Language-Independent Hearing Screening - Increasing the Feasibility of a Hearing Screening Self-Test at School-Entry.

A tablet-based language-independent self-test involving the recognition of ecological sounds in background noise, the Sound Ear Check (SEC), was adapted to make it feasible for young children. Two experiments were conducted. The first experiment investigated the SEC's feasibility, as well as its sensitivity and specificity for detecting childhood hearing loss with a monaural adaptive test procedure. In the second experiment, the SEC sounds, noise, and test format were adapted based on the findings of the first experiment. The adaptations were combined with three test procedures, one similar to

Non-invasive alternatives to the Wada test in the presurgical evaluation of language and memory functions in epilepsy patients.

The cognitive outcome of the surgical removal of an epileptic focus depends on the assessment of the localisation and functional capacity of language and memory areas which need to be spared by the neurosurgeon. Traditionally, presurgical evaluation of epileptic patients has been achieved by means of the intracarotid amobarbital test assisted by neuropsychological measures. However, the advent of neuroimaging techniques has provided new ways of assessing these functions by means of non-invasive or minimally invasive methods, such as anatomical and functional magnetic resonance imaging, positro

Near-infrared spectroscopy as an alternative to the Wada test for language mapping in children, adults and special populations.

The intracarotid amobarbital test (IAT) is the most widely used procedure for pre-surgical evaluation of language lateralization in epileptic patients. However, apart from being invasive, this technique is not applicable in young children or patients who present mental retardation and/or language deficits. Functional magnetic resonance imaging (fMRI) is increasingly employed as a non-invasive alternative. Again, this method is more difficult to use with young children, especially hyperactive ones, since they have to remain motionless during data acquisition. The aim of this study was to determ

A direct test for lateralization of language activation using fMRI: comparison with invasive assessments in children with epilepsy.

This study introduces a direct method of assessing cerebral lateralization for language based on fMRI activation. The method, derived from a voxel-based morphometry study by C. H. Salmond et al. (2000, Hum. Brain Mapping 11, 223-232), bases lateralization on the direct statistical comparison of the magnitude of task-induced activation in homotopic regions of the two hemispheres. Lateralization results obtained with this direct method were compared to those obtained with a widely used method which involves the calculation of a laterality index (LI) based on the number of significantly activated

Automated Question-and-Answer-Based Speech-In-Noise Hearing Test Using a Large Language Model.

A conversational speech-in-noise test administered without human supervision is desirable to reduce the need to attend the clinic. This study aimed to create and evaluate a novel question-and-answer-based speech-in-noise test called 'CHAT', where stimuli were synthesised using text-to-speech (TTS), responses captured using automatic speech recognition (ASR), and responses evaluated using a large language model (LLM). Three tests were compared: the newly developed CHAT, the Adaptive Sentence List (ASL) and the Four Alternative Auditory Feature Test (FAAF). CHAT used synthetic speech while ASL a

Leveraging computerized adaptive testing for cost-effective evaluation of large language models in medical benchmarking.

Large language models (LLMs) are increasingly applied in healthcare, yet their evaluation relies predominantly on static benchmarks that are costly, contamination-prone, and lack calibrated measurement properties for fine-grained performance tracking. We developed and validated a computerized adaptive testing (CAT) framework grounded in item response theory to enable scalable, psychometrically rigorous assessment of standardized medical knowledge in LLMs. A two-phase study comprising Monte Carlo simulations and empirical evaluation of 38 LLMs was conducted between July and September 2025. Evaluating the Performance of Large Language Models on Palliative Care Test Questions: A Mixed Methods Study.

BACKGROUND: Little is known about large language model (LLM) performance on palliative care (PC)-related knowledge-based tasks. We evaluated two LLMs in answering PC-related test questions and explaining their answer choice rationale. METHODS: LLMs were prompted to answer 25 randomly selected questions from the Fast Facts Quiz and provide their answer choice rationale. Three PC educators ranked and rated LLM-generated answer choice explanations versus the test's answer key explanations. Linear fixed-effect models evaluated reviewer ranking, and ordinal logistic regression evaluated reviewer r

Evaluation of causal reasoning for large language models in contextualized clinical scenarios of laboratory test interpretation.

This study evaluates causal reasoning in large language models (LLMs) using 99 clinically grounded laboratory test scenarios mapped to Pearl's Ladder of Causation: association, intervention, and counterfactual reasoning. We focused on common lab tests such as Hemoglobin A1c (HbA1c), creatinine, and vitamin D, and paired them with clinically relevant causal factors, including age, gender, obesity, and smoking. Two LLMs, GPT-o1 and Llama-3.2-8b-instruct, were tested, with responses rated by four medically trained human experts. GPT-o1 demonstrated superior discriminative performance (AUROC overa

Large language models' capabilities in responding to tuberculosis medical questions: testing ChatGPT, Gemini, and Copilot.

This study aims to evaluate the capability of Large Language Models (LLMs) in responding to questions related to tuberculosis. Three large language models (ChatGPT, Gemini, and Copilot) were selected based on public accessibility criteria and their ability to respond to medical questions. Questions were designed across four main domains (diagnosis, treatment, prevention and control, and disease management). The responses were subsequently evaluated using DISCERN-AI and NLAT-AI assessment tools. ChatGPT achieved higher scores (4 out of 5) across all domains, while Gemini demonstrated superior p

An update on determination of language dominance in screening for epilepsy surgery: the Wada test and newer noninvasive alternatives.

The intracarotid amobarbital procedure or Wada test has been the gold standard for lateralization of language dominance before epilepsy surgery. It is based on deactivation of language cortex with intracarotid anesthesia. However, it is an invasive test with risks and discomforts, and it also has limitations. There has been great interest in replacing the Wada test with a noninvasive procedure. One alternative, repetitive magnetic stimulation works by deactivating language cortex, but most other promising alternatives are based on brain activation. Functional magnetic resonance imaging (fMRI),

Determination of hemisphere dominance for language: comparison of frontal and temporal fMRI activation with intracarotid amytal testing.

The reliability of frontal and temporal fMRI activations for the determination of hemisphere language dominance was evaluated in comparison with intracarotid amytal testing (IAT). Twenty-two patients were studied by IAT (bilateral in 13, unilateral in 9 patients) and fMRI using a paradigm requiring semantic decisions. Global and regional (frontal and temporoparietal) lateralisation indices (LI) were calculated from the number of activated (r>0.4) voxels in both hemispheres. Frontolateral activations associated with the language task were seen in all patients, temporoparietal activations in 20

A Prospective Observational Cohort Study of Language Preference and Preoperative Cognitive Screening in Older Adults: Do Language Disparities Exist in Cognitive Screening and Does the Association Between Test Results and Postoperative Delirium Differ Based on Language Preference?

BACKGROUND: A greater percentage of surgical procedures are being performed each year on patients 65 years of age or older. Concurrently, a growing proportion of patients in English-speaking countries such as the United States, United Kingdom, Australia, and Canada have a language other than English (LOE) preference. We aimed to measure whether patients with LOE underwent cognitive screening at the same rates as their English-speaking counterparts when routine screening was instituted. We also aimed to measure the association between preoperative Mini-Cog and postoperative delirium (POD) in bo
