Cervical artery dissection (CAD), including carotid and vertebral artery dissection (VAD), is an uncommon but important cause of ischemic stroke in young adults. We report the case of a 32-year-old previously healthy woman who developed lateral medullary (Wallenberg) syndrome secondary to a right VAD following a high-intensity CrossFit session involving heavy lifting. She initially presented with right-sided headache and dizziness, which were misattributed to migraine, and over the following days developed dysphagia, right facial hypoesthesia, Horner's syndrome, and contralateral sensory loss.

INTRODUCTION AND IMPORTANCE: A unique etiology of stroke, lateral medullary syndrome (LMS), is a consequence of posterior inferior cerebellar artery or vertebral artery thromboembolic conditions. LMS patients present particularly with ipsilateral hyperalgesia, ipsilateral ataxia, and Horner's syndrome. Our case signifies that neurogenic origin should always be considered in the absence of local causes of dysphagia. Early diagnosis could prevent LMS complications, including neurological disabilities. A scarcity of research related to dysphagia in LMS, and its outcomes exists. Therefore, the obj

Central alveolar hypoventilation syndrome (CAHS) is a rare and potentially fatal condition. However, respiratory care for patients with CAHS caused by lateral medullary infarction (CAHS-LMI) remains an important unsolved problem. We describe 2 patients with CAHS-LMI and review the case reports for 17 previously described patients. Patient 1 was a 78-year-old man who was referred to our hospital because of dizziness. After admission, Wallenberg syndrome developed. Magnetic resonance imaging showed left LMI. He had hypercapnia and respiratory acidosis the next afternoon and temporarily received

Herpes simplex encephalitis is a rare complication of Herpes virus infections. Innate immune mechanisms are the first line of defence encountered by invading infectious agents. A 41-year-old man was admitted to the neurology department with the complaints of fever, headache, vertigo, tinnitus and ataxia. His first brain Magnetic Resonance Imagine showed nodular lesions in the medulla oblongata and the second showed a new left occipital lobe lesion in addition. In sera, Herpes Simplex Virus IgG and M values were positive and liver enzymes were found to be elevated. His diagnosis was Herpes ence

In a patient with unilateral pharyngeal paralysis, rotation of the head to the paralyzed side can effectively close the hemipharynx on that side. However, the exact level or place of closure is unknown. Serial computed tomography of the pharynx in a patient with lateral medullary syndrome showed that hemipharyngeal closing occurred at the level of the hyoid bone, or the hypopharyngeal cavity above the pyriform sinus, and that the entire space of the bilateral pyriform sinuses remained open despite the head rotation.

We present a patient with relapsing polychondritis who presented with findings of lateral medullary syndrome, known as the syndrome of Wallenberg, produced by infarction of a wedge of lateral medulla lying posterior to the inferior olivary nucleus. Angiographic examination showed complete occlusion of the right posterior inferior cerebellar artery and segmental narrowings in the basilar artery. We believe that the angiographic findings were based on an underlying vasculitis because of his relapsing polychondritis. This is the first case of relapsing polychondritis with angiographically demonst

Lateral medullary syndrome is a collection of different neurological symptoms after ischemic/hemorrhagic insult to either the posterior inferior cerebellar artery/vertebral artery or rarely the anterior inferior cerebellar artery, causing infarction of ipsilateral cerebellum and posterolateral medulla (Tiedt and Weidauer, 2013). Dizziness, nausea, vertigo, vomiting, nystagmus, ataxia, dysphagia, hoarseness of voice, ptosis, and sensory impairment of face and body are typical. Here we report an unusual presentation of a 47-year-old female who was hypertensive and diabetic and who complained of

BACKGROUND: Diagnosis of lateral medullary syndrome (LMS) is often delayed due to elusive clinical presentations and frequently non-revealing neuroimaging tests. We aimed to investigate the use of ipsilateral vocal cord paresis (VCP) identified on neck computed tomography angiography (CTA) as an early diagnostic sign for LMS.
METHODS: Medical records were queried for patients admitted with LMS between 1/2012 and 10/2020. A control group of patients undergoing CTA for transient or no neurological symptoms was matched for sex and age. Clinical data were collected by a stroke neurologist. Two ne

We have recently encountered a rare case of fibromuscular dysplasia (FMD) of the vertebral artery (VA) presenting as lateral medullary syndrome. A 39-year-old male was admitted to our hospital due to vertigo, dysarthria and numbness of the left face and the right limbs. A magnetic resonance imaging (MRI) and magnetic resonance angiography (MRA) of brain revealed lateral medullary infarction in the territory of the left posterior inferior cerebellar artery (PICA). The angiography of the VA revealed tubular stenosis of the left extracranial VA and a focal vascular kinking as well as web in the r

