Auer Rod-Like Inclusions in Prolymphocytic Leukemia.

BACKGROUND: Prolymphocytic leukemia (PLL) is a generalized malignancy of the lymphoid tissue, usually of B cell type. Auer rod-like inclusions in prolymphocytic leukemia cells are an extremely rare event; the inclusions are very similar to the Auer rods morphologically. METHODS: We describe a case of B-cell PLL presenting with Auer rod-like inclusions. The diagnosis was eventually proven by the morphology, cytochemical staining, immunophenotypes, and electron microscopy. RESULTS: Auer rod-like inclusions are pathological changes of mitochondria with increasing density of matrix and disappear

[Prolymphocytic leukemia, description of 2 cases].

We present the clinical and laboratory features of 2 patients with B prolymphocytic leukemia. Both are females of the fifth and seventh decade of life. One had the classical clinical picture of massive splenomegaly and a high white cell count, with characteristic prolymphocytes, and the other was asymptomatic, with a low white cell count. The cells were positive to B cell lineage reagents with strong surface immunoglobuline (Ig) and unreactive to T cell antibodies. Analysis of Ig genes at the DNA level demonstrated that both cases had heavy-chain gene rearrangements, confirming the B-cell orig

BCL1, a murine model of prolymphocytic leukemia. II. Morphology and ultrastructure.

The transplantable murine B-cell leukemia, BCL1, was examined by light and electron microscopy. The results show that the BCL1 cell closely resembles the prolymphocyte of human prolymphocytic leukemia (PLL). The spleen of animals transplanted with BCL1 is diffusely infiltrated by BCL1-prolymphocytes. Splenectomized animals given transplants of BCL1 develop a lymphomalike disease with less leukemia and bone marrow involvement than the nonsplenectomized transplant recipients. Ultrastructural features of the BCL1 cell are identical in the splenectomized and nonsplenectomized animals.

Prolymphocytic Leukemia: New Insights in Diagnosis and in Treatment.

PURPOSE OF REVIEW: We aimed to produce a comprehensive update on clinical and biological data regarding two rare lymphoid neoplasms, B and T prolymphocytic leukemias, and assess therapeutic management in the light of new molecular insights and the advent of targeted therapies. RECENT FINDINGS: B cell prolymphocytic leukemia (B-PLL) diagnosis remains challenging in the absence of clear immunophenotypic or cytogenetic signature and overlap with mantle cell lymphoma. New molecular defects have been identified in T cell prolymphocytic leukemia (T-PLL), especially in the JAK STAT pathway. Like in

Splenic B-Cell Lymphomas with Diffuse Cyclin D1 Protein Expression and Increased Prolymphocytic Cells: A Previously Unrecognized Diagnostic Pitfall.

Prolymphocytic transformation is a concept usually applied in the context of chronic lymphocytic leukemia/small lymphocytic lymphoma to describe the presence of a high percentage of prolymphocytes in peripheral blood (usually more than 55%). Prolymphocytic transformation has also been reported in mantle cell lymphoma (MCL) but only rarely in splenic marginal zone lymphoma (SMZL). We present two splenic B-cell lymphomas presenting in the leukemic phase and with increased prolymphocytes, both classified as SMZL with prolymphocytic transformation. One case clinically simulated B-prolymphocytic le

Enzymatic differences between chronic lymphocytic leukemia and prolymphocytic leukemia.

Chronic lymphocytic leukemia and prolymphocytic leukemia of the B-cell immunophenotype are closely related disorders, but differ in their cytomorphologic and clinical features. In an attempt to differentiate further between these two forms of leukemia, we measured adenosine deaminase and purine nucleoside phosphorylase activities by using a linked-enzyme spectrophotometric assay on peripheral-blood leukemic cells from seven patients with chronic lymphocytic leukemia, three patients with prolymphocytic leukemia, and one patient with prolymphocytoid transformation of chronic lymphocytic leukemia

Prolymphocytic leukemia: histologic findings in atypical cases.

Significant enlargement of peripheral lymph nodes is not characteristic of prolymphocytic leukemia. The absence of this physical feature of the disease has been so consistent in previous reports that it has come to have considerable importance in the differential diagnosis of prolymphocytic leukemia. We describe two cases of prolymphocytic leukemia, in which, in sharp contrast to other published cases, striking lymphadenopathy was present throughout the clinical course. In one case, the disease responded dramatically, but briefly, to L-asparaginase. The immunologic characteristics of cells fro

Splenic B-Cell Lymphoma/Leukemia With TCL1 Gene Rearrangement.

