Fracture in an arthrodesed Charcot's knee joint.

Charcot's joint is a type of neuro-arthropathy, where asymmetrical damage of the involved joint happens haphazardly, without following any described pattern. We present a rare case of Charcot's joint involving the knee joint in an adult male following spina bifida. His knee joint was successfully fused after two surgeries. Later, he sustained a fracture through the arthrodesis after a fall, which we managed surgically. The fracture through the knee arthrodesis was managed surgically by an open reduction and internal fixation, using a 14-hole broad low contact dynamic compression locking plate

Immunohistochemical examination in arthrofibrosis of the knee joint.

INTRODUCTION: Arthrofibrosis (AF) is the result of increased cell proliferation and synthesis of matrix proteins (collagen I, III, and VI). Especially after invasive knee surgery, e.g., ligament reconstruction or knee replacement, abnormal fibroblast proliferation with pathological periarticular fibrosis can be observed leading to severely limited joint motion. The pathogenesis of AF is currently not fully understood. The present work aims to determine pathogenic factors. MATERIALS AND METHODS: A descriptive, histological and immunohistochemical comparative study was performed on tissue sampl

A biomimicking design for mechanical knee joints.

In this paper we present a new bioinspired bicondylar knee joint that requires a smaller actuator size when compared to a constant moment arm joint. Unlike existing prosthetic joints, the proposed mechanism replicates the elastic, rolling and sliding elements of the human knee. As a result, the moment arm that the actuators can impart on the joint changes as function of the angle, producing the equivalent of a variable transmission. By employing a similar moment arm-angle profile as the human knee the peak actuator force for stair ascent can be reduced by 12% compared to a constant moment arm

Osteoarthritis of knee joint in metabolic syndrome.

Pathogenesis of osteoarthritis may have a systemic metabolic component. This study was undertaken to assess the prevalence of primary knee osteoarthritis (OA) in a sample of Egyptian patients with metabolic syndrome (MetS) and to examine the relationship of metabolic syndrome and its components with clinical, functional, and radiographic findings of knee OA. A total of 60 patients (55 females, 5 males) diagnosed as having MetS according to the National Cholesterol Education Program Adult Treatment Panel III (ATP III) criteria and 30 obese subjects without MetS (24 females, 6 males), serving as

Response of knee fibrocartilage to joint destabilization.

OBJECTIVE: A major challenge to understanding osteoarthritis (OA) pathology is identifying the cellular events that precede the onset of cartilage damage. The objective of this study is to determine the effect of joint destabilization on early changes to fibrocartilage in the joint. DESIGN/METHODS: The anterior cruciate ligament was transected in collagen reporter mice (Col1CFP and ColXRFP). Mineralization labels were given every 2 weeks to measure new mineralized cartilage apposition. Novel fluorescent histology of mineralized tissue was used to characterize the changes in fibrocartilage at

Gout and coexisting pseudogout in the knee joint.

PURPOSE: We report the unusual case of a 63-year-old man with gout and coexisting pseudogout (calcium pyrophosphate dihydrate crystal deposition disease) affecting the same joint. In this report, we describe the clinical features of the patient in addition to the characteristics of the crystals extracted from his affected joint, examined using compensated polarized microscopy. METHODS: Synovial fluid and specimens of the lateral meniscus of the patient's knee joint taken during arthroscopy were analyzed using compensated polarized microscopy to evaluate the presence and type of crystals. [Tuberculosis of the knee joint: a case report].

Tuberculosis may develop in the skeletal system apart from its primary location, the lungs. A 22-year-old male patient presented with complaints of severe pain and swelling in the left knee and difficulty in walking. The symptoms first appeared 12 years before and he underwent open surgical debridement and arthroscopic debridement at two other centers, at which time aspiration cultures taken from the knee yielded no growth. Physical examination showed a painful left knee with swelling, increased temperature, effusion, and limited range of motion. Based on history, physical examination, radiolo

Development of the human knee joint ligaments.

BACKGROUND: Many studies have been published on the development of the human knee joint, but scant attention has been given to the development of the knee joint ligaments. The only elements that have received much attention are the cruciate ligaments and their relationships with the synovial membrane. METHODS: We summarize our observations on the development of the knee joint ligaments in 50 serially sectioned human embryonic and fetal lower limbs (26 embryos and 24 fetuses). RESULTS: The patellar ligament begins to form in O'Rahilly stage 20, with the muscle fibers of the quadriceps muscle

[Osteoarthritis of the knee joint: a field study].

