Large language models forecast patient health trajectories enabling digital twins.

Generative artificial intelligence is revolutionizing digital twin development, enabling virtual patient representations that predict health trajectories, with large language models (LLMs) showcasing untapped clinical forecasting potential. We developed the Digital Twin-Generative Pretrained Transformer (DT-GPT), extending LLM-based forecasting solutions to clinical trajectory prediction. DT-GPT leverages electronic health records without requiring data imputation or normalization and overcomes real-world data challenges such as missingness, noise, and limited sample sizes. Benchmarking on non

Semantic orientation for indoor navigation system using large language models.

Autonomous robots play an important role in modern indoor navigation, but existing systems often struggle to achieve seamless human interaction and semantic understanding of environments. This paper presents an Artificial Intelligence (AI)-driven object recognition system enhanced by Large Language Models (LLMs), such as GPT-4 Vision and Gemini, to bridge this gap. Our approach combines vision-based mapping techniques with natural language processing and interactions to enable intuitive collaboration on navigation tasks. By leveraging multimodal input and vector space analysis, our system achi

Re-evaluating Theory of Mind evaluation in large language models.

The question of whether large language models (LLMs) possess Theory of Mind (ToM)-often defined as the ability to reason about others' mental states-has sparked significant scientific and public interest. However, the evidence as to whether LLMs possess ToM is mixed, and the recent growth in evaluations has not resulted in a convergence. Here, we take inspiration from cognitive science to re-evaluate the state of ToM evaluation in LLMs. We argue that a major reason for the disagreement on whether LLMs have ToM is a lack of clarity on whether models should be expected to match human behaviours,

Optimal large language models to screen citations for systematic reviews.

Recent studies highlight the potential of large language models (LLMs) in citation screening for systematic reviews; however, the efficiency of individual LLMs for this application remains unclear. This study aimed to compare accuracy, time-related efficiency, cost, and consistency across four LLMs-GPT-4o, Gemini 1.5 Pro, Claude 3.5 Sonnet, and Llama 3.3 70B-for literature screening tasks. The models screened for clinical questions from the Japanese Clinical Practice Guidelines for the Management of Sepsis and Septic Shock 2024. Sensitivity and specificity were calculated for each model based

Personalized Impression Generation for PET Reports Using Large Language Models.

Large language models (LLMs) have shown promise in accelerating radiology reporting by summarizing clinical findings into impressions. However, automatic impression generation for whole-body PET reports presents unique challenges and has received little attention. Our study aimed to evaluate whether LLMs can create clinically useful impressions for PET reporting. To this end, we fine-tuned twelve open-source language models on a corpus of 37,370 retrospective PET reports collected from our institution. All models were trained using the teacher-forcing algorithm, with the report findings and pa

Enhancing Postmarketing Surveillance of Medical Products With Large Language Models.

IMPORTANCE: The Sentinel System is a key component of the US Food and Drug Administration (FDA) postmarketing safety surveillance commitment and uses clinical health care data to conduct analyses to inform drug labeling and safety communications, FDA advisory committee meetings, and other regulatory decisions. However, observational data are frequently deemed insufficient for reliable evaluation of safety concerns owing to limitations in underlying data or methodology. Advances in large language models (LLMs) provide new opportunities to address some of these limitations. However, careful cons

Perceived political bias in large language models reduces persuasive abilities.

Conversational AI has been proposed as a scalable way to correct public misconceptions, yet its effectiveness may depend on perceptions of political neutrality. As large language models become objects of partisan conflict, political elites increasingly portray them as ideologically aligned. We test whether such credibility attacks reduce persuasion by conversational AI. In a preregistered US survey experiment (N = 2,144), participants completed a three-round conversation with ChatGPT (GPT-4.1) about an economic policy misconception they personally held. The conversation corrected misconception

Large language models pass a standard three-party Turing test.

The Turing test has been widely discussed as a test of machine intelligence, but it also provides a measure of how humans distinguish other humans from machines. We evaluated 4 systems (ELIZA, GPT-4o, LLaMa-3.1-405B, and GPT-4.5) in two randomized, controlled, and preregistered Turing tests on independent populations. Participants had 5 min conversations simultaneously with another human participant and one of these systems before judging which conversational partner they thought was human. When prompted to adopt a humanlike persona, GPT-4.5 was judged to be the human 73% of the time: signific

Comparative assessment of large language models for microbial phenotype assignment.

BACKGROUND: Large language models (LLMs) are increasingly used to extract knowledge from text, yet their coverage and reliability in biology remain unclear. Microbial phenotypes are especially important to assess, as comprehensive data remain sparse except for well-studied organisms and they underpin our understanding of microbial characteristics, functional roles, and applications. RESULTS: Here, we systematically assessed the biological knowledge encoded in publicly available LLMs for structured phenotype assignment of microbial species. We evaluated the performance of up to 57 LLMs across

Large language models with image processing in automated Cobb angle.

BACKGROUND: The degree of scoliosis is assessed through the Cobb angle, which quantifies severity and is measured by clinicians on radiographs. With the increasing adoption of artificial intelligence (AI) in clinical workflows, there is uncertainty as to whether large language models (LLMs) with image processing capabilities can streamline and improve spinal deformity classification. This study aims to assess the diagnostic capabilities of 4 leading LLMs: ChatGPT, Gemini, Perplexity and Grok in calculating Cobb angles from radiographs. METHODS: A cross-sectional analysis of 122 scoliosis pati

Benchmarking large language models against practicing clinicians on psychopathological assessment.

Psychiatry's reliance on language makes LLMs a natural tool for psychopathological assessment, yet structured, item-level assessments from psychiatric clinical interviews remain under-researched. In this proof-of-concept study, 10 LLMs assessed transcripts of three simulated psychiatric interviews across all 100 items of the Association for Methodology and Documentation in Psychiatry (AMDP) system, benchmarked against 108 early-career clinicians rating full audiovisual recordings, using an expert consensus panel as reference. GPT-5.1 and Gemini-3-Pro-Preview achieved the highest accuracy (0.72

Causal networks guiding large language models: application to COVID-19.

In the context of diagnosis of COVID-19, this paper shows how to convert a Causal Network to a Large Language Model (LLM). The Causal Network was converted to the language model using prompts and completions. Prompts were composed from the full-factorial combination of the text associated with statistically significant variables in the Causal Network. Completions were based on the evaluation of the probability of COVID-19 using the Causal Network. The accuracy of the Causal Network and LLM was tested using two databases. The first database was based on a survey of 822 patients, collecting 12 d
