The Ebola epidemic of 2014-2015 was one of the most significant public health threats of the 21st century, a crisis that challenged leadership in West Africa and around the world. Using the experience of Liberia's epidemic control efforts, we highlight the critical role that leadership played during four phases of the epidemic response: (1) crisis recognition and early mobilization; (2) the emergency phase; (3) the declining epidemic; and (4) the long tail. We examine how the decisions and actions taken in each phase of the epidemic address key crisis leadership tasks, including sense-making,

Globally, unmet need for postpartum family planning remains high, while immunization services are among the most wide-reaching and equitable interventions. Given overlapping time frames, integrating these services provides an opportunity to leverage existing health visits to offer women more comprehensive services. From March through November 2012, Liberia's government, with support from the Maternal and Child Health Integrated Program (MCHIP), piloted an integrated family planning and immunization model at 10 health facilities in Bong and Lofa counties. Vaccinators provided mothers bringing i

A rabies outbreak in Zorzor District, Liberia, in 1982 resulted in 31 known bitten and 12 known exposed patients. Human diploid cell strain (HDCS) vaccine was used to vaccinate 40 patients. Of these, 34 were vaccinated at Curran Lutheran Hospital, Zorzor, Liberia, mostly by the intradermal (i.d.) route. Five of 28 bitten patients who started their vaccinations did not complete the 4-dose course, including a 16-year-old boy who did not return after the first injection and later died of rabies. There were also 2 deaths in 3 known bitten but unvaccinated patients. None of the 23 bitten who receiv

During the 2014-16 Ebola outbreak, Liberia suffered 2,544 deaths, including 8% of its doctors, nurses and midwives. The Government responded in 2016 with a seven-year Health Workforce Program (HWP), the goal of which was "the creation of a fit-for-purpose, motivated, and highly-skilled workforce." The US Health Resources and Services Administration (HRSA) with the support from the US President's Emergency Plan for AIDS Relief (PEPFAR), established Resilient and Responsive Health Systems project to assist Liberia in strengthening health workforce and health systems. As the HWP approaches its fi

BACKGROUND: A cluster of cases of unexplained multi-organ failure was reported in children at Bardnesville Junction Hospital (BJH), Monrovia, Liberia. Prior to admission, children's caregivers reported antibiotic, antimalarial, paracetamol, and traditional treatment consumption. Since we could not exclude a toxic aetiology, and paracetamol overdose in particular, we implemented prospective syndromic surveillance to better define the clinical characteristics of these children. To investigate risk factors, we performed a case-control study.
METHODS: The investigation was conducted in BJH betwee

Substance use is prevalent among youth in postconflict African countries and is associated with a number of public health problems such as poverty, child homelessness, and school truancy. This qualitative study explores the risk factors associated with substance use among Liberian youth from the perspective of public-school students. Nine focus groups were conducted with 72 Liberian public-school students (35 female, 37 male). Multiple risk factors for substance use among Liberian youth were identified through qualitative analysis, including emotional instability, gender, fear of academic fail

BACKGROUND: During the Ebola virus disease (EVD) epidemic in Liberia, contact tracing was implemented to rapidly detect new cases and prevent further transmission. We describe the scope and characteristics of contact tracing in Liberia and assess its performance during the 2014-2015 EVD epidemic.
METHODOLOGY/PRINCIPAL FINDINGS: We performed a retrospective descriptive analysis of data collection forms for contact tracing conducted in six counties during June 2014-July 2015. EVD case counts from situation reports in the same counties were used to assess contact tracing coverage and sensitivity

Haemoglobin variants, beta-thalassaemia and glucose-6-phosphate dehydrogenase (G-6-PD) types were studied in 702 individuals from Buchanan, Liberia. In this population haemoglobins S and C, beta-thalassaemia and G-6-PD deficiency were found together. There was a considerable tribal variation. In the tribes of eastern Liberia the S- and C-genes were uncommon and the beta-thalassaemia gene was rather frequent, while in western Liberia the S- and C-genes were more frequent and beta-thalassaemia uncommon. In the central and northern parts the S- and beta-thalassaemia genes were found together in r

