Red cell alloantibodies in patients with haemoglobinopathies.

The present study was carried out to determine the evidence of alloimmunization against red blood cells in 364 patients transfused in our center over a period of 4 years (1990-1993). Among these patients, 127 were thalassemic and 182 had sickle cell disease (SCD). In 55 control patients, who received blood matched for the ABO, Rhesus and Kell antigen systems from the outset of transfusion, no immunization was detected. However, in the study group, who initially received blood matched only for ABH and Rh D antigens, the frequency of alloimmunization was 7.76% (24/309). Only one antibody was det

A Rare Case of Anti-Kpa Antibody Identification in a Chinese Patient.

BACKGROUND: Here we present a rare case of anti-Kpa alloantibody identified in a Chinese individual, along with a literature review. METHODS: An antibody identification test was conducted to clarify the antibody specificity. To further characterize this antibody, we assessed the reactivity of the patient's serum with relative antigen-positive red blood cells at different test phases. We also investigated the sensitivity of the antigen to DTT and papain. We separated the pa-tient's own RBCs from the transfused ones using capillary centrifugation and performed direct antiglobulin test and Kell

[A hemolytic transfusion reaction due to Anti-Ku antibody in a patient with Knull phenotype: the first case in Korea].

Knull phenotype completely lacks all Kell system antigens. Anti-Ku antibody is seen in immunized persons with Knull phenotype by transfusion or pregnancy. It can cause a fatal hemolytic transfusion reaction. A 66-yr-old male patient with liver cirrhosis visited emergency center due to acute bleeding. The patient was at hypovolemic shock status: his blood pressure was 80/50 mmHg, pulse rate was 110/min and hemoglobin level was 4.4 g/dL. Because of the presence of antibody against high incidence antigen, we could not find any compatible blood for the patient. Nevertheless, 4 units of packed RBCs

Identification of RHCE and KEL alleles in large cohorts of Afro-Caribbean and Comorian donors by multiplex SNaPshot and fragment assays: a transfusion support for sickle cell disease patients.

To lower the alloimmunization risk following transfusion in blacks, we developed two genotyping assays for large-scale screening of Comorian and Afro-Caribbean donors. One was a multiplex SNaPshot assay designed to identify ce(s) (340), ceMO/AR/EK/BI/SM, ce(s) , ce(s) (1006) and KEL*6/*7 alleles. The other was a multiplex fragment assay designed to detect RHD, RHDψ and RHCE*C and 455A>C transversion consistent with (C)ce(s) Type 1 and DIII Type5 ce(s) . Variant RHCE*ce alleles or RH haplotypes were detected in 58·69% of Comorians and 41·23% of Afro-Caribbeans. The ce(s) allele, (C)ce(s) Type 1

Red blood-cell alloantibodies in multiply transfused patients in the occupied Palestinian territory: a pilot study.

BACKGROUND: Red blood-cell transfusion has greatly reduced the mortality and morbidity in multiply transfused patients with thalassaemia and sickle cell disease. However, this can result in red blood-cell isoimmunisation with autoantibodies and alloantibodies, which can lead to serious complications such as delayed haemolytic transfusion reaction. The aim of this study was to assess the frequency and types of alloantibodies in multiply transfused patients living in the north of the West Bank. METHODS: This pilot study was done at three thalassaemia centres in Nablus, Jenin, and Tulkarm in the

Eluate analysis of anti-Bg a associated renal allograft rejection.

Renal allograft rejection, following development of a positive indirect antiglobulin reaction, occurred in a multiply transfused recipient. Panel techniques confirmed both anti-Bga and anti-Kell antibodies. Recognition of the concordant antigenic specificity of the Bga and HL-A7 antigens led to an investigation of the potential role of this antibody in the rejection phenomenon. In the absence of serological cytotoxicity, a modified elution technique was used to directly obtain immunoglobulin from the rejected allograft. The eluate obtained displayed specificity for the Bga red blood cell antig

Anti-Kpa-induced severe delayed hemolytic transfusion reaction.

Kpa is a low-frequency antigen occurring in less than 2 percent of the Caucasian population. Mild to moderate delayed hemolytic transfusion reactions (DHTR) and hemolytic disease of the fetus and newborn attributable to anti-Kpa have been reported. Severe overt DHTR has not been reported with anti-Kpa. A case of a severe DHTR attributed to anti-Kpa after multiple RBC transfusions is being reported. A 52-year-old Caucasian woman received multiple units of RBCs for a lower gastrointestinal bleed. She was referred to our institution for hepatic and renal failure, which was supported by laboratory

A challenging case of hemolytic disease of the fetus and newborn (HDFN) due to anti-Ku in a K0 (Kellnull) mother.

Hemolytic disease of the fetus and newborn (HDFN) due to an antibody in the Kell blood group system can be associated with severe fetal anemia. This case report details the challenges of managing a Kellnull mother with anti-Ku that affected her fetus/newborn. A gravida 4 para 3 woman at term underwent an emergency lower caesarean section because of fetal distress. The baby was intubated because of low oxygen saturation. An urgent request for a hematology workup showed severe anemia and erythroblastosis fetalis. Unfortunately, no compatible blood was found, and the baby died. The case was refer

Resolving Blocked Antigen Phenomenon in Hemolytic Disease of the Fetus and Newborn Due to Anti-K.