BACKGROUND: Splenic B-cell lymphoma/leukemia includes a group of indolent small B-cell lymphomas/leukemias that share overlapping morphological and immunophenotypic features, making accurate diagnosis challenging. Specific gene rearrangements have not been commonly associated with splenic B-cell lymphomas/leukemias, and the TCL1 gene rearrangement-characteristic of T-cell prolymphocytic leukemia-has not been previously reported in B-cell lymphomas/leukemias. Here, we report two unique cases of splenic B-cell lymphoma/leukemia harboring an IGH::TCL1 rearrangement. CASE PRESENTATION: Patient #1

Fibromodulin, an extracellular matrix protein: characterization of its unique gene and protein expression in B-cell chronic lymphocytic leukemia and mantle cell lymphoma.

Fibromodulin is an extracellular matrix protein normally produced by collagen-rich tissues; the fibromodulin gene has been found to be the most overexpressed gene in B-cell chronic lymphocytic leukemia. In this study, fibromodulin was expressed at the gene level (reverse transcription-polymerase chain reaction [RT-PCR]) in all patients with B-CLL (n = 75) and in most (5 of 7) patients with mantle cell lymphoma (MCL). No mutations in the fibromodulin gene were detected. Fibromodulin was also detected at the protein level in the cytoplasm of the B-CLL cells and in the supernatant after in vitro

[Prolymphocytic and hairy cell leukemias as special forms of chronic lymphatic leukemia].

Cytochemical and immunological studies were performed on "hairy" cells from 5 patients. The results, primarily the detection of surface immunoglobulins after incubation in immunoglobulin-free medium support the hypothesis that hairy cells are B-cells. Further parameters for B-cell lineage are the failing suppression of unspecific esterases by NaF, the presence of tartrate-resistant acid phosphatase and the absence of phagocytic capacity and glass adherence (skin window). A lysozyme could not be demonstrated. Of two patients with prolymphocytic leukemia one was characterized as T- and one as B-

Chronic lymphoid leukemias: recent advances in biology and therapy.

There exists a wide variety of lymphoid leukemias derived from B and T lymphocytes. These diseases have distinct immunologic and biologic features as well as varied responses to therapeutics. The most common lymphoid leukemia is chronic lymphocytic leukemia (CLL) which is a clonal proliferation of a subset of B cells expressing the CD5 antigen. Prolymphocytic leukemia is usually derived from B cells and shares some features with CLL but is clearly a distinct entity. Hairy-cell leukemia is a B cell malignancy that is uniquely responsive to a variety of biologic and chemotherapeutic agents. Wald

[Non-Hodgkin's lymphoma. Cytology and cytochemistry].

Imprints of lymph nodes and tumor specimens from 442 patients with non-Hodgkin's lymphoma (NHL) were evaluated cytologically and cytochemically. With the exception of hairy cell leukemia, special forms of peripheral pleomorphic T-cell lymphoma, and lymph node plasmacytoma, all types of NHL of the Kiel classification were included in this study. The investigations were performed on slides stained with Pappenheim (May-Grünwald-Giemsa) or conventional cytochemical techniques for substrate and enzyme demonstration, such as periodic acid Schiff (PAS), neutral and acid non-specific alphanaphthyl ace

Prolymphocytic transformation of chronic lymphocytic leukemia.

This report describes eight patients with B-cell chronic lymphocytic leukemia whose disease became more aggressive over a variable period of time. This clinical progression was associated with a change in cell morphology from small lymphocytes to an increasing number of large transformed lymphocytes in the blood, bone marrow, and lymph nodes. In the peripheral blood, the predominant large cell was a prolymphocyte. The small lymphocytes and the prolymphocytes had identical cell surface markers in each patient. However, the prolymphocytes had a greater density of surface immunoglobulin than did

Prolymphocytic leukemia.

Prolymphocytic leukemia is a rare chronic lymphoproliferative disorder that includes two subtypes, B cell and T cell, each with its own distinct clinical, laboratory and pathological features. T-cell prolymphocytic leukemia has an aggressive course with short median survival and poor response to chemotherapy. With the use of the purine analogue pentostatin more than half of patients will have a major response and a minority will have a complete remission, usually lasting months. With the introduction of alemtuzumab, most patients who progressed despite treatment with pentostatin had a major re

Detection of subgroups of chronic B-cell leukemias by FMC7 monoclonal antibody.

