Banana juice reduces bioavailability of levodopa preparation.

This study aimed to examine the effects of banana juice on levodopa bioavailability in rats. When a levodopa preparation (EC-Doparl tablets) was orally administered with banana juice made by mixing with a fresh banana and water, there were significant decreases in C(max) (17.4+/-2.5 vs. 8.6+/-3.1 microg/ml; alpha=0.05) and AUC (1882.8+/-49.2 vs. 933.5+/-286.6 microg.min/ml; alpha=0.05) for levodopa. On the other hand, administration of the levodopa preparation with a commercial beverage containing 10% banana juice resulted in no significant change in C(max) or AUC. These results indicate that

Factors affecting levodopa effects in Parkinson's disease.

Levodopa is the gold standard for the treatment of Parkinson's disease (PD) because of its outstanding clinical efficacy. However, the majority of patients experience various adverse reactions, including the wearing-off phenomenon, the on-off phenomenon, dyskinesia and psychiatric symptoms. The response to levodopa depends not only on the intrinsic responsiveness of the patients, but also on various other important factors including the type of levodopa preparation, its absorption/metabolism, the blood-brain barrier, age at onset of disease and concomitant use of anti-parkinsonian drugs. This

Periodic triphasic waves in levodopa-induced encephalopathy.

Four patients with idiopathic Parkinson's disease developed subacute confusional states, associated with delusions, hallucinations, and myoclonus, following an increase in the dose of levodopa. The EEG revealed periodic generalized triphasic waves. The clinical condition and EEG cleared following levodopa dose reduction or discontinuation. This is the first report of encephalopathy associated with periodic triphasic activity following levodopa treatment.

Dream phenomena induced by chronic levodopa therapy.

Twenty-seven of eighty-eight (30.7%) Parkinsonian patients on chronic levodopa or levodopa/carbidopa therapy developed drug related dream phenomena. The patients reported three separate types of new dreams which we have classified as vivid dreams, night terrors and nightmares. These dreams are correlated to the duration of levodopa therapy although the mechanism of their production is unclear.

Duration of "Good On" time per dose: Immediate-release carbidopa-levodopa vs. extended-release carbidopa-levodopa (IPX203, CREXONT®).

BACKGROUND: For Parkinson's disease patients with motor fluctuations, the duration of benefit per levodopa dose is a key metric that reflects a patient's clinical response. OBJECTIVE: Determine the difference in mean durations of "Good On" time per dose of subjects randomized to extended-release carbidopa-levodopa (ER CD-LD; IPX203; CREXONT®) vs. immediate-release (IR) CD-LD in the RISE-PD trial. METHODS: "Good On" time per dose was assessed at the end of the IR CD-LD dose adjustment phase (Visit 2/Week 3) and compared to End of Study (Visit 7/EOS) between IPX203 and IR CD-LD groups. In addi

Comparison of blood dopamine in Parkinson's patients treated with levodopa carbidopa entacapone vs levodopa benserazide: A randomized controlled trial.

BACKGROUND: Levodopa combined with carbidopa and entacapone (LCE) and levodopa combined with benserazide (LB) are commonly used treatment options in every stage of Parkinson's disease. Although both combinations aim to enhance dopamine bioavailability and improve motor symptoms, their short-term biochemical effects remain to be clarified. This study aims to compare the short-term effects of LCE and LB on plasma dopamine levels in patients with early-stage, unilateral Parkinson's disease. METHODS: This prospective, single-center, randomized controlled study included patients diagnosed with ear

Evaluation of levodopa dose and magnitude of dopamine depletion as risk factors for levodopa-induced dyskinesia in a rat model of Parkinson's disease.

Levodopa dose and severity of Parkinson's disease (PD) are recognized risk factors for levodopa-induced dyskinesia (LID) in humans. The purpose of the present study was to evaluate the ability of these variables to predict severity of LID in a rat model of PD. Varied concentrations of 6-hydroxy-dopamine were injected into the midbrain to produce wide ranges of dopamine depletion in striatum. Three weeks later, rats were given daily injections of levodopa (2-10 mg/kg i.p.) plus benserazide (12.5 mg/kg i.p.) for 15 days. Abnormal involuntary movements (AIMs) were measured for limb, axial, orolin

Opicapone Pharmacokinetics and Effects on Catechol- O -Methyltransferase Activity and Levodopa Pharmacokinetics in Patients With Parkinson Disease Receiving Carbidopa/Levodopa.

OBJECTIVES: Levodopa (LD) administered with dopa decarboxylase inhibitor is predominantly metabolized in the periphery by catechol- O -methyltransferase (COMT) to 3- O -methyldopa (3-OMD). Catechol- O -methyltransferase inhibition can improve treatment outcomes by decreasing variability in circulating LD concentrations. Opicapone is a once-daily COMT inhibitor approved in the US adjunctive to carbidopa (CD)/LD in patients with Parkinson disease experiencing "OFF" episodes. This study aimed to evaluate the pharmacokinetics and pharmacodynamics of once-daily opicapone 50 mg adjunctive to CD/LD i

Functionalized liposomes for intranasal levodopa delivery to the brain.

