Human brain beta-hydroxybutyrate and lactate increase in fasting-induced ketosis.

Ketones are known to constitute an important fraction of fuel for consumption by the brain, with brain ketone content generally thought to be low. However, the recent observation of 1-mmol/L levels of brain beta-hydroxybutyrate (BHB) in children on the ketogenic diet suggests otherwise. The authors report the measurement of brain BHB and lactate in the occipital lobe of healthy adults using high field (4-T) magnetic resonance spectroscopy, measured in the nonfasted state and after 2- and 3-day fasting-induced ketosis. A 9-mL voxel located in the calcarine fissure was studied, detecting the BHB

Mild ketosis, metabolic syndrome and genetic-lifestyle interactions in the UK Biobank.

The relationship between mild ketosis and metabolic syndrome (MetS) remains unclear. We aimed to investigate the association between serum ketone levels and MetS and to examine how genetic and lifestyle factors influence this relationship. We conducted a cross-sectional observational study using data from the UK Biobank, comprising 269 178 participants. Participants were categorised into low and high serum ketone groups based on β-hydroxybutyrate levels (cut-off: 0·12 mM). Dietary patterns were assessed using validated questionnaires, and a polygenic risk score (PRS) was generated to examine g

Beyond Ketosis: Dietary Therapies and the Microbiota-Gut-Brain Axis in Epilepsy.

Background: Epilepsy is a complex neurological disorder in which growing evidence supports a significant role for the microbiota-gut-brain axis (MGBA) in modulating neuroinflammation, neuronal excitability, and treatment responsiveness. Beyond their traditional role in inducing ketosis, dietary therapies may influence epilepsy by modulating gut microbial ecology, intestinal barrier integrity, immune signaling, and microbiota-derived metabolites. Methods: This narrative review critically examines current clinical and experimental evidence regarding the relationship between epilepsy, gut microbi

HbA1c as a Screening tool for Ketosis in Patients with Type 2 Diabetes Mellitus.

Ketosis in patients with type 2 diabetes mellitus (T2DM) is overlooked due to atypical symptoms. The objective of this study is to evaluate the value of hemoglobin A1c (HbA1c) as a screening tool for ketosis in T2DM patients. This retrospective study consisted of 253 T2DM patients with ketosis at Shanghai 10th People's Hospital during a period from January 1, 2011 to June 30, 2015. A control group consisted of 221 T2DM patients without ketosis randomly selected from inpatients during the same period. Receiver operating characteristic curve (ROC) analysis was used to examine the sensitivity and

Prevalence of subclinical ketosis and relationships with postpartum diseases in European dairy cows.

Subclinical ketosis (SCK) is defined as concentrations of β-hydroxybutyrate (BHBA) ≥ 1.2 to 1.4 mmol/L and it is considered a gateway condition for other metabolic and infectious disorders such as metritis, mastitis, clinical ketosis, and displaced abomasum. Reported prevalence rates range from 6.9 to 43% in the first 2 mo of lactation. However, there is a dearth of information on prevalence rates considering the diversity of European dairy farms. The objectives of this study were to (1) determine prevalence of SCK, (2) identify thresholds of BHBA, and (3) study their relationships with postpa

Induction of the apolipoprotein AI gene by fasting: a relationship with ketosis but not with ketone bodies.

Apolipoprotein AI (apoAI) expression is inversely related to the incidence of atherosclerosis. ApoAI expression is also influenced by the nutritional state and diabetes. We used both cell culture and animal models to examine the effect of fasting and ketoacidosis on apoAI gene expression. Two days of food deprivation in rats increased hepatic and intestinal apoAI mRNA by 2.6- and 2.3-fold, respectively (P < .05). The absolute concentration of plasma apoAI did not change. However, the plasma apoAI concentration relative to the plasma concentration of serum proteins was increased 23% (P < .05).

Dapagliflozin Alters Plasma Metabolome in Patients With Type 1 Diabetes During Ketosis.

AIMS: Use of sodium-glucose cotransporter 2 inhibitors (SGLT2i) in patients with type 1 diabetes (T1D) is limited by increased risk of ketoacidosis. We aimed to determine metabolic pathways affected by SGLT2i treatment in adults with T1D during ketosis induced by insulin withdrawal. MATERIALS AND METHODS: Plasma metabolomic and lipidomic analysis by ultra-high-performance liquid chromatography/mass spectrometry was performed at baseline and after supervised insulin withdrawal in 17 adults with T1D treated with usual care (UC) and (DP) dapagliflozin (10 mg daily for 14 days). RESULTS: Plasma

Ketosis regulates K+ ion channels, strengthening brain-wide signaling disrupted by age.

