Difficult indications for partial laryngectomy.

Difficult indications for certain partial laryngectomies are presented. Anatomically, the difficulties stem from the thyroid cartilage and from the paraglottic space in which invasion (difficult to determine) is contradictory for partial surgery. The principal objective is the cure of the laryngeal cancer all in maintaining laryngeal function. The surgeon must conserve a 'minimal' arytenoid unit' (the whole formed by the pharyngoepiglottic fold, the aryepiglottic fold, the arytenoid and the corresponding cord). Other difficulties not carcinogenic are found: physical conditions, social and prof

Current attitudes of laryngectomy patients.

Improved surgical techniques and adjunctive therapeutic measures are producing more long-term survivors of laryngeal carcinoma and facilitating voice preservation through the methods of conservation surgery; however, when surgery includes total laryngectomy, prolonged rehabilitation involving many disciplines may help patients to adjust to many new aspects of their daily lives and to avoid the severe depression which may attend this procedure. In order to evaluate the program currently in use at our institution and to provide suggestions for developing and improving rehabilitative programs, we

The Benefits of Silicone Laryngectomy Tubes at the Time of Laryngectomy-A Case Series Spanning 17 Years.

OBJECTIVES: Although total laryngectomy (TL) is a well-established surgical procedure with clear functional or oncologic indications, the peri- and postoperative care for those undergoing TL is variable, particularly regarding postlaryngectomy tracheostoma management. This study examined TL outcomes from a single institution with the immediate perioperative use of soft silicone laryngectomy tubes. More specifically, we explored potential complications associated with immediate perioperative use of a flexible laryngectomy tube (LaryTube and StomaSoft) and the use of heat and moisture exchange (

Post-Laryngectomy stricture and pharyngocutaneous fistula: Review of techniques in primary pharyngeal reconstruction in laryngectomy.

OBJECTIVE: The purpose of this study was to find a correlation between closure technique in pharyngeal closure and outcomes of both pharyngocutaneous fistula and post-laryngectomy stricture after laryngectomy. STUDY DESIGN: Retrospective Chart Review. METHODS: We retrospectively reviewed a total of 151 patients over a 20-year period from January 1994 to December of 2013 who underwent primary pharyngeal reconstruction after total laryngectomy specifically looking at the closure technique in relation to pharyngo-cutaneous fistula (PCF) and post-laryngectomy stricture postoperatively. Quality of Life Trajectories After Partial Laryngectomy Versus Total Laryngectomy Stratified by Voice Rehabilitation Success.

BACKGROUND: Although partial laryngectomy (PL) is often preferred for functional preservation, long-term quality-of-life (QoL) outcomes after total laryngectomy (TL) may vary according to voice rehabilitation success. We compared five-year longitudinal QoL trajectories among PL, TL with successful voice rehabilitation (TL-S), and TL with failed rehabilitation (TL-F). METHODS: Ninety-two patients were categorized into PL (n = 52), TL-S (n = 21), and TL-F (n = 19). QoL was assessed from baseline to 5 years postoperatively using the European Organisation for Research and Treatment of Cancer Qual

A "royal" total laryngectomy that never was.

With this historical vignette, ending the series dedicated to the pioneers of total laryngectomy, published in the EuropeanAnnals of Otorhinolaryngology Head & Neck Diseases to mark the 150th anniversary of the first description of this surgical procedure in humans, the authors recount what history called "The Crown Prince's illness", and wonder what might have happened if the total laryngectomy proposed for Crown Prince Friedrich had finally been performed.

[Laryngectomy-still state of the art?].

Total laryngectomy provides a curative approach for patients with advanced laryngeal and hypopharyngeal cancer without distant metastasis. Especially in stage cT4a disease, laryngectomy is superior to primary radio(chemo)therapy in retrospective studies. Further relevant indications for the procedure are tumor-related laryngeal dysfunction such as dysphagia and aspiration, as well as cancer recurrence after primary radio(chemo)therapy. The surgical procedure is highly standardized, with an appropriate safety profile. The subsequent loss of voice must be compensated by voice rehabilitation (voi

Laryngectomy rehabilitation in the United Kingdom.

PURPOSE OF REVIEW: Organ preservation treatment paradigms have led to profound changes in the management of locally advanced laryngeal cancer. Whereas there has been a reduction in primary total laryngectomies, the proportion of patients needing laryngectomy after failed chemoradiation has increased. These changes have affected the rehabilitation service requirements and outcomes following rehabilitation in this group of patients. It is particularly important to understand the current issues as UK service commissioning and funding arrangements are undergoing significant changes at this time.

