Nutritional support during pregnancy and lactation.

Needs for virtually all nutrients increase during pregnancy and lactation but those for iron are the most difficult to meet from a normal diet. During pregnancy, nutritional objectives include helping the woman to increase the quality of her diet in protective nutrients as well as energy to meet the elevated needs of gestation, ensuring satisfactory weight gain patterns, prescribing iron and possible folic acid supplements, ensuring that intakes of alcohol, drugs, and other potentially harmful substances are moderate, preparing her for feeding the infant, and coping with any special diet-relat

[Microbiologic characteristics of postpartum lactation mastitis].

In the bacteriological survey of 410 patients with purulent mastitis Staphylococcus aureus was found to be the main etiological factor of the disease. In most cases the strains of phage group I, phage type 80 were isolated. In unilateral mastitis during the acute period staphylococci were isolated from the the milk samples taken from the affected mammary gland in 77.4% of cases, and from the milk samples from the healthy mammary gland in 62.3% of cases. The strains isolated from the milk of both mammary glands and the surgical wound were identical. By the time of clinical convalescence staphyl

Plasma prolactin activity in inappropriate lactation.

Using a bioassay dependent on the development of a lactogenic response in rabbit mammary tissue cultured in vitro, prolactin distinct from immunoreactive growth hormone has been found in the plasma of patients of both sexes with inappropriate lactation with and without evidence of pituitary tumours. It has also been found in one patient with primary hypothyroidism and galactorrhoea, and in another during chlorpromazine therapy, but not in nine patients with gynaecomastia without galactorrhoea. Plasma prolactin levels were examined in seven patients during oral glucose tolerance tests: no chang

Human cytomegalovirus (HHV5) reactivation during lactation.

A globally prevalent human cytomegalovirus (HCMV), capable of establishing a lifelong infection in the host, exists in a latent state and can reactivate. Viral transmission can occur through contact with the bodily fluids of infected individuals, such as saliva, semen, urine, and breast milk. The infection is usually asymptomatic. However, immunocompromised individuals are more prone to experience severe complications, including pneumonia, organ damage, and retinitis. Because of high levels of HCMV in breast milk, breastfeeding by seropositive mothers may lead to viral transmission, which is m

Breast and Nipple Dermatoses During Lactation.

Lactation and breastfeeding can present both psychological and physical challenges for breastfeeding mothers. In addition, many nursing mothers will also suffer from breast and nipple dermatoses during this period, compounding these difficulties. Common causes of breast and nipple dermatitis during lactation include eczema, psoriasis, mastitis, mammary Paget's disease, Raynaud's phenomenon, and herpes virus infection, all of which may arise or be exacerbated during breastfeeding. This article summarises the common causes of breast and nipple dermatitis during lactation, as well as their invest

Pregnancy and Lactation Associated Bone Fragility.

The skeleton is a storehouse of mineral that can be borrowed from in times of need, such as for reproduction. Skeletal resorption is normally modest during pregnancy but can be excessive when dietary calcium intake or absorption are insufficient for maternal and fetal needs. In contrast, substantial skeletal resorption is hormonally programmed to occur during lactation, with a loss of 5% to 10% of bone density from the spine over the first 6 months, independent of dietary calcium intake. The maternal skeleton is the main source of calcium in milk. Normally bone resorption during reproduction i

Psychotropic drugs in pregnancy and lactation.

INTRODUCTION: The prescription of psychotropic drugs during pregnancy is a constantly increasing practice, but its safety information remains limited. On the other side, while concerns about teratogenicity persist, it is also evident that mothers' psychiatric disorders represent a risk factor for both them and the infants and cannot be neglected or left untreated. Therefore, careful monitoring and dose adjustments are essential to minimize risks while ensuring treatment efficacy. AREAS COVERED: The present article is a narrative review of the literature on psychotropic drug use in pregnancy a

Hypercalcemia Associated with Pregnancy and Lactation.

Hypercalcemia during pregnancy is a risk for adverse maternal and fetal consequences. Although primary hyperparathyroidism is by far the most common etiology of hypercalcemia in pregnancy, an array of other etiologies of hypercalcemia associated with pregnancy and lactation have been described. Parathyroidectomy continues to be the preferred treatment for primary hyperparathyroidism. Medical management options are limited.

