Knee surgery, sports traumatology, arthroscopy : official journal of the ESSKA2012eng
The hip is a highly stable joint. Non-traumatic dislocation of the hip is extremely uncommon. In this article, we report two cases of non-traumatic hip dislocations following hip arthroscopy. In both cases, capsulotomy and ileopsoas tenotomy had been performed. These cases raise questions about the importance of the natural stabilisers of the hip. Level of evidence V.

Knee stiffness with severe flexion and extension impediment is a common clinical condition that challenges orthopaedic surgeons. Arthroscopic release techniques have been reported, mostly for slight extension or flexion deficits. However, for severe flexion- and extension-impeding knee stiffness, especially when there is no intra-articular space in which to place the arthroscope, effective arthroscopic release techniques are still being pursued. We introduce a systemic arthroscopic release technique to address this special condition, in which a trans-infrapatellar fat pad release technique is

Five revision total knee arthroplasties (TKAs) involving severe femoral bone loss were performed in 1994. Each had sufficiently severe femoral bone loss in which collateral ligament origins and posterior capsular attachments were violated. A paradoxical phenomenon was observed in each case. Unlike primary TKAs, in which larger distal femoral bone resection leads to laxity of the knee joint in extension, these cases with severe distal femoral bone loss, after initial component selection, developed the opposite situation, a flexion contracture. It was hypothesized that femoral bone loss involvin

The distal soft tissue procedure has evolved into an indispensable additional surgical procedure to increase the corrective effect in hallux valgus surgery. Considering the biomechanical development of hallux valgus deformity, degenerative changes of the soft tissues around the first metatarsophalangeal joint contribute much more to the deformity than changes in the bony structures which can rather be seen as degenerative changes secondary to the deformity. Thus the principles in hallux valgus correction should aim to reverse all pathogenetic steps leading to deformity: release of the contract

Correction of the hallux valgus deformity is dependent upon recognition of the specific anatomic deformity, followed by selection of a procedure that addresses the specific deformity. A hallux valgus deformity with a subluxated metatarsophalangeal joint has a soft tissue and bony component, both of which need to be corrected, or a recurrence may result. The distal soft tissue procedure addresses the lateral soft tissue contracture by releasing the adductor hallucis, lateral joint capsule and transverse metatarsal ligament. The capsule is medially elongated and the medial eminence needs to be e

Surgical exposure of the anterior column, anterior wall, pelvic brim, and sacroiliac joint is accessible through the lateral window of the ilioinguinal approach. Residual attachment of the abdominal muscles and inguinal ligament to the anterior superior iliac spine (ASIS) is often a limiting factor to expanded distal and medial exposure, especially in patients with a large abdomen that hangs over the pelvis. An ASIS osteotomy has been described to improve exposure, particularly of the distal anterior wall and joint capsule, pubic ramus, and anterior quadrilateral plate. However, an ASIS osteot

Adhesive capsulitis presents clinically as limited, active and passive range of motion caused by the formation of adhesions of the glenohumeral joint capsule. Radiographically, it is thickening of the capsule and rotator interval. The pathology of the disease, and its classification, relates to inflammation and formation of extensive scar tissue. Risk factors include diabetes, hyperthyroidism, and previous cervical spine surgery. Nonsurgical management includes physical therapy, corticosteroid injections, extracorporeal shock wave therapy, calcitonin, ultrasonography-guided hydrodissection, an

INTRODUCTION: Children affected by obstetric brachial plexus palsy have an internal rotation contracture of the shoulder and a deformed glenohumeral joint. In 2003, Pearl proposed doing an arthroscopic release of the shoulder to restore external rotation and allow the glenohumeral joint to remodel. The goal of the current study was to evaluate the active and passive shoulder external rotation range of motion and glenohumeral joint remodelling in children treated with arthroscopic-directed release.
MATERIALS AND METHODS: Between 2004 and 2010, 18 children with passive external rotation under 1

A 68-year-old woman underwent arthroscopic capsular release and manipulation under general anesthesia for adhesive capsulitis. Six hours postoperatively, she suffered an anterior glenohumeral joint dislocation with passive forward flexion while working with physical therapy. Postoperative workup revealed pseudocholinesterase deficiency. Pseudocholinesterase deficiency may increase the risk of atraumatic glenohumeral dislocation in the immediate postoperative period. Caution should be taken concerning early physical therapy in these patients to minimize the risk of dislocation.

Surgical exposures for complex injuries about the elbow are technically demanding because of the high density of neurologic, vascular, and ligamentous elements around the elbow. The posterior approaches (ie, olecranon osteotomy, triceps-reflecting, triceps-splitting, triceps-reflecting anconeus pedicle flap, paratricipital) include techniques used to navigate the area around the triceps tendon and anconeus muscle. These approaches may be extended to gain access to the entire joint. The ulnar nerve, the anterior and posterior capsules, and the coronoid process are addressed by means of a medial

The literature to find the best approach to correct a chronic mallet finger deformity has been reviewed. All the evidence we found was type IV mallet finger injury, based on the CEBM classification. In the European literature, if correction of the proximal interphalangeal joint is not needed, and surgery is to be done on the distal interphalangeal joint only, then the most frequently reported technique involves the conversion of the chronic injury into an acute one by excising the scar and part of the joint capsule, and the extensor tendon is reattached with minor variations. An 80% to 100% su

Open release of elbow stiffness via the lateral approach has proven to be a reliable option in the treatment of elbow stiffness over the last years. For evaluation of patient outcome after this procedure the current literature and studies were reviewed and analyzed with special reference to the increase in range of motion and patient satisfaction. It appears that particularly posttraumatic elbow stiffness with contracture of the capsule and minor alterations of the joint surface can achieve improvement in mobility and patient satisfaction. Therefore careful selection of patients is important f

The success rate of the operation of lateral release for pain caused by the patella is reported as being between 14% and 99%. The choice between arthroscopic or open procedures does not seem to affect the results. The wide ranges of results probably reflects differences in patients selection or the method and investigations of follow up. The early term outcome usually show better results than long term follow up. This study evaluates the indications for the operation of lateral releases and discusses the result of 36 out of a total of 42 patients who were follow up for 3 years later surgery. W

There has been much debate about the pathogenesis of atlantoaxial rotatory fixation. Intraarticular abnormality has not been well documented thus far. This is the first case of chronic atlantoaxial rotatory fixation in which atlantoaxial joints were examined by consecutive arthrography. A 7-year-old girl was diagnosed with atlantoaxial rotatory fixation on a three-dimensional CT scan. Arthrography of the lateral atlantoaxial joints indicated a rupture of the joint capsule on the dislocated side at first, followed by a successful repair after 5 weeks' immobilization with a halo apparatus.