Aging is associated with impaired signaling between brain regions when measured using resting-state fMRI. This age-related destabilization and desynchronization of brain networks reverses itself when the brain switches from metabolizing glucose to ketones. Here, we probe the mechanistic basis for these effects. First, we confirmed their robustness across measurement modalities using two datasets acquired from resting-state EEG (Lifespan: standard diet, 20-80 years, N = 201; Metabolic: individually weight-dosed and calorically-matched glucose and ketone ester challenge, μ a g e = 26.9 ± 11.2

Beyond ketosis: the search for the mechanism underlying SGLT2-inhibitor benefit continues.

Despite the impressive clinical benefits and widespread adoption of sodium glucose cotransporter 2 inhibitors (SGLT2i) to treat all classes of heart failure, their cardiovascular mechanisms of action are poorly understood. Proposed mechanisms range broadly and include enhanced ketogenesis, where the mild ketosis associated with SGLT2i use is presumed to be beneficial. However, in this issue of the JCI, carefully conducted metabolic flux studies by Goedeke et al. comparing the effects of SGLT2i and exogenous ketones suggest differential effects. Thus, the mechanisms of action for SGLT2i are lik

Role of nutritional ketosis in the improvement of metabolic parameters following bariatric surgery.

BACKGROUND: Ketone bodies (KB) might act as potential metabolic modulators besides serving as energy substrates. Bariatric metabolic surgery (BMS) offers a unique opportunity to study nutritional ketosis, as acute postoperative caloric restriction leads to increased lipolysis and circulating free fatty acids. AIM: To characterize the relationship between KB production, weight loss (WL) and metabolic changes following BMS. METHODS: For this retrospective study we enrolled male and female subjects aged 18-65 years who underwent BMS at a single Institution. Data on demographics, anthropometrics

Relation of Subclinical Ketosis of Dairy Cows with Locomotion Behaviour and Ambient Temperature.

Rumination time, chewing time and drinking time are indicators that can be assessed in case of cow disease. In this research, two groups of cows were formed: cows with subclinical ketosis (SCK; n = 10) and healthy cows (HG; n = 10). Behaviour such as walking activity, feeding time with head position up, feeding time with head position down, change of activity and average, minimal and maximal ambient temperature of cows were recorded by the RumiWatch noseband system (RWS; RumiWatch System, Itin+Hoch GmbH, Liestal, Switzerland). The RWS comprises a noseband halter with a built-in pressure sensor

Hyperglycaemia and ketosis in a non-diabetic patient--an unusual cause of delayed recovery.

We report a case of hyperglycaemia and ketosis developing in a non-diabetic patient who underwent a neurosurgical procedure under general anaesthesia. A 52-year-old non-diabetic female patient underwent excision of acoustic neuroma under general anaesthesia. Pancreatic function was not disturbed and she received a single dose of dexamethasone (8 mg) and paracetamol (1 g). Delayed recovery from anaesthesia occurred. On investigation, she was found to have hyperglycaemia and ketosis. She was further managed on the line of diabetic ketoacidosis. After 24 hours, when blood glucose had normalised a

Cancer cachexia: influence of systemic ketosis on substrate levels and nitrogen metabolism.

The aim of this study was to determine whether a ketogenic diet could decrease nitrogen losses in cachectic cancer patients and at the same time reduce the supply of glucose for tumor energy metabolism. Five patients with malignant disease and severe weight loss (mean 32%) were fed via a fine bore nasogastric tube. A normal diet was given for 6 d and this was followed by 7 d of an isonitrogenous, isocaloric, ketogenic diet. Both diets were well tolerated. At 7 d the mean ketone body concentration in the blood of patients fed the ketogenic diet was 1.21 +/- 0.33 mM. This ketosis was associated

Gestational Ketosis Compromises Nephron Endowment and Long-Term Kidney Function in Offspring.

KEY POINTS: Maternal ketosis disrupted nephrogenesis, reduced proliferation, and suppressed Myc and cell cycle signaling. Offspring exhibited persistent kidney deficits, including reduced nephron endowment and compromised long-term kidney function. Gestational ketosis developmental risks highlighted the need for metabolic monitoring and dietary guidance during pregnancy. BACKGROUND: The increasing popularity of ketogenic diets raises concerns regarding their safety during pregnancy. While mild ketosis is a common metabolic adaptation in gestation, the effect of diet-induced, supraphysiologica

Current Understanding of Bovine Ketosis: From Molecular Basis to Farm-Level Management.

Bovine ketosis represents a critical metabolic challenge in modern dairy production, primarily affecting high-yielding cows during the periparturient period. The disorder arises from severe negative energy balance (NEB), where energy demands exceed supply, triggering substantial mobilization of adipose tissue and hepatic accumulation of non-esterified fatty acids (NEFAs). This stress overwhelms hepatic oxidative capacity, leading to incomplete fatty acid oxidation and excessive ketone body production-primarily β-hydroxybutyrate (BHB), acetoacetate, and acetone. While genomics enables the ident