Is emergency laryngectomy a waste of time?

INTRODUCTION: Patients who present with airway obstruction due to carcinoma of the larynx may be managed by tracheotomy followed by definitive tumor surgery at a later date. An alternative is emergency laryngectomy, defined as total laryngectomy performed within 24 hours, for a previously untreated and undiagnosed malignancy. In this study, we compare and contrast the outcome of 13 patients managed by tracheotomy and delayed laryngectomy to another group of patients, previously reported, managed by emergency laryngectomy. MATERIALS AND METHODS: Stridor due to malignant laryngeal neoplasm was

[Functional recovery in subtotal laryngectomies].

Subtotal laryngectomy, a valid treatment for carefully selected patients, is a safe oncologic procedure which preserves the cricoid-arytenoid unit creating a successful "neo-larynx" with valid phonatory and deglutition functions. At the E.N.T. Clinic of "La Sapienza" University of Rome from Jan. 1984 to Feb. 1992, 85 subjects underwent subtotal laryngectomy, 50 of which then underwent phoniatric examination. The remaining patients were not suitable candidates because of the trachealis cannula (14), or because they were lost at follow up (16). A total of 50 male subjects were examined. Twenty-e

[Deglutition and partial supracricoid laryngectomies].

The indications of partial supracricoid laryngectomy have become more numerous since the first descriptions made by Majer, Labayle and Piquet. The study of the published functional results emphasizes that false passages during deglutition are the main postoperative problem. This technical note is aimed at drawing attention to the various points to be respected intra- and postoperatively to allow swallowing in as physiological a manner as possible.

Pathways and pitfalls in partial laryngectomy.

Some 350 surgical specimens obtained at partial or total laryngectomy were photographed immediately after operation and processed for study by serial section. Intralaryngeal barriers to the spread of cancer are demonstrated on one hand and pathways of infiltration and destruction on the other. The following conclusions are based on these studies: two major elastic tissue barriers that influence the spread of vocal cord cancer are the dense mass of connective tissue at the anterior commissure which prevents invasion of the thyroid cartilage until very late, and the conus elasticus, under which

[Indications for botulinum toxin in laryngectomy].

The botulinum toxin is a highly potent neurotoxin, used since several years in the treatment of the focal musculary dystonias. We define the laryngeal dystonia as a clinical entity, which is represented by a spasmodic dysphonia or an inspiratory dyspnea without dysphonia, related to a specific dystonia of the thyroarytenoid muscles. The laryngeal functional exploration (i.e. fibroscopy, videostroboscopy, acoustic analysis, computerized voice analysis), the neurological and electrophysiological assessment allow to make diagnosis and evaluation of the therapeutical results of this rare laryngeal

Supracricoid partial laryngectomy cricohyoidoepiglottopexy (SCPL-CHEP) versus vertical partial laryngectomy for the treatment of glottic carcinoma.

The study aimed to compare the survival rate and functional outcome of supracricoid partial laryngectomy cricohyoidoepiglottopexy (SCPL-CHEP) and vertical partial laryngectomy (VPL) in the treatment of glottic carcinoma. Thirty-nine patients underwent SCPL-CHEP and 45 patients underwent VPL between 2002 and 2007. Post-operative clinical staging were performed according to the UICC staging criteria (2002). The primary endpoints were survival time and recurrence rate. The secondary endpoints were the laryngeal functions including pronunciation, swallowing, and respiration. Overall, the estimated

[Methods of Voice Restoration following Laryngectomy].

In Germany, approximately 950-1200 laryngectomies are performed annually due to extensive hypopharyngeal and laryngeal carcinomas. The removal of the larynx results in significant life changing functional losses, particularly in voice production and communication, resulting in psychological issues and social withdrawal. Rehabilitation requires a multidisciplinary team focusing on restoring communication skills. Methods include pseudowhispering, esophageal speech, external mechanical and electromechanical aids, and tracheoesophageal speech prostheses. Pseudowhispering uses air from the mouth an

Epiglottoplasty technique in endoscopic partial laryngectomy.

The main advantage of endoscopic laser surgery for laryngeal cancer is to allow tumour resection, while limiting functional sequelae, thereby improving the postoperative course. In this type of surgery, the epiglottis is often partially resected, leaving a raw zone without any reconstruction. The surgical technique described here involves endoscopic reconstruction of the epiglottis after partial resection. The sectioned edge of the epiglottis is sutured to the base of the tongue to create a neoepiglottis and to reconstruct the vallecula, thus resembling preoperative anatomy, allowing improveme