Lateral medullary syndrome (LMS), or Wallenberg syndrome, typically arises from ischemia of the dorsolateral medulla and is characterized by crossed sensory deficits, ipsilateral cranial nerve involvement, cerebellar ataxia, and Horner's syndrome. We report a 66-year-old hypertensive female who presented with classical clinical features suggestive of LMS, including ipsilateral facial sensory loss, contralateral body sensory impairment, palatal weakness, ipsilateral Horner's syndrome, and cerebellar signs. However, diffusion-weighted magnetic resonance imaging revealed a predominantly paramedia

Lateral medullary infarction is rarely associated with Opalski syndrome and autonomic dysfunction. Herein, we present the first case involving a 66-year-old patient with cancer who developed mild dysarthria, left-sided Horner's syndrome and lower limb-dominant ataxic hemiparesis, reduction in right-sided painful thermal sensation, and severe truncal ataxia, followed by orthostatic headache. The Schellong test revealed severe orthostatic hypotension. The patient was diagnosed with Opalski syndrome associated with headache attributed to orthostatic hypotension caused by the cerebral infarction i

We describe an adult patient who developed widespread skin ulceration in the left trigeminal nerve distribution due to a Wallenberg syndrome from an acute ischemic stroke in the left lateral medulla. Trigeminal trophic syndrome is a rare consequence of a trigeminal nerve pathway injury characterized by the triad of persistent ulceration, anesthesia, and paresthesia of the same trigeminal dermatomes. It is an unusual complication of stroke that most likely developed in this patient from a combination of neurotrophic skin changes worsened by self-inflicted injury from scratching in a hypoestheti

Gorham-Stout disease (GSD) is a rare condition in which spontaneous, progressive resorption of bone occurs. There are no previous reports of patients with fatal progression of GSD with skull base osteomyelitis (SBO) and lateral medullary syndrome (LMS). We present the case of a 27-year-old man diagnosed with GSD with involvement of the maxillofacial bones and skull base. The patient developed SBO; LMS resulted from progressive osteolysis, and the patient died of associated brainstem stroke. Careful follow-up with special emphasis on the early detection of intracranial complications is critical

Altered pain appreciation and autonomic function are hallmarks of Cardiac syndrome X, Irritable bowel syndrome and Reflex sympathetic dystrophy. Both pain appreciation and autonomic function are controlled by the lateral medulla. This hypothesis proposes that lateral medullary ischaemia at a microvascular level is responsible for these syndromes and could also be linked to other conditions where autonomic dysfunction is a major feature such as late-onset asthma, type 2 diabetes and essential hypertension. Autonomic function is controlled by the nucleus tractus solitarius, which acts as the mai

INTRODUCTION: Lateral medullary syndrome, a common type of posterior circulation stroke, typically presents with classic symptoms including vertigo, gait ataxia, hoarseness, and dysphagia. In several cases, it may also lead to ipsilateral facial pain consistent with trigeminal neuralgia. Current management mainly involves pharmacological or surgical interventions, which are often associated with adverse effects and limited efficacy.
CASE PRESENTATION: A 52-year-old ethnic Han male patient with trigeminal neuralgia secondary to lateral medullary syndrome received structure-based medical acupun

Lateral medullary syndrome is rare in pediatrics. It is characterized by neurological deficits due to an ischemic lesion in the lateral medulla. The authors describe a 17-year-old boy who developed lateral medullary syndrome in the context of a hyperflexion neck injury while diving in shallow water with traumatic vascular injury. He had "crossed" neurological deficits above and below the neck. His magnetic resonance angiography showed intra- and extracranial left vertebral artery occlusion and his magnetic resonance imaging showed signal abnormality involving the left lateral medulla and infer

INTRODUCTION: Wallenberg's syndrome and ipsilateral paresis due to combined infarction of the lateral medullary and cervical spinal infarction is known as Opalski syndrome. This rarely described syndrome was reported, to our knowledge, with DWI MRI, only once.
CASE REPORT: We report the case of a 43-year-old man with autosomal dominant polycystic kidney disease who, after a brief episode of coma, developed Wallenberg syndrome and ipsilateral hemiparesis. Initial diffusion weighted-imaging MRI showed a high-intensity signal involving the lateral medulla oblongata and the spinal cord; but FLAIR

A 48-year-old man with no known risk factor for cerebrovascular disease, other than cigarette smoking, experienced the sudden onset of a mixed lateral and medial medullary syndrome. Computed tomography scan failed to show any definite abnormality. Magnetic resonance imaging scans revealed hemorrhage restricted to the left dorsolateral medulla. Angiography showed abnormal arteries originating from the left vertebral artery with small niduses located on the surface of the medulla and contralateral cerebellum. Small brain-stem hemorrhages are a contraindication to thrombolytic or anticoagulant th
