Progressive cubitus varus due to a bony physeal bar in a 4-year-old girl following a supracondylar fracture: a case report.

We report a case of progressive cubitus varus deformity caused by a physeal bar following a supracondylar humeral fracture in a 4-year-old girl. Malreduction is thought to be the commonest cause of this deformity, which is nonprogressive. A corrective osteotomy in cases like ours should be deferred until skeletal maturity.

[Medial rotation deformity of the hip in cerebral palsy: surgical treatment by transposition of gluteal muscles].

INTRODUCTION: Medial rotation deformity of the hip is a problem to patients who are handicapped by cerebral palsy but able to walk, because the knees point inward during gait ("kissing patellae") and cause falls and frequent injuries. Knees and ankles are subject to stress and, therefore, they assume compensating positions. Lower legs assume position of valgus and external rotation, whereas feet rotate either inwards or outwards. Secondary deformities make gait more difficult and cause rapid tear of footwear. AIM: The purpose of the paper was to retrospectively analyze the effects of transpos

Miscellaneous non-inflammatory musculoskeletal conditions. Jaccoud's arthropathy.

Jaccoud's arthropathy (JA) is a condition characterised clinically by 'reversible' joint deformities such as swan neck, thumb subluxation, ulnar deviation, 'boutonniere' and hallux valgus, along with an absence of articular erosions on a plain radiograph. JA was initially described in patients with rheumatic fever (RF), but as this disorder has become rare the main clinical entity associated to JA at present is systemic lupus erythematosus (SLE). JA has also been described in other connective tissue diseases, infections and neoplasia. In general, its prevalence in either SLE or RF is around 5%

The rheumatoid swan-neck deformity.

It is important to evaluate each swan-neck deformity to determine the mobility and radiographic condition of the PIP joint. This information forms a basis for logical treatment. In those fingers with little or no loss of motion, the treatment alternatives include DIP joint fusions, dermadesis, or flexor tenodesis. With significant loss of PIP joint motion, an attempt is made to first restore passive motion by manipulation and lateral band or skin releases. It then becomes essential to restore flexor tendon excursion. In those patients with destroyed joint surfaces the salvage procedures of fus

Hammertoe surgery: waist resection of the proximal phalanx, a more simplified procedure.

A technic for correcting hammertoe and other toe deformities by waist resection of the proximal phalanges of the toes effectively corrected these conditions in 95 patients and 466 toes. This procedure can be done quickly and easily with low morbidity. Through a very short incision over the waist of the proximal phalanx, a subperiosteal resection is carried out which permits settling of the resected bone ends together within the periosteal tube, resulting in bony union. The proximal phalanx is effectively shortened and the imbalance of the tendon length to the bony arch, which originally produc

Valgus ankle deformity and arthritis.

This article describes several conditions that may result in valgus ankle arthritis. The emphasis is on correction of pathology or deformity to prevent valgus arthritis from developing. The surgical techniques available for the treatment of this form of ankle arthritis, once it develops, are described also.

Naviculocuneiform arthrodesis.

The adult acquired flatfoot is a challenging clinical entity with numerous procedures available to the surgeon for correction of the deformity. In a supple stage II flatfoot, naviculocuneiform arthrodesis with appropriate ancillary soft tissue augmentation and other adjunctive osseous procedures has proven effective with reproducible success. It may be used to balance a symptomatic flatfoot, re-establishing medial column stability and alignment while maintaining essential tri-tarsal joint motion. In addition to a review of the evidence-based medicine regarding this procedure, a detailed surgic

[Pachydermodactyly: a report of two cases].

Pachydermodactyly is a superficial benign, symmetrical and painless fibromatosis, characterized by a swelling of the proximal interphalangeal joints of the fingers. Histological examination shows epidermal hyperplasia increase of dermal collagen bundles and an increased number of fibroblasts. We report two cases of pachydermodactyly in male teenagers. The aetiology remains unknown. Pachydermodactyly has to be distinguished from knuckle pads.

A cadaveric study of ulnar nerve strain at the elbow associated with cubitus valgus/varus deformity.

BACKGROUND: Cubital tunnel syndrome can be caused by overtraction and dynamic compression in elbow deformities. The extent to which elbow deformities contribute to ulnar nerve strain is unknown. Here, we investigated ulnar nerve strain caused by cubitus valgus/varus deformity using fresh-frozen cadavers. METHODS: We used six fresh-frozen cadaver upper extremities. A strain gauge was placed on the ulnar nerve 2 cm proximal to the medial epicondyle of the humerus. For the elbow deformity model, osteotomy was performed at the distal humerus, and plate fixation was performed to create cubitus val

Volar Plate Capsulodesis With Internal Brace Augmentation for the Claw Hand.

Claw hand deformity is characterized by hyperextension of the metacarpophalangeal joints and flexion of the proximal interphalangeal joints. Surgical treatment of claw hand deformities often involves soft tissue procedures such as tendon transfers or volar plate capsulodesis. However, tissue attenuation over time can lead to recurrence of the deformity. Augmentation with an internal brace may help to address this problem. This report presents a novel technique for the treatment of claw hand deformity involving augmentation of the traditional volar plate capsulodesis with internal brace. Two il

Procedure selection for the flexible adult acquired flatfoot deformity.

Adult acquired flatfoot represents a spectrum of deformities affecting the foot and the ankle. The flexible, or nonfixed, deformity must be treated appropriately to decrease the morbidity that accompanies the fixed flatfoot deformity or when deformity occurs in the ankle joint. A comprehensive approach must be taken, including addressing equinus deformity, hindfoot valgus, forefoot supinatus, and medial column instability. A combination of osteotomies, limited arthrodesis, and medial column stabilization procedures are required to completely address the deformity.

Flexural Limb Deformities of the Carpus and Fetlock in Foals.

Early recognition and treatment of congenital and acquired flexural deformities of the carpi and fetlocks of foals can lead to conformation correction and an athletic future. Treatment is often based on rigid external coaptation assisted by systemic medical treatment. Foals that readily respond to treatment and correct conformation faults can have normal adult athletic expectations.