To clarify the natural history of osteoarthritis of the knee joint, a field study of the knee joint was performed on the inhabitants of Matsudai Town, Niigata Prefecture in 1979 and 1986. The study included the same 979 individuals, involving 820 women and 159 men, aged between 40 and 65 years in 1986 as in 1979. Narrowing of the femoro-tibial joint space was found in 17.7% and 26.1% of the women, and in 5.3% and 12.0% of the men, in 1979 and 1986, respectively. The individuals without osteoarthritic change and those with subchondral sclerosis or osteophytes in the femoro-tibial joint space ex

[Arthroscopic therapy of arthrofibrosis of the knee joint].

Arthrofibrosis following ACL reconstruction is a well-recognized and disturbing complication. It may result in a greater functional deficit than the original ACL deficiency. The purposes of this retrospective study were to present our results of arthroscopic arthrolysis and gentle manipulation in 16 consecutive cases in the past almost 3 years, to determine and define the etiology and risk factors, to make recommendations regarding prevention and to develop a time-related management concept. All the 16 patients had previously undergone open ACL reconstruction and had developed arthrofibrosis.

Radiographic assessment of the knee joint in osteoarthritis.

The development of radiographic systems for the grading of osteoarthritis requires knowledge of the reproducibility of their individual component features. This paper reports the reproducibility, both within and between observers, for five commonly used radiographic features of osteoarthritis in the tibiofemoral and patellofemoral compartments of the knee joint. The results suggest that assessments of joint space narrowing, osteophyte, and bony contour in the tibiofemoral compartments are more reproducible than those of sclerosis and cyst. Patellofemoral assessments, with the exception of oste

Arthroscopic evaluation of synovitis in the knee joints.

It is not always easy to diagnose certain types of monoarthritis of the knee joints. In such cases, it is essential to evaluate the condition of the synovitis both macroscopically and microscopically, as the synovial appearance (synovitis) closely reflects the pathological condition (arthritis) of the knee joints. Arthroscopy is an excellent tool for visualizing and evaluating the condition of the synovium macroscopically. On arthroscopic examination, precise evaluation of the villi often provides clues, enabling accurate diagnosis. In addition, histological sections obtained from biopsy speci

[The value of sonography in injuries of the knee joint].

In an experimental study we examined three knee specimens with regard to the value of ultrasound for imaging soft tissue structures. Subsequently we documented artificial lesions at the menisci and ligaments. With this experience we started a prospective study comparing the preoperative ultrasound investigation with the results of arthroscopy in 101 patients. For ultrasound evaluation we used a 5 MHz linear and a 7.5 MHz sector scanner. In the experimental as well as in the clinical study the menisci were well visualised. For medial meniscus lesions the accuracy and the predictive value was 81

[Traumatic cartilage damage of the knee joint].

Thirty-five traumatic lesions to cartilage were recorded, together with other injuries, from 225 arthroscopic examinations in two years. Most of the lesions (20) were located at the femoral condyle. The patella was involved in eight cases. However, if the patellar surface was related to the surface of the femoral condyles, the former was just as strongly affected by traumatic effects as the latter. No unambiguous localisation was possible in six cases, most of them smaller cartilage abrasions. Optional therapy proved to be applicable via the arthroscope in seven cases, while small incisions in

[Arthroscopy of the traumatized knee joint in childhood].

63 children underwent diagnostic arthroscopy of the knee carried out under general anaesthesia, 46 of them presenting an acute haemarthrosis, 17 because of recurrent pain of effusion that developed under remobilisation stress two or more weeks after the accident. Arthroscopy provides--in addition to radiographs and stability test--complete evaluation of internal derangement of the knee. Clinical diagnosis based on examination and radiographs alone was confirmed by arthroscopy in only 25% in our series. Several arthrotomies would have been performed following "safe" clinical diagnosis, but coul

[Causes and pathophysiology of infection of the knee joint].

The origin of an infection of the knee joint can be of an endogenous/hematogenous nature or, in posttraumatic cases, of an exogenous nature. Hematogenous infections are found mostly in children, but also in patients with reduced immune functions, e.g. after organ transplantations. Posttraumatic joint infections are caused by open injuries, intraarticular injections, operations, or they can be transmitted from the surroundings. They are relatively rare after accident surgery and orthopedic surgery. Two statements are of special importance for pathophysiology: 1. An intraarticular infection will

[Study of the development of the human knee joint].

By studying a complete series of sections of the human knee joint during the period in which the joint gap develops we have found out that the knee joint contains a high broad septum (the mediastinum genus), in which there are many broad vessels. These vessels feed the cruciate ligament anlages, the meniscus anlages and the cartilagenous models of the adjacent bone anlages. In the human embryo the mediastinum genus splits the knee joint into a lateral and a medial half. Communication between these 2 halves is between the small contact surfaces of the patella and femur anlages.