High-titer antibodies are a cause of false-negative reactions in red blood cell antigen phenotyping, an event referred to as blocked antigen phenomenon (BAP). In hemolytic disease of the fetus and newborn, BAP complicates laboratory workups as fetal phenotype is helpful in confirming the responsible antibody. Acid elution techniques, techniques using ethylenediaminetetraacetic glycine acid, as well as those using chloroquine diphosphate have been used to resolve BAP; however, ethylenediaminetetraacetic glycine acid destroys K-antigen expression and chloroquine diphosphate is not always effecti

The evaluation of extended analysis of serum, amniotic fluid and cardiotocography in the management of pregnancy complicated by immunization in Rh or Kell antigens.

The estimation of protein and bilirubin concentration together with checking the activity of LDH, AspAT, AlAT and Ech in maternal serum and amniotic fluid was performed with intention to improve the accuracy of antenatal prediction in hemolytic disease of newborns. With the same intention, the cardiotocography was introduced. After study on forty-six pregnancies, the usefulness of estimation of optical density, protein and bilirubin concentration in amniotic fluid and calculation of bilirubin/protein ratio was confirmed. The other investigated methods appeared useless.

[Significance of alloantibodies other than anti-D hemolytic disease of the fetus and newborn (HDF/N)].

OBJECTIVE: Because of immunoprophylaxis, the proportion between HDF/N caused by anti-D and other antibodies have changed. We assessed HDF/N due to non anti-D among women with antibodies detected during a screening programme. MATERIAL AND METHODS: Blood samples from 507 women with antibodies were examined for Ig class of antibody, specificity, the titer by routine serological tests and for functional activity by chemilumine-scence test (CLT). The father's and fetal/newborn's blood was also examined. RESULTS: Among 507 women, anti-D was detected in 231 (45.5%), non-anti-D (potentially clinical

Erythrocyte genotyping for transfusion-dependent patients at the Azienda Universitaria Policlinico of Naples.

BACKGROUND AND OBJECTIVES: Although minor erythrocyte antigens are not considered clinically significant in sporadic transfusions, they may be relevant for multi-transfusion patients. When serological assay is not conceivable, molecular genotyping allows predicting the red blood cell phenotype, extending the typing until minor blood groups. The aim of this study was to evaluate the utility of blood group genotyping and compare the molecular typing of erythrocyte antigens with the established serological methods. MATERIALS AND METHODS: We selected 225 blood donors and 50 transfusion-dependent

High-dose intravenous gamma globulin: does it have a role in the treatment of severe erythroblastosis fetalis?

The role of high-dose intravenous (IV) gamma globulin in the treatment of erythroblastosis fetalis was assessed in five pregnancies with severe Rh (four) or Kell (one) isoimmunization. These women were treated with IV gamma globulin (1.0 g/kg body weight) once a week. In addition, fetal blood transfusions were performed when indicated. In four patients with Rh sensitization, high-dose IV gamma globulin treatment had no apparent effect on the total number of intrauterine transfusions required, the interval between transfusions, or the volume of blood required at each transfusion. The treatment

Sonographic detection of fetal hydrops. A report of two cases.

Ultrasound can be useful for fetal assessment in Kell-isoimmunized pregnancies. Because severe hydrops may develop within six days, ultrasound should be repeated more frequently than once per week even if the delta OD450 values are low.

Chronic granulomatous disease and the Mcleod phenotype. Successful treatment of infection with granulocyte transfusions resulting in subsequent hemolytic transfusion reaction.

A young man with X-linked chronic granulomatous disease of childhood, who is of the rare McLeod phenotype with antibodies in his serum shown to be hemolytic and reactive against all red cells with normal expressions of the Kell antigens, developed a severe Nocardia pneumonia with abscess formation and was subsequently treated successfully with granulocyte transfusions in spite of the presence of anti-KX in the patient's serum. The anti-KX did not appear to alter significantly the effectiveness of the transfused granulocytes; it did, however, cause a mild hemolytic transfusion reaction. The pat

A genetic survey in the Bhil tribe of Madhya Pradesh, Central India.

Examination of blood groups, plasma proteins and red cell enzyme types (23 loci), in a sample of 145 Bhils, a tribal group of Madhya Pradesh, Central India, demonstrates their genetic uniqueness. They differ in a number of systems from the nearby nontribal groups both of Hindus and Muslims. The results suggest that the Bhil frequencies include vestiges of the ancestral genepool of a more widespread aboriginal population whose influence is detectable in the gene frequencies of some other populations in India.

Investigation of the Prevalence of Non-ABO Blood Group Antigens in Infertile Women Referred to University Hospitals in Iran: A Cross-Sectional Study.

BACKGROUND AND AIMS: Infertility nowadays is considered a disease and a significant global health issue. Despite extensive research, the cause of approximately 15% of infertility cases remains unknown. Additionally, despite the high financial and emotional costs associated with infertility treatment, there is no single universal method that definitively leads to pregnancy and the birth of a healthy baby. This study investigates the prevalence of minor blood group antigens in infertile and fertile women in Iran, contributing to the understanding of blood group distributions in this population.

Red Cell Phenotyping of Rh and Kell in Voluntary Blood Donors at a Tertiary Care Center in Jamnagar.

Background The Rh and Kell blood group systems are among the most clinically significant in transfusion medicine due to their immunogenic potential. Alloimmunization of these antigens can lead to hemolytic transfusion reactions and hemolytic disease in the fetus and newborn. Despite their clinical importance, antigen phenotyping beyond ABO and RhD is limited in many blood centers worldwide, including in India. This study aims to determine the prevalence of Rh subgroups and Kell antigens among voluntary blood donors in Jamnagar to enhance transfusion safety. Methods This prospective observation
