Acute Promyelocytic Leukemia Treatment Masking Hepatic Tuberculosis: A Management Dilemma.

Acute promyelocytic leukemia is a form of acute myeloid leukemia (AML) that is characterized by presence of a promyelocytic leukemia-retinoic acid receptor alpha fusion. In most patients, this fusion is detected on conventional karyotype as the t(15;17)(q24.1;q21.2) translocation, but some patients have cryptic translocations with a normal karyotype. Historically, AML is associated with a poor prognosis. Treatment with all-trans retinoic acid and arsenic trioxide assures long-term survival in the majority of patients. This treatment is generally well-tolerated but may cause hepatotoxicity. Thi

Deep learning identifies Acute Promyelocytic Leukemia in bone marrow smears.

BACKGROUND: Acute promyelocytic leukemia (APL) is considered a hematologic emergency due to high risk of bleeding and fatal hemorrhages being a major cause of death. Despite lower death rates reported from clinical trials, patient registry data suggest an early death rate of 20%, especially for elderly and frail patients. Therefore, reliable diagnosis is required as treatment with differentiation-inducing agents leads to cure in the majority of patients. However, diagnosis commonly relies on cytomorphology and genetic confirmation of the pathognomonic t(15;17). Yet, the latter is more time con

[Diagnostic efficacy of WT1 expression in asymptomatic acute promyelocytic leukemia].

We report a case of early asymptomatic acute promyelocytic leukemia (APL) with leukopenia as the only hematologic abnormality. A 55-year-old woman was referred to our hospital with leukopenia (white blood cell [WBC] count of 1,500/µl with 36% neutrophils), which was incidentally determined during an annual medical checkup. Two months before the presentation, her WBC was 3,400/µl with 60% neutrophils. A WBC count was 1,200/µl with 40% neutrophils. Immature myeloid cells were not observed. Her hemoglobin level and platelet count were normal. Moreover, no clinical or laboratory evidence was sugge

Diagnosis of Acute Promyelocytic Leukemia After Presentation to Neuro-Ophthalmology.

BACKGROUND: Acute promyelocytic leukemia (APML) is a medical emergency that can initially present with neuro-ophthalmologic signs. Early recognition is crucial, and immediate treatment with all-trans retinoic acid (ATRA) and arsenic trioxide (ATO) can be life-saving. The goal of this study was to describe patients who first presented to neuro-ophthalmology and were subsequently diagnosed with APML. METHODS: We retrospectively reviewed consecutive patients seen at a tertiary neuro-ophthalmology centre. Patients with an unknown diagnosis of APML at presentation who subsequently went on to recei

Herpes zoster during arsenic trioxide therapy for acute promyelocytic leukemia.

Historically, arsenic exposure has been associated with herpes zoster (HZ) infection, however the risk is not well characterized in arsenic trioxide (ATO) treated patients with acute promyelocytic leukemia (APL). We aimed to characterize the risk of HZ in 112 ATO treated patients with APL with and without antiviral prophylaxis (AVP). HZ occurred in 13/112 (11.6%) within 6 months of completing ATO, including one case of HZ encephalitis. AVP reduced the incidence of HZ (17.5% vs. 4.1%, RR 0.24 [95% CI 0.05-1.0, p = .025]) with a number needed to treat of 7.7. HZ despite AVP occurred later than H

Acute promyelocytic leukemia with JAK2 V617F and severe differentiation syndrome.

Myeloproliferative neoplasms transformed into AML usually have a poor prognosis. We report a case of essential thrombocythemia with myelofibrosis that transformed into acute promyelocytic leukemia (APL) with both the t(15;17) translocation as well as the JAK2 V617F mutation. Clinically, this case was notable for severe differentiation syndrome despite treatment with high-dose dexamethasone. Cytokine production by differentiating APL cells was not directly abrogated by JAK2 inhibitors in vitro, suggesting that JAK2 V617F enhances the hyperinflammatory response downstream of cytokines. JAK1/2 in

Pseudomonas aeruginosa infection with marrow suppression simulating acute promyelocytic leukemia.

Two cases of Pseudomonas aeruginosa infection, with hemograms and marrow aspirates simulating acute promyelocytic leukemia, are presented. The absence of either Auer rods or abnormal granulation, as well as the clinical history of infection, are important clues to the benign nature of the process. Experimental and clinical evidence to support the role of Pseudomonas endotoxin in myeloid suppression and maturation arrest is reviewed.

Pediatric acute myeloid leukemia patients with i(17)(q10) mimicking acute promyelocytic leukemia: Two case reports.

BACKGROUND: Chromosome i(17)(q10) abnormality is mainly associated with chronic myeloid leukemia (CML), myelodysplastic syndrome/myeloproliferative tumors (MDS/MPD), and acute myeloid leukemia (AML). The role of i(17)(q10) in AML is still unknown, the differences between AML and acute promyelocytic leukemia (APL)-like AML with i(17)(q10) need more research. This study aimed to investigate the clinical characteristics and laboratory evidence of 2 AML cases with i(17)(q10), similar to APL phenotype. CASE SUMMARY: Both pediatric patients were males; case 1 had newly diagnosed AML, and case 2 sho

Cytogenetic findings in patients with acute promyelocytic leukemia and a case of CML blast crisis with promyelocytic proliferation.

