Ocular manifestations of Sjögren's syndrome: keratoconjunctivitis sicca.

As part of the triad of Sjögren's syndrome (SS), keratoconjunctivitis sicca (KCS) can be found in virtually all SS patients. This term emphasizes not only the reduced tear production but also the pathologic changes in the epithelial cells of the ocular surface. The symptoms of dry eye may vary from one patient to another depending on the severity of dryness, the ability of the diminished tear film to moisten the ocular surface, and the patient's tolerance for ocular discomfort. Treatment of the dry eyes is remarkably effective, and most patients benefit from moisturizing eyedrops to replace th

[New approaches to the treatment of keratoconjunctivitis sicca].

UNLABELLED: A new method has been developed for the treatment of severe forms of keratoconjunctivitis sicca (KCS) that involves the use of an original cyclosporine A (CyA) saturated soft contact lens (SCL) together with preservative-free artificial tears therapy. AIM: to evaluate the effectiveness of the newly developed treatment for KCS based on the use of medical SCL saturated with 0.05% CyA. MATERIAL AND METHODS: The patients (43 men, 60 eyes) with severe KCS were divided into 2 groups. Group 1 included 21 patients (30 eyes), who received artificial tears and wore 0.05% CyA-saturated sili

Autoimmunity in the pathogenesis and treatment of keratoconjunctivitis sicca.

Dry eye is a chronic corneal disease that impacts the quality of life of many older adults. Keratoconjunctivitis sicca (KCS), a form of aqueous-deficient dry eye, is frequently associated with Sjögren's syndrome and mechanisms of autoimmunity. For KCS and other forms of dry eye, current treatments are limited, with many medications providing only symptomatic relief rather than targeting the pathophysiology of disease. Here, we review proposed mechanisms in the pathogenesis of autoimmune-based KCS: genetic susceptibility and disruptions in antigen recognition, immune response, and immune regula

Treatment of keratoconjunctivitis sicca with topical androgen.

OBJECTIVE: Androgens have been reported to influence lipid production of sebaceous glands and even many ocular tissues. The effect of topical androgen therapy on a 54-year-old patient with keratoconjunctivitis sicca (KCS) and decreased lipid phase of the tear film is reported. METHODS: For assessment of the lipid phase of the tear film, break up time (BUT) and lipid layer thickness (LLT) were monitored during 6 months before treatment as well as 3 months while using a daily topical androgen therapy. RESULTS: During the topical androgen therapy the pathological lipid phase of the tear film wa

Advances in the diagnosis and management of keratoconjunctivitis sicca.

Keratoconjunctivitis sicca is a common ocular surface disease that develops in patients with aqueous tear deficiency. Recent advances have been made in diagnosis, pathogenesis, and therapy of this condition. Advances in diagnosis include improved understanding of the specificity of the tests used for diagnosis, elucidation of the mechanism of the ocular surface rose bengal and fluorescein staining that occurs in this condition, and the expanded use of impression cytology. Advances in pathogenesis include the concept that keratoconjunctivitis sicca is a condition of abnormal growth and differen

[Keratoconjunctivitis sicca in patients with lipoprotein (a) elevation].

BACKGROUND: Elevated Lp(a) levels are known as risk factor for premature coronary artery disease and apoplexy. To our knowledge ophthalmologic alterations have not been described in combination with elevated Lp(a) levels. We examined whether patients with LP (a) elevation had an increased incidence of keratoconjunctivitis sicca. PATIENTS AND METHODS: 27 patients, 16 women and 11 men were examined. The Lp(a) value was in all patients definitively pathological. The following examinations were performed: history, slitlamp examination, Schirmer-test, impressioncytology, fluorescein staining of th

Impression cytology in patients with keratoconjunctivitis sicca.

Impression cytology is a simple and painless procedure that allows the collection of the superficial layers of the conjunctival epithelium. Each sample is assigned a grade of epithelial metaplasia, and goblet cell density is calculated in each one. We have studied the superior and temporal bulbar conjunctiva of dry eyes and have compared it with that of normal controls. In normal and dry eyes we find a statistically significant difference both in goblet cell density and grade of metaplasia, between superior and temporal bulbar conjunctiva. The differences in grade of metaplasia and goblet cell

The central corneal thickness in keratoconjunctivitis sicca.

The central corneal thickness of 17 patients with keratoconjunctivitis sicca was compared with the thickness recorded in a control group of 105 subjects with healthy eyes. A moderate, but highly statistically significant central corneal thinning was found in the keratoconjunctivitis sicca group. The possible etiology and clinical importance of this finding is briefly discussed.

A new rabbit model for keratoconjunctivitis sicca.

The authors created a new rabbit model for keratoconjunctivitis sicca by cauterizing the lacrimal gland excretory duct and surgically removing the nictitating membrane and harderian gland. Although the slit-lamp examination findings remained normal for the first 8 wk postoperatively, tear-film osmolarity was elevated by postoperative day 1. Corneal epithelial glycogen levels declined progressively, and conjunctival goblet cell density remained decreased. Multiple controls indicated that closure of the lacrimal gland excretory duct was required for elevation of tear film osmolarity, which, in t

Tear diluents in the treatment of keratoconjunctivitis sicca.

To determine the optimum solution concentration for lowering elevated tear film osmolarity in keratoconjunctivitis sicca (KCS), tear osmolarity was measured in four KCS patients before and after instillation of either an isotonic saline solution or one of four hypotonic saline solutions (range, 75-225 mOsm/L). Average tear osmolarity one minute after instillation was significantly lower with the hypotonic solutions than with the isotonic saline (mean +/- SEM, 290 +/- 3 mOsm/L vs. 317 +/- 1 mOsm/L, P less than 0.0005). Solutions 150 mOsm/L or less were most effective in lowering osmolarity; the

Serologic studies in patients with keratoconjunctivitis sicca.

Thirty-two patients with keratoconjunctivitis sicca (KCS) were screened for the presence of antinuclear antibodies, rheumatoid factor, and autoantibodies associated with Sjögren's syndrome (designated SS-A and SS-B). None of these patients had or were found to have clinical evidence of connective-tissue disease. The conditions of 19 (59%) patients were antinuclear-antibody-positive and 18 (56%) were rheumatoid-factor-positive. We found SS-A and/or SS-B autoantibodies in ten (31%) patients. There seems to be a high incidence of serologic abnormalities in patients with KCS, even when those patie

Osmolarity of tear microvolumes in keratoconjunctivitis sicca.

Determinations of tear film osmolarity were performed to evaluate their usefulness in diagnosing keratoconjunctivitis sicca (KCS) and to evaluate the possible role of elevated tear osmolarity in this disorder's pathogenesis. Tear samples were obtained using a new technique that virtually eliminates the problems of reflex tearing and sample evaporation. The tear osmolarity of 36 samples obtained from 31 normals eyes averaged 302 +/- 6.3 (SD) mOsm/liter; 38 samples obtained from 30 KCS eyes averaged 343 +/- 32.3 (SD) mOsm/liter. The sensitivity of a single measurement was 94.7% and the specifici

Studies of the viral flora in keratoconjunctivitis sicca.

Fifty patients suffering from KCS and exhibiting various patterns of corneal staining with fluorescein were examined for adenovirus types 3, 7, 8, and 14, herpes simplex, and the TRIC group by tissue culture and immunofluorescein techniques. No viruses were detected by either methods, and it is considered that presumption of virus involvement in these cases is untenable.
