Patients With Larger Left Atrial Appendage Orifice Presented Worse Prognosis Contributed by Acute Heart Failure After Left Atrial Appendage Closure.

Background Left atrial appendage (LAA) closure (LAAC) could prevent stroke in patients with atrial fibrillation. However, LAAC may impair the compliance of the left atrium and result in poor prognosis. This study aimed to comparatively evaluate the prognosis of LAAC among patients with different sizes of LAA orifice. Methods and Results Three hundred two consecutive patients who underwent successful LAAC were included and divided into 4 groups based on LAA orifice size that was measured using transesophageal echocardiography. Clinical outcomes including thromboembolic events, major cardiocereb

Left Atrial Appendage Closure in Patients With Intracranial Hemorrhage and Nonvalvular Atrial Fibrillation.

BACKGROUND: Left atrial appendage (LAA) closure is primarily performed in patients who have contraindications to anticoagulants. However, anticoagulants are recommended for the first few weeks after LAA closure to prevent the device related thrombus. Purpose of this study was to evaluate if short term use of anticoagulant is safe and effective after LAA closure in patients with a history of intracranial hemorrhage. METHODS: It is a retrospective observational study done at a single center. Baseline characteristics, perioperative, and postoperative complications of patients with a history of i

Left atrial appendage closure - new choice for no-option patients with atrial fibrillation.

AIM: Remodeling and impaired blood flow in left atrial appendage (LAA), which occurs in patients with atrial fibrillation (AF), may lead to thrombus formation and possible thromboembolic complications. Although there are several pharmacological antithrombotic possibilities, some patients with several co-morbidities and contraindications to such treatment cannot be offered any of them. Therefore LAA closure systems may be an attractive alternative. We present our early experience with two currently available different LAA transcatheter closure systems (Watchman and Amplatzer Cardiac Plug). MET

Recurrent Left Pleural Effusion Following Left Atrial Appendage Closure With the Watchman Device.

A 75-year-old woman with recent left atrial appendage closure with the Watchman device (Boston Scientific, Natick, Massachusetts) presented with recurrent left pleural effusion. The constellation of chest pain, pericardial effusion, and exudative pleural effusion were suggestive of an inflammatory process precipitated by microperforation of the fixation anchors during the Watchman placement. (Level of Difficulty: Intermediate.).

Transcatheter closure of patent foramen ovale, atrial septal defect and left atrial appendage.

Intracardiac defects such as atrial septal defect (ASD) and patent foramen ovale (PFO) are common forms of congenital intracardiac apertures which can be successfully closed percutaneously. Since the initial description of an atrial septal defect closure device in the mid 1970s by King and Mills, transcatheter closure of atrial septal defects and patent foramen ovale using various devices has now become an established practice in many centers. The left atrial appendage is a trabeculated remnant of the embryonic left atrium. This is an important source of emboli related to atrial fibrillation.

Multimodality imaging assessment of the left atrial appendage for percutaneous closure.

Left atrial appendage (LAA) closure has emerged as a critical therapeutic option for stroke prevention in patients with atrial fibrillation who are unsuitable for long-term oral anticoagulation. Multimodality imaging plays a pivotal role throughout the LAA closure process, from pre-procedural planning to long-term follow-up. This review focuses on the complementary roles of cardiac computed tomography (CCT) and transesophageal echocardiography (TEE), outlining their respective strengths and limitations in various phases of LAA management, while also discussing the roles of intracardiac echocar

The left atrial appendage: An enigmatic friend or foe and implications of closure.

The left atrial appendage (LAA) is often thought of as a vestigial organ serving as a nidus for clot formation in those with atrial fibrillation (A-fib). The LAA, however, has unique anatomy which allows it to serve special functions in the human body. Closing the LAA has been shown to decrease the risk of thromboembolic events in patients who cannot tolerate anticoagulation. Several methods of closure exist including percutaneous endocardial closure, epicardial closure, and surgical clipping. In addition to decreasing stroke risk, there appears to be physiologic changes that occur after LAA c

Who can benefit from percutaneous left atrial appendage closure? Recent advances.

Atrial fibrillation (AF) confers a substantial risk of stroke. Direct  oral anticoagulants (DOACs) are highly effective for stroke prevention and form the cornerstone of AF management. Nevertheless, many eligible patients do not receive or do not maintain long-term anticoagulation because of bleeding risk, prior hemorrhage, or other contraindications. Percutaneous left atrial appendage closure (LAAC) has emerged as a non-pharmacological alternative. In 2026, two major randomized trials addressing the role of LAAC as an alternative to anticoagulation were published: CHAMPION-AF and CLOSURE-AF.

Descending Aorta Perforation During Transseptal Puncture for Left Atrial Appendage Closure.

BACKGROUND: Aortic root injury during transseptal puncture (TSP) is a rare but well-recognized complication. However, injury to the descending aorta has not previously been described in this context. CASE SUMMARY: A 57-year-old woman underwent left atrial appendage closure. During TSP using the VersaCross Connect radiofrequency-powered system and a 15-F steerable sheath, an unrecognized perforation of the descending aorta occurred. Despite repeated transthoracic echocardiograms showing no pericardial effusion and initial hemodynamic stability, she subsequently developed a mediastinal hematoma

Direction-Controlled Retrieval of an Embolized Left Atrial Appendage Closure Device.

BACKGROUND: Embolization of left atrial appendage (LAA) closure devices is an uncommon but potentially serious complication. Conventional snaring techniques may not provide adequate control of device orientation, increasing the risk of anchor entrapment and vascular injury. CASE PRESENTATION: A 67-year-old man with persistent atrial fibrillation and recurrent colonic diverticular bleeding underwent LAA closure using a 31-mm WATCHMAN FLX Pro device. Despite satisfactory positioning based on the PASS (Position, Anchor, Size, Seal) criteria, device embolization to the aortic arch occurred shortl

Left Atrial Appendage Closure: Between Imaging Precision and Uncertain Clinical Benefit.

Left atrial appendage closure has emerged as an established nonpharmacological strategy for stroke prevention in selected patients with atrial fibrillation, particularly those in whom long-term oral anticoagulation is problematic. Its contemporary role, however, is defined by a central tension between procedural precision and clinical uncertainty. Advances in multimodality imaging, including transesophageal echocardiography, cardiac computed tomography, three-dimensional echocardiography, fusion imaging, and emerging computational tools, now allow highly accurate assessment of left atrial appe

Left atrial appendage closure: Current status, unresolved issues, and future perspectives.

Left atrial appendage closure (LAAC) is an established therapy for stroke prevention in patients with nonvalvular atrial fibrillation (AF) who cannot tolerate long-term oral anticoagulation (OAC). In Japan, LAAC was introduced in 2019 and has been performed in more than 10,000 cases a0s of 2025. Although indications remain limited to high-risk patients with OAC contraindications, procedural volume continues to rise. This review outlines the current status, remaining challenges, and future perspectives of LAAC, with emphasis on the WATCHMAN device. The WATCHMAN FLX and FLX Pro have enhanced pro
