Hematopoietic SCT: a useful treatment for late metachromatic leukodystrophy.

In metachromatic leukodystrophy (MLD), the deficiency of the lysosomal enzyme arylsulfatase A (ARSA) leads to demyelination in the central and peripheral nervous system and ultimately to death. Allogeneic hematopoietic SCT (HSCT) is currently the only treatment for adult and late-onset juvenile MLD, although it is still in question because of insufficient follow-up. We wanted to determine whether HSCT could halt the progression of adult and late-onset juvenile MLD. Four treated unrelated patients and three untreated siblings were included in the study, and followed regularly for up to 18 years

Lentiviral hematopoietic stem cell gene therapy benefits metachromatic leukodystrophy.

Metachromatic leukodystrophy (MLD) is an inherited lysosomal storage disease caused by arylsulfatase A (ARSA) deficiency. Patients with MLD exhibit progressive motor and cognitive impairment and die within a few years of symptom onset. We used a lentiviral vector to transfer a functional ARSA gene into hematopoietic stem cells (HSCs) from three presymptomatic patients who showed genetic, biochemical, and neurophysiological evidence of late infantile MLD. After reinfusion of the gene-corrected HSCs, the patients showed extensive and stable ARSA gene replacement, which led to high enzyme express

Biochemical profiling to predict disease severity in metachromatic leukodystrophy.

Metachromatic leukodystrophy is a neurodegenerative disease that is characterized by a deficiency of arylsulfatase A, resulting in the accumulation of sulfatide and other lipids in the lysosomal network of affected cells. Accumulation of sulfatide in the nervous system leads to severe impairment of neurological function with a fatal outcome. Prognosis is often poor unless treatment is carried out before the onset of clinical symptoms. Pre-symptomatic detection of affected individuals may be possible with the introduction of newborn screening programs. The ability to accurately predict clinical

Enzyme, cell and gene-based therapies for metachromatic leukodystrophy.

Metachromatic leukodystrophy (MLD) is a demyelinating storage disease caused by deficiency of the lysosomal enzyme arylsulfatase A (ARSA). Lack of ARSA activity leads to the accumulation of galactosylceramide-3-O-sulfate (sulfatide) in the central and peripheral nervous systems. Based on the age at onset, the disease is usually classified into three forms: the late-infantile form, which manifests in the second year of life; the juvenile variants (onset between 4 and 12 years), which are subdivided into early-juvenile (EJ, onset before 6 years) and late-juvenile (LJ, onset after 6 years); and t

ARSA gene mutations in five Chinese metachromatic leukodystrophy patients.

The objective was to identify arylsulfatase A mutations, if any, in five Chinese patients with metachromatic leukodystrophy. This would be the first such study in China. All eight exons and exon-intron boundaries of the arylsulfatase A gene (ARSA) were amplified with polymerase chain reaction, which was followed by direct DNA sequencing. Patient 1 exhibited a homozygous mutation at c.954G>A (p.W318X) in exon 5. Patient 2 exhibited compound heterozygous mutations, identified as one allele with the c.862C>T (p.R288C) missense mutation in exon 5 and the other allele with the c.1338dupC frameshift

Unusual gallbladder findings in two brothers with metachromatic leukodystrophy.

Characteristic biliary tree abnormalities in metachromatic leukodystrophy (MLD) include gallbladder polyposis and haemobilia. We report two brothers with MLD, who presented with uncommon biliary complications. One presented with gastric outlet obstruction secondary to gallbladder enlargement, which was treated by percutaneous aspiration. He later developed gallbladder carcinoma with liver metastases. His brother demonstrated US findings consistent with gallstones.

Neuronal depletion of cerebellum in late infantile metachromatic leukodystrophy.

