A review of Janus kinase inhibitors for the treatment of Covid-19 pneumonia.

BACKGROUND: In inflamed tissue, immune cells are accumulated, and various intercellular signals are involved in the pathogenesis. Janus kinases (JAKs) are typical tyrosine kinases involved in mediating the signaling of multiple cytokines and growth factors and induce the transcription of molecules related to inflammation or immunity via the transcription factor signal transducers and activators of transcription (STAT). Hence, they have garnered significant interest as a therapeutic target. JAK inhibitors have been evaluated as a major drug for remission induction in the treatment of autoimmune

Janus kinase (JAK) inhibitors in the treatment of neoplastic and inflammatory disorders.

The Janus kinase (JAK) family of nonreceptor protein-tyrosine kinases consists of JAK1, JAK2, JAK3, and TYK2 (Tyrosine Kinase 2). Each of these proteins contains a JAK homology pseudokinase (JH2) domain that interacts with and regulates the activity of the adjacent protein kinase domain (JH1). The Janus kinase family is regulated by numerous cytokines including interferons, interleukins, and hormones such as erythropoietin and thrombopoietin. Ligand binding to cytokine receptors leads to the activation of associated Janus kinases, which then catalyze the phosphorylation of the receptors. The S

Janus Kinase inhibitors for the treatment of hospitalized patients with COVID-19.

PURPOSE OF REVIEW: Janus Kinase (JAK) inhibitors have been successfully utilized in the clinical treatment of several rheumatologic (e.g. rheumatoid arthritis) and inflammatory diseases (e.g. hemophagocytic lymphohistiocytosis). Based on the growing evidence that moderate and severe COVID-19 infections are associated with a dysregulated inflammatory state, this class of medications has been repurposed as a potential therapy for COVID-19, an infection caused by Severe Acute Respiratory Syndrome Coronavirus 2. RECENT FINDINGS: Three JAK inhibitors have been evaluated in human studies of COVID-1

[Response of dermatomyositis with lung involvement to Janus kinase inhibitor treatment].

We report on a 32-year-old male patient presenting with anti-MDA-5 and anti-Ro52 antibody positive hypomyopathic dermatomyositis (CADM) with clinically leading interstitial pulmonary involvement. Under several immunosuppressive treatment regimens including high-dose steroids, cyclophosphamide, rituximab, immunoglobulins, plasmapheresis, ciclosporin and mycophenolate mofetil, pulmonary involvement was refractory to progressive. Based on the detection of a clear-cut interferon signature by flow cytometric determination of SIGLEC-1 as an interferon-dependent marker, treatment with the Janus kinas

Janus kinase inhibitors in the management of lichen planus: a systematic review.

BACKGROUND AND OBJECTIVES: Lichen planus (LP) is a chronic, lymphocytic, inflammatory skin disorder that can affect the skin, hair follicles, nails, and mucous membranes. It has limited treatment options. The pathogenesis of LP includes inflammation driven by interferons and the Janus kinase-signal transducer and activator of transcription (JAK-STAT) signaling pathway. Thus, Janus kinase inhibitors (JAKi) have been proposed as a viable therapeutic approach for LP. JAKi have recently been approved to treat some lymphocytic skin disorders such as alopecia areata and atopic dermatitis. The purpos

[The application of Janus kinase inhibitors for the common pediatric dermatological disorders].

Janus kinase(JAK) inhibitors can effectively regulate excessive immune and inflammatory responses by targeting and blocking the JAK-signal transducers and activators of transcription pathway. This provides an important direction for breaking through the traditional limitations in the treatment of immune-related skin diseases in children. This article reviews the mechanism of action of JAK inhibitors and the approved indications, and systematically summarizes the current application status of these inhibitors in the treatment of pediatric dermatological disorders. In common pediatric skin disea

Treatment of Livedoid Vasculopathy with the Janus Kinase Inhibitors: A Case Series.

INTRODUCTION: Livedoid vasculopathy (LV) is a rare chronic recurrent occlusive disease characterized by painful ulcers and atrophic porcelain-white scars. Owing to the lack of strict treatment guidelines, the commonly used traditional treatment methods are not effective in some patients. Therefore, there is an urgent need to develop new therapies for LV. CASE PRESENTATIONS: Here we report 4 cases of LV that were successfully treated with the Janus kinase (JAK) inhibitors. The present study revealed a significant improvement in clinical activity and pain relief in patients with LV after the ad

[The role of Janus kinase inhibitors in the treatment of juvenile idiopathic arthritis].

Juvenile idiopathic arthritis (JIA) presents ongoing treatment challenges despite advances in treatment over the last few years. This review presents that recent studies highlight the potential of Janus kinase inhibitors (JAKi) to significantly reduce disease activity. However, concerns about long-term safety, including cardiovascular risks and malignancy, underscore the need for more research. While JAKi offers a promising therapeutic avenue in pediatric rheumatology, the long-term safety profile requires further investigation to assert its place in standard treatment guidelines.

Janus kinase inhibitors for the treatment of lichen Sclerosus: A systematic review.

AIM: The current treatment options for lichen sclerosus (LS) remain limited. We aimed to systematically assess the evidence on the effects of Janus kinase (JAK) inhibitors in treating LS. METHODS: We performed a systematic review and searched PubMed, Cochrane, Embase and the Airiti Library from inception to 16 January 2025. As we expected a lack of relevant randomized trials, we also included relevant single-arm trials, case reports and case series. The risk of bias of included case reports and case series was evaluated using Murad's tool, while single-arm trials were assessed using Alsinbili

Direct Effects of the Janus Kinase Inhibitor Baricitinib on Sensory Neurons.

Therapeutically, the Janus kinase (Jak) 1/Jak2 inhibitor baricitinib reduces the pathology of rheumatoid arthritis and may also reduce pain. Here, we investigated whether baricitinib directly affects joint nociceptors. We recorded action potentials from nociceptive C- and A∂-fibers of the normal and inflamed knee joint in anesthetized rats to monitor their responses to innocuous and noxious joint rotation. In isolated and cultured dorsal root ganglion (DRG) neurons, we examined Stat3 activation using Western blots and monitored excitability using patch-clamp recordings. Intra-articular injecti

Janus Kinase Inhibitors: A New Tool for the Treatment of Axial Spondyloarthritis.

Axial spondyloarthritis (axSpA) is a chronic inflammatory disease involving the spine, peripheral joints, and entheses. This condition causes stiffness, pain, and significant limitation of movement. In recent years, several effective therapies have become available based on the use of biologics that selectively block cytokines involved in the pathogenesis of the disease, such as tumor necrosis factor-α (TNFα), interleukin (IL)-17, and IL-23. However, a significant number of patients show an inadequate response to treatment. Over 10 years ago, small synthetic molecules capable of blocking the a

The Use of Janus Kinase Inhibitors in Alopecia Areata: A Review of the Literature.

Alopecia areata (AA) is a chronic, immune-mediated disorder that targets hair follicle epithelium, thereby restricting hair growth in localized patches. Although several therapies for AA have been tested, responses with traditional therapies have been limited. In recent years, numerous reports have been published of patients with AA responding to Janus kinase (JAK) inhibitors. This literature review aims to describe AA pathophysiology, explore how and why JAK inhibitors can be used for AA treatment, and review published case reports, case series, and open-label studies published to date.