OBJECTIVE: The study evaluated functionalized liposomes as potential carriers for intranasal delivery of levodopa. METHODS: Lipid film hydration method was used to obtain conventional and functionalized liposomes with polyethylene glycol or maleimide-PEG. The liposome structure was analyzed by dynamic light scattering and 1H-NMR spectroscopy. Isolated sheep nasal mucosa was used for mucoadhesion and mucous penetration studies. Levodopa release was assessed using a Franz diffusion cell. In vivo experiments were conducted using a method based on the inhibition of dopaminergic transmission. RES

Rigidity in Parkinson's Disease: The Objective Effect of Levodopa.

BACKGROUND: Quantitative evidence of levodopa-induced beneficial effects on parkinsonian rigidity in Parkinson's disease (PD) is lacking. Recent research has demonstrated the velocity-dependent nature of objective rigidity in PD and revealed its neural underpinning. OBJECTIVE: The present study aimed to examine the effect of levodopa on objective rigidity in PD. METHODS: Eighteen patients with PD underwent clinical and instrumental evaluations of muscle tone in the OFF and ON states. The clinical assessments focused on rigidity in the most affected upper limb. The biomechanical components of

Levodopa, homocysteine and Parkinson's disease: What's the problem?

Elevated circulating homocysteine levels have been associated with cognitive impairment and cardio-cerebro-vascular events. Levodopa treatment of Parkinson's disease tends to further elevate circulating homocysteine levels due to the metabolism of levodopa via catechol-O-methyltransferase (COMT). COMT co-factors are vitamins B12, B6 and folic acid. Accumulating deficiencies of these vitamins are presumed to be the substrate for the homocysteine elevation. B-vitamin therapy reduces homocysteine levels. This begs the question of whether Parkinson's disease patients on levodopa should be concurre

Mechanisms of peripheral levodopa resistance in Parkinson's disease.

Parkinson's disease (PD) is an increasingly common neurodegenerative condition. The disease has a significant negative impact on quality of life, but a personalized management approach can help reduce disability. Pharmacotherapy with levodopa remains the cornerstone of treatment, and a gratifying and sustained response to this treatment is a supportive criterion that argues in favor of an underlying diagnosis of PD. Yet, in daily practice, it is not uncommon to encounter patients who appear to have true PD, but who nevertheless seem to lose the responsiveness to levodopa (secondary non-respond

Influence of levodopa on orthostatic hypotension in Parkinson's Disease.

AIM OF THE STUDY: Orthostatic hypotension presents in all phases of Parkinson's Disease (PD) and occurs in about 80% of patients. There is some debate in the literature as to the relationship of orthostasis to the standard drug treatments. A distinct tendency towards hypotension has been found, especially for treatment with levodopa (LD). We therefore wanted to investigate the influence of LD on blood pressure response in PD patients. METHODS: We examined prospectively PD patients using lying-to-standing orthostatic tests (the modified Schellong test). The patients underwent measurements on t

[Vitamin B6 deficiency caused by the use of levodopa].

BACKGROUND: There are indications that the use of levodopa/carbidopa can cause symptomatic vitamin B6 deficiency. However, this has only been described for patients who used the product in the form of an intestinal gel. CASE DESCRIPTION: We are describing 3 patients who developed vitamin B6 deficiency while using levodopa/carbidopa in tablet form. All 3 patients developed symptoms consistent with peripheral axonal polyneuropathy. CONCLUSION: The cases of our patients are providing support for the idea that oral use of levodopa/carbidopa can cause symptomatic vitamin B6 deficiency.

Levodopa is not a useful treatment for Lesch-Nyhan disease.

Lesch-Nyhan disease (LND) is characterized by dystonia, cognitive abnormalities, and self-injurious behavior. No effective therapies are available. LND is associated with a presynaptic dopaminergic deficit, but the reported effects of dopamine replacement therapy are conflicting. The current prospective open-label study assesses the effects of levodopa on both neurological and behavioral features of LND. All 6 study participants discontinued levodopa early, due to lack of effect and sometimes worsening of motor function. The results provide important clues for pathophysiological mechanisms and

Levodopa in Parkinson's disease: from the past to the future.

IMPORTANCE OF THE FIELD: Levodopa is the mainstay of symptomatic treatment for Parkinson's disease (PD). Although other treatments have been developed in the last 30 years, most patients use levodopa in view of its superior efficacy in controlling PD symptoms. Unfortunately, levodopa is associated with long-term motor complications (motor fluctuations and dyskinesias). The main causes of these undesirable effects are the narrowing of the therapeutic window with the natural progression of the disease, pulsatile dopaminergic stimulation due to the short half-life of the drug and erratic absorpti

Levodopa responsiveness in disorders with parkinsonism: a review of the literature.

A literature review was conducted to investigate whether or not levodopa (LD) responsiveness (LR) is a useful criterion in the diagnosis of parkinsonian disorders. Although LR does appear to differ among the parkinsonian disorders, there is considerable confusion in the literature. While most patients with Parkinson's disease (PD) have a sustained benefit from LD, a small minority of patients with documented PD do not respond. The literature suggests that the LR rate is higher for multiple system atrophy (MSA), progressive supranuclear palsy (PSP), and corticobasal degeneration (CBD) than base