Metabolomics during canine pregnancy and lactation.

During pregnancy and parturition, female dogs have to cope with various challenges such as providing nutrients for the growth of the fetuses, hormonal changes, whelping, nursing, milk production, and uterine involution. Metabolomic research has been used to characterize the influence of several factors on metabolism such as inter- and intra-individual factors, feeding, aging, inter-breed differences, drug action, behavior, exercise, genetic factors, neuter status, and pathologic processes. Aim of this study was to identify metabolites showing specific changes in blood serum at the different ph

Treatments for breast engorgement during lactation.

BACKGROUND: Engorgement is the overfilling of breasts with milk, often occurring in the early days postpartum. It results in swollen, hard, painful breasts and may lead to premature cessation of breastfeeding, decreased milk production, cracked nipples and mastitis. Various treatments have been studied but little consistent evidence has been found on effective interventions. OBJECTIVES: To determine the effectiveness and safety of different treatments for engorgement in breastfeeding women. SEARCH METHODS: On 2 October 2019, we searched Cochrane Pregnancy and Childbirth's Trials Register, Cl

Breast Imaging in Pregnancy and Lactation.

All breast disorders found during pregnancy and lactation should be carefully evaluated. Most of them are benign, but it is essential to exclude pregnancy-associated breast cancer (PABC), which is too often diagnosed late. The first-line imaging technique is ultrasound (US), which must be completed by mammography if there is any clinical or US suspicious sign . In lactating patients with PABC , breast magnetic resonance imaging (MRI) can be useful for local assessment.Management depends on the precise analysis and BI-RADS classification of the lesion. During pregnancy and lactation, there is a

Immunomodulatory drugs in pregnancy and lactation.

Pregnancy presents challenges for women with autoimmune diseases. It is associated with significant physiological, hormonal and immunomodulatory changes which are complex and vary according to the stage of pregnancy Pregnancy planning and counselling should be offered Autoimmune diseases such as rheumatoid arthritis tend to improve in pregnancy while systemic lupus erythematosus may increase in activity During pregnancy the chosen regimen should control or prevent underlying disease activity and minimise risk to the fetus. Ideally, women should be on a stable regimen before conception Poorly c

Treatments for breast engorgement during lactation.

BACKGROUND: Breast engorgement is a painful condition affecting large numbers of women in the early postpartum period. It may lead to premature weaning, cracked nipples, mastitis and breast abscess. Various forms of treatment for engorgement have been studied but so far little evidence has been found on an effective intervention. OBJECTIVES: This is an update of a systematic review first published by Snowden et al. in 2001 and subsequently published in 2010. The objective of this update is to seek new information on the best forms of treatment for breast engorgement in lactating women. SEARC

A rare case of lactation anaphylaxis.

Lactation anaphylaxis (LA) is an extremely rare condition with only five other cases having been previously reported in the literature. Patients will exhibit cutaneous symptoms, respiratory symptoms, gastrointestinal symptoms, and/or cardiovascular symptoms. Post-partum, the drop in hormones is thought to facilitate an exaggerated histamine response. An elevated serum tryptase level performed between 15 minutes to three hours of onset of symptoms will confirm a true anaphylactic reaction. It is important to be aware of this extremely rare medical condition and its management with antihistamine

Does the circadian system regulate lactation?

Environmental variables such as photoperiod, heat, stress, nutrition and other external factors have profound effects on quality and quantity of a dairy cow's milk. The way in which the environment interacts with genotype to impact milk production is unknown; however, evidence from our laboratory suggests that circadian clocks play a role. Daily and seasonal endocrine rhythms are coordinated in mammals by the master circadian clock in the hypothalamus. Peripheral clocks are distributed in every organ and coordinated by signals from the master clock. We and others have shown that there is a cir

[Pregnancy, lactation and bone mineral density].

Evidence shows that pregnancy and lactation lead to the decrease of bone mineral density (BMD), which is related to mobilization of skeletal calcium. Numeorous mechanisms are involved in maternal adaptation to the increased demand for calcium. BMD has been proved to return to the baseline level after weaning. Pregnancies and long or repeated periods of feeding do not seem to determine the diagnosis of osteoporosis in later life. In fact, women with multiple pregnancies have been shown to have the same or higher BMD and lower fracture risk compared with nulliparous women. Pregnancy and lactatio