The cerebellums of two children with late infantile metachromatic leukodystrophy were investigated by light and electron microscopy. These observations showed that the morphological abnormalities were comprised not only of severe demyelination and presence of metachromatic granules but also marked loss of Purkinje cells and granule cells. The depletion of Purkinje cells is considered to have resulted from demyelination of their myelinated axons. On the other hand, the reduction of granule cells is considered to be caused by the loss of Purkinje cells and presynaptic elements such as mossy fibe

Spectrum of ARSA mutations in Iranian patients with metachromatic leukodystrophy.

Metachromatic leukodystrophy (MLD) is an autosomal recessive disorder caused by mutations in the arylsulfatase A (ARSA) gene. Few studies have assessed the spectrum of ARSA mutations among Iranian patients. Here, we report eight Iranian patients with clinical features of MLD. Whole exome sequencing led to identification of the underlying mutation in ARSA gene in these patients. Among identified mutations was the recurrent c.938G > C (p.R313P) mutation in exon 5 of this gene, showing its relatively high frequency among Iranians. The results of this study helps in design of population-specific p

Encapsulated cells as an enzyme replacement therapy for metachromatic leukodystrophy.

Metachromatic leukodystrophy (MLD) is a lysosomal storage disorder caused by an inherited deficiency of arylsulfatase A (ARSA) and characterized by accumulation of sulfatides in both glial cells and neurons resulting in myelin degeneration in the central and peripheral nervous systems, neuronal degeneration and neuroinflammation. Currently, no effective treatment is available for patients once symptoms are already present. Libmeldy©, an Ex vivo gene therapy, is the gold standard for pre-symptomatic patients with late-infantile and early juvenile MLD, and early symptomatic patients with early-j

[Analysis of ARSA mutations in a Chinese family with metachromatic leukodystrophy].

OBJECTIVE: To identify arylsulftase A gene (ARSA) mutations in a Chinese family with MLD. METHODS: There were two MLD patients in the investigated family. The proband, an 11-year-old girl, was well until the age of 5 years, when she began to experience difficult walking and mental regression. Magnetic resonance imaging (MRI) of her brain showed widespread demyelination, nerve conduction velocity reduced, and ASA activity measured in white blood cells was zero. So, the child was diagnosed having MLD. The proband's young brother also got the same phenotype except clinical symptom being milder t

Automated Deep Learning-Based Demyelination Load Segmentation in Metachromatic Leukodystrophy.

PURPOSE: Metachromatic leukodystrophy (MLD) is a rare lysosomal storage disorder characterized by progressive white matter demyelination. Quantification of demyelinated white matter on MRI-typically expressed as the demyelination load-serves as a key imaging biomarker of disease burden, enabling objective monitoring beyond visual rating scales. However, current semi-automated pipelines are limited by manual interaction, pediatric brain variability, and differences in MRI acquisition. This study aimed to develop and validate a self-configuring convolutional neural network (CNN) for automated se

Allogeneic haematopoietic stem cell transplantation in late-onset metachromatic leukodystrophy.

Metachromatic leukodystrophy (MLD) is a progressive neurometabolic disease which can be treated with hematopoietic stem cell transplantation (HSCT). With emerging therapies like gene therapy, understanding the effectiveness of HSCT in late-onset forms has become increasingly important. Late-onset forms are rare, and the limited evidence on eligibility and outcomes complicates treatment decisions. The aim was to compare the long-term outcomes of the largest European cohort of HSCT-treated and untreated late-juvenile and adult MLD patients. In this observational multi-center registry-based study

[Metachromatic leukodystrophy in adult patient initially diagnosed as multiple sclerosis].

The article presents a case of metachromatic leucodystrophy (MLD) of the adult onset type, in which atypical initial course could suggest multiple sclerosis. MLD is an inherited, metabolic, degenerative disease of the nervous system. It is caused by a deficiency of a lysosomal enzyme--arylsulphatase A, with the storage of cerebroside sulphate and demyelination affecting mainly the central and peripheral nervous system. In the reported case of a female aged 21 the clinical onset of the disease was at the age 20. The first neurological symptoms included a mild spastic paraparesis, ataxia of lowe