Monoclonal antibody FMC7 detects subgroups of B-cell leukemias that have arisen from cells in late stages of B-cell maturation. FMC7 was studied by flow cytometry on cell samples from 192 patients with a diagnosis of chronic lymphocytic leukemia (CLL) or lymphoma. The leukemic cells from 16 patients were reactive with this antibody. These 16 cases were evaluated for other surface markers, morphology of cells, and clinical characteristics. Of the 16 patients, 14 had cells that strongly expressed surface immunoglobulin (SIg). This is atypical of CLL cells, which characteristically show weak expr

The last revised "Euro-American classification" of lymphoid leukemias and non-Hodgkin's lymphomas: the same inaccuracies and inconsistencies in a chaotic complexity.

After a review of the recent physiologic, cellular genetic and molecular genetic acquisitions, a critical comment of the proposed classification is presented concerning especially a) the inclusion in the so-called "precursor B-lymphoblastic leukemias", which are pre-B neoplasias, of Burkitt's leukemic lymphoma, the cells of which are sIg+, hence B and not pre B; b) the inclusion in the chronic B lymphocytic leukemia of the so-called Galton's "prolymphocytic" leukemia, the cells of which are also sIg+, thus B and not pro B. In fact, the transformed blastoid medium size cells of this leukemia pr

T cell prolymphocytic leukemia. 2 cases having a postthymic helper phenotype with complement receptors and 14q+ chromosome abnormality.

We describe 2 elderly patients with splenomegaly and progressive bone marrow failure due to infiltration by leukemic prolymphocytes with a single prominent nucleolus. In each case the leukemia cells had a unique helper T cell phenotype with complement receptors and contained coarse blocks of acid alpha-naphthyl butyrate. A 14q+ chromosome was among the abnormalities that marked one abnormal T cell clone. Histology of the spleen showed mainly red pulp infiltration merging with periarteriolar regions and different from the pseudonodular pattern described in many cases of B cell prolymphocytic le

Lymphocyte function-associated antigen 1 (LFA-1) is a marker of mature (immunocompetent) lymphoid cells. A survey of lymphoproliferative diseases in man.

Lymphocyte function-associated antigen 1 (LFA-1) is a membrane glycoprotein involved in a wide variety of functional activities mediated by human leukocytes. For example, expression of LFA-1 is required on immunoregulatory lymphocytes for functional activity. The appearance of LFA-1 in lymphocyte ontogeny was studied using leukemic cells as probes of distinct maturation stages. LFA-1 was expressed on the neoplastic cells of all mature T-cell neoplasias including T-prolymphocytic leukemia cells but not on immature T-cell blasts in acute lymphoblastic leukemia. The expanded T gamma-cell populati

A Novel t(10;22) Translocation Harboring an IGL Gene Deletion in a CLL Patient Transforming to B-PLL with 1q Gain.

We report on a rare case of B-cell prolymphocytic leukemia (B-PLL) in a patient with a history of chronic lymphocytic leukemia (CLL) that showed a novel translocation t(10;22)(q21;q11.22) and an interstitial deletion of 11q14.1-q23.3 in 2017. The chromosome microarray analysis (CMA) confirmed the 11q22 deletion and revealed a small interstitial deletion of IGL gene. In 2018, the patient presented with worsening lymphocytosis, anemia and thrombocytopenia. The peripheral blood smear revealed an increased prolymphocyte population, which comprised 60.4% of lymphoid cells, establishing a diagnosis

Prolymphocytic Progression in Chronic Lymphocytic Leukemia Without Richter Transformation: An Eighteen-Year Clinical-Diagnostic Correlation.

Chronic lymphocytic leukemia (CLL) is an indolent neoplasm of small, mature B lymphocytes characterized by clumped chromatin and scant cytoplasm. A minor subset of neoplastic cells may exhibit prolymphocytic morphology, with prominent nucleoli and moderate cytoplasm. An increased proportion of prolymphocytes in peripheral blood often correlates with an accelerated or aggressive disease course, distinct from Richter transformation (RT), which more commonly manifests as diffuse large B-cell lymphoma and requires intensified therapy. We report an 18-year clinical course of CLL in a 57-year-old ma
