We report a case of lithiasis of the left ectopic pelvic kidney. A 70-year-old man was admitted to our department for left lower back pain. He has been aware of gross hematuria since three or four years before. Computed tomography demonstrated the left ectopic pelvic kidney with a 16-mm stone and a 5-mm stone in the renal pelvis which was accompanied by hydronephrosis. There are regional variations of the amplitude, frequency and waveform of spontaneous contractions within the isolated renal pelvis and calyx of the multicalyceal pyeloureter of the pig. The contractility of these structures was studied in normal and hydronephrotic preparations. Tissues were obtained from normal and chronically obstructed animals. In 20 normal preparations, the contractile frequency of the renal pelvis was 4.9 +/- 2.1 contractions/min, while that of the calyx was 9.5 +/- 2.2 (mean +/- SD). A renal calyceal diverticulum is a cystic cavity within the renal parenchyma, which is lined by transitional epithelium and communicates with the calyx or, more rarely, the renal pelvis. In children, this anomaly is extremely rare. The article describes the resection of the middle calyceal diverticulum in an 8-year-old child using retroperitoneoscopic access.

INTRODUCTION: In the treatment of pyelo multi calyceal renal lithiasis, although we utilize both rigid and flexible instruments, the greatest challenge when trying to achieve a stone-free status after the procedure with a single access, is represented by the presence of residual caliceal stones difficult to reach for the length of the infundibular system in which they are localized, and major axis of it at an acute angle with respect to the axis of the cannula entrance. The effect of anatomical factors such as lower infundibulopelvic angle (LIPA), lower infundibulum diameter (LID) and inferior calyceal length (ICL) on renal stone formation was investigated.