PURPOSE: More than 60% of unintended pregnancies globally end in induced abortions. In Liberia, limited access to contraceptives leads to many unintended pregnancies, with little known about the incidence of induced abortion and unintended pregnancy in the country. This study aimed to estimate the incidence of induced abortion and unintended pregnancy in Liberia in 2021.
METHODS: The Abortion Incidence Complications Method was used. First, the induced abortions that resulted in women seeking facility-based post-abortion care (PAC) in 2021 were estimated using data from a nationally representa

BACKGROUND: Malaria is a major cause of illness and death in Liberia. Given the high burden of disease and limited resources, Liberia implemented a subnational tailoring (SNT) approach. This approach involved stakeholder engagement, data review, and advanced analytics to update transmission risk assessment, optimize intervention targeting, and revise the national operational plan.
METHODS: An SNT team was established to determine intervention targeting criteria, compile and analyse relevant data sources, and stratify malaria risk and its determinants to inform geographical targeting of interv

Insect-borne viruses may account for a significant proportion of non-malaria and non-bacterial febrile illnesses in Liberia. Although the presence of many arthropod vectors has been documented, the collective burden of arbovirus infections and baseline pre-existing immunity remains enigmatic. Our goal was to determine the seroprevalence of arbovirus exposure across the country using a resource-sparing, multiplex immunoassay to determine IgG responses to immunodominant antigens. 532 human serum samples, from healthy adults, collected from 10 counties across Liberia, were measured for IgG reacti

BACKGROUND: Long-term persistence of Ebola virus (EBOV) in immunologically privileged sites has been implicated in recent outbreaks of Ebola virus disease (EVD) in Guinea and the Democratic Republic of Congo. This study was designed to understand how the acute course of EVD, convalescence, and host immune and genetic factors may play a role in prolonged viral persistence in semen.
METHODS: A cohort of 131 male EVD survivors in Liberia were enrolled in a case-case study. "Early clearers" were defined as those with 2 consecutive negative EBOV semen test results by real-time reverse-transcriptio

In Liberia, the physician assistant (PA) profession began in the mid-1960s. PAs have had a major role in providing access to healthcare for patients, many of whom live in poverty and reside in remote areas where access to physicians may be severely limited. In 1964, representatives from UNICEF selected Agnes N. Dagbe, MS, RN, to be the first director of a new PA program to be developed at the Tubman National Institute of Medical Arts. Dagbe was sent to Russia to learn about their feldsher profession, which served as a significant source of inspiration for the newly emerging PA profession in Li

Liberia's community health program went from concept to nationwide scale in 4 years due to the Liberian Government's vision and its partnership with implementing organizations and donors. The next community health policy will tackle the unfinished agenda related to quality, resilience, and sustainability. Liberia's experience offers valuable lessons for innovating, and institutionalizing a compensated, effective cadre of community health assistants.

BACKGROUND: In Liberia, an estimated 32% of children under 5 are stunted. Malnutrition and hunger worsened during the country's civil war and were further exacerbated by the 2014-2016 outbreak of Ebola virus disease. Studies examining adherence to recommended infant and young child feeding practices frequently do so with an emphasis on the knowledge, attitudes and beliefs of mothers and caregivers. Often overlooked are the structural factors that enable or constrain their agency to practise evidence-based recommendations.
METHODS: Between July and December 2017, we surveyed 100 Liberian mothe

During the Ebola Virus Disease (EVD) epidemic in West Africa (2014-2016), many faculty, staff, and trainees from U.S. academic medical centers (i.e., teaching hospitals and their affiliated medical schools; AMCs) wished to contribute to the response to the outbreak, but many barriers prevented their participation. Here, the authors describe a successful long-term academic collaboration in Liberia that facilitated participation in the EVD response. This Perspective outlines the role the authors played in the response (providing equipment and training, supporting the return of medical education)

BACKGROUND: In malaria-endemic countries in West Africa, sickle cell disease (SCD) contributes to childhood mortality. Historically, Liberia had regions wherein hemoglobin S and beta-thalassemia trait were mutually exclusive. Data on hemoglobinopathies in the Monrovia, the capital, are outdated and do not reflect urban migration. Updating the epidemiology of SCD is necessary to plan a public health and clinical agenda. Neither newborn screening (NBS) nor screening tools were available in country. This pilot study aimed to determine the feasibility of NBS using a South-South partnership and def