Nineteen patients with acute promyelocytic leukemia (APL) and one with promyelocytic blast crisis were studied by a methotrexate cell synchronization technique and by 24-hour short-term culture. Nineteen cases of APL included two cases of the microgranular variant (M3V). Except in one case of M3V, t(15;17) was detected in all patients. The breakpoints were determined as 15q22 and 17q12-21. A chronic myeloid leukemia (CML) blast crisis patient with high promyelocyte count also had a t(15;17) as well as a masked Ph chromosome. Other abnormalities, such as +8,del(7q) and an i(17q), were also obse

Cytochemistry of acute promyelocytic leukemia (M3): leukemic promyelocytes exhibit heterogeneous patterns in cellular differentiation.

Cytochemical investigation of leukemic promyelocytes from 25 cases of acute promyelocytic leukemia (M3) disclosed two major cellular differentiation categories: (1) the pure neutrophilic (N) type (16 cases) with strong myeloperoxidase (MPO) and naphthol-ASD chloroacetate esterase (Es-chl), but lacking the monocytic enzyme NaF-sensitive alpha-naphthyl butyrate esterase (Es-b), and (2) the mixed neutrophilic/monocytoid (N/M) type (seven cases) with strong Es-b as well as strong MPO, all cases exhibiting Es-dual (Es-b + Es-chl) positive cells. Two more cases with unusual phenotypes were noted: on

Current management and emerging pharmacotherapeutic strategies in acute promyelocytic leukemia.

INTRODUCTION: Acute promyelocytic leukemia (APL) has evolved from one of the most rapidly fatal neoplasms into the most curable acute myeloid leukemia subtype, following the sequential clinical introduction of all-trans retinoic acid (ATRA) and arsenic trioxide (ATO). Despite cure rates now approaching 100% in non-high-risk disease, early death occurring before or shortly after treatment initiation and relapse/refractory APL cases remain the principal obstacles to achieving a definitive cure. AREAS COVERED: This review summarizes current standard-of-care APL management, including risk stratif

Pancytopenia Misattributed to Parvovirus B19 Infection Revealing Acute Promyelocytic Leukemia.

Pancytopenia is a diagnostic challenge due to its broad differential diagnosis, including infections and hematological malignancies. Parvovirus B19 infection is a recognized but uncommon cause of transient pancytopenia. However, positive viral serology or viremia may lead to diagnostic anchoring and delay the identification of underlying malignant disorders. We report the case of a 17-year-old previously healthy female presenting with persistent fever and tonsillitis. Laboratory investigations revealed pancytopenia with agranulocytosis. Blood serology and polymerase chain reaction (PCR) were p

Hemostatic abnormalities in acute promyelocytic leukemia: clinical implications and mechanisms.

PURPOSE OF REVIEW: Acute promyelocytic leukemia (APL) is a subtype of acute myeloid leukemia. The long-term prognosis for APL is generally more favorable than for other types of acute leukemia due to the use of differentiation therapies. However, early hemorrhagic death remains a significant clinical challenge in APL patients. RECENT FINDINGS: Real-world studies reveal that the rate of early hemorrhagic death remains high (10-30%) in APL patients in the era of differentiation therapy. APL patients have systemic hemostatic changes including activation of coagulation followed by consumption of

Natural products as potential drug treatments for acute promyelocytic leukemia.

Acute promyelocytic leukemia (APL), which was once considered one of the deadliest types of leukemia, has become a curable malignancy since the introduction of all-trans retinoic acid (ATRA) and arsenic trioxide (ATO) as clinical treatments. ATO, which has become the first-line therapeutic agent for APL, is derived from the natural mineral product arsenic, exemplifying an important role of natural products in the treatment of APL. Many other natural products, ranging from small-molecule compounds to herbal extracts, have also demonstrated great potential for the treatment and adjuvant therapy

Rapid diagnostic approach to PML-RARalpha-positive acute promyelocytic leukemia.

INTRODUCTION: Acute promyelocytic leukemia (APL) is a distinct entity characterized by a specific bone marrow morphology and a rearrangement of the PML- and the RARalpha-gene mostly due to a balanced translocation between the long arm of chromosome 15 with a breakpoint in 15q22 and the long arm of chromosome 17 with a breakpoint in 17q12-21. The introduction of all trans retinoic acid (ATRA) into treatment protocols has improved the outcome of APL dramatically. Therefore, it is essential to establish the diagnosis of APL as quickly and as reliably as possible. MATERIALS AND METHODS: We invest

Acute Promyelocytic Leukemia, Retinoic Acid, and Arsenic: A Tale of Dualities.

Acute promyelocytic leukemia (APL) is driven by the promyelocytic leukemia (PML)/retinoic acid receptor α (RARA) fusion oncoprotein. Over the years, it has emerged as a model system to understand how this simple (and sometimes sole) genetic alteration can transform hematopoietic progenitors through the acquisition of dominant-negative properties toward both transcriptional control by nuclear receptors and PML-mediated senescence. The fortuitous identification of two drugs, arsenic trioxide (ATO) and all-trans-retinoic acid (ATRA), that respectively bind PML and RARA to initiate PML/RARA degrad
