intraoperative wake-up test is a true form of monitoring even if its latency long and its precision variable. A 2011 survey of 117 Fre

Iatrogenic spinal cord injury is the most feared complication of scoliosis surgery. The importance of combined somatosensory evoked potentials (SEP) and motor evoked potentials (MEP) monitoring during spine surgery is well known. The current authors retrospectively evaluated the results of neurophysiological intraoperative monitoring (IOM) in a large population of patients who underwent surgical treatment for spinal deformity. Intraoperative monitoring of SEPs and transcranial electrical stimulation MEPs (TES-MEP) was performed in 172 successive patients who underwent surgical treatment of idi

BACKGROUND: Neurophysiologic intraoperative monitoring (NIM) has gone through a renaissance since the advent of computer technology. Currently, both motor and sensory cranial nerves, including the IInd and VIIIth cranial nerves, can be intraoperatively controlled by means of small and mobile systems.
PATIENTS AND METHODS: In order to estimate the value of NIM using the new generation of computer systems, we analyzed the records of 379 patients who underwent skull base surgery since 1996. These comprised NIM of the IInd, VIIth, VIIIth cranial nerves in most cases and of the IXth, Xth, XIth, an

Intraoperative neurophysiological monitoring (IONM) has achieved popularity because it facilitates monitoring of the functional integrity of neural structures under general anesthesia. It aids in the early detection of injury and minimizes postoperative neurologic deficit or neurologic morbidity from surgical manipulations of various neurologic structures. The patient mentioned in this case report presented with lower limb radiculopathy and was diagnosed with diastematomyelia Type II, and she was planned for surgical intervention under general anesthesia. Preoperatively, a team of surgeons, an

OBJECTIVE: Dysphagia represents the main complication of posterior fossa neurosurgery. Adequate diagnosis of this complication is warranted to prevent untimely extubation with subsequent aspiration. Intraoperative neurophysiologic monitoring (IONM) modalities may be used for this purpose. However, it is not known which IONM modality may be significant for diagnosis. This study aimed to define the most significant IONM modality for dysphagia prognostication after posterior fossa neurosurgery.
METHODS: The analysis included 46 patients (34 with tumors of the fourth ventricle and 12 with brainst

Intraoperative neuromonitoring was introduced in the second half of the 20th century with the goal of preventing patient morbidity for patients undergoing complex operations of the central and peripheral nervous system. Since its early use for scoliosis surgery, the growth and utilization of IOM techniques expanded dramatically over the past 50 years to include spinal tumor resection and evaluation of cerebral ischemia. The importance of IOM has been broadly acknowledged, and in 1989, the American Academy of Neurology (AAN) released a statement that the use of SSEPs should be standard-of-care

BACKGROUND: Intraoperative neurophysiologic monitoring has become a standard tool for mitigating neurologic injury during spinal deformity surgery. Significant monitoring changes during deformity correction are relatively uncommon. This study characterizes precipitating factors for neurologic injury and relates significant events and postoperative neurologic prognosis.
METHODS: All spinal deformity surgeries at a West African hospital over a 12-month period were reviewed. Patients were included if complete operative reports, monitoring data, and postoperative neurologic examinations were avai

Neurophysiologic intraoperative monitoring of the brainstem auditory evoked potentials (BAEPs) is a widely used method to assess the functional integrity of the central auditory system during surgery involving the brainstem or the cranial nerves. The purpose of this study is to describe our experience with neurophysiologic intraoperative monitoring of BAEPs during posterior fossa decompression (PFD) surgery for the management of Chiari I malformation. Although suboccipital craniectomy is the standard surgical technique applied in all cases undergoing PFD, the role of dural patch grafting (dura

PURPOSE: Intraoperative neurophysiological monitoring (IONM) is a tool that can improve safety during spinal surgery. Patients with neuromuscular scoliosis (NMS) have inherent neural axis or primary muscle pathology that may present challenges to neuromonitoring. An understanding of IONM event rates, patterns, and outcomes would better prepare surgeons to counsel patients and families.
METHODS: IONM data from pediatric spine deformity correction procedures in patients with neuromuscular scoliosis at a single quaternary care center between 2012 and 2022 were reviewed to determine cases with mo
