Lightning strike is the most common environmental cause of sudden cardiac death, but it may also be associated with a myriad of injuries to various organ systems. Direct myocardial injury may be manifest as electrocardiographic alterations or elevation in cardiac-specific isoenzymes; however, significant electrical cardiac trauma appears uncommon. A case is presented of severe acute cardiomyopathy in a "Takotsubo" distribution causing cardiogenic shock following lightning strike in a previously healthy 37-year-old woman. Although rarely identified in this context, Takotsubo cardiomyopathy (als

INTRODUCTION: Lichtenberg figures are reddish, fern-like patterns that appear on the skin when a patient is struck by lightning. These appear to be a result of an inflammatory response as current spreads out causing ionisation and heat effects and damage to the small subcutaneous capillaries.
CASE REPORT: A 30-year-old lady was brought to the accident and emergency department with a history of momentary loss of consciousness. All that the patient could recollect was that she had been on the phone when she heard a loud bang following which she 'blacked out'. On examination, she had characteris

Trauma and burn injuries often present with multiple complications, necessitating a coordinated, multidisciplinary approach to management. This case series reviews the outcomes and challenges of treating high-risk trauma and burn patients, with a focus on complex polytrauma, alcohol withdrawal, high-voltage electrical injuries, and lightning strikes. Each case underscores the importance of early intervention, multidisciplinary team involvement, and individualized treatment protocols for improving patient outcomes in critically injured burn victims.

Lightning strike-related deaths are unusual, and the victim bodies can present different lesions due to the different injury mechanisms associated with this event. Since the post mortem assessment can be challenging, the evaluation of the characteristics of the skin lesions becomes fundamental to reconstruct the event. Due to the paucity of literature on this topic, the authors report the case of a 59-year-old man found dead near his home after a thunderstorm. Initially considered a murder by gunshot, the autopsy revealed the typical lightning strike lesions, also known as Lichtenberg figures.

INTRODUCTION: Lightning strikes can cause diverse neuro-otologic injuries. Tympanic membrane rupture is common, whereas bilateral profound sensorineural hearing loss with vestibular dysfunction and perilymphatic fistula is rare. Cochlear implantation for lightning-induced deafness has been reported only once. A farm worker struck by lightning sustained second-degree burns and intracranial hemorrhages requiring emergency neurosurgical management and cervical wound closure. During intensive care, clear left-sided otorrhea prompted ENT evaluation, revealing bilateral tympanic membrane perforation

BACKGROUND: Lightning-related injuries most often involve impairment of the functions of the central and peripheral nervous systems, usually including cognitive dysfunctions. We evaluated the cognitive deficit of a patient who had survived a lightning strike and measured the improvement after her cognitive training. This therapeutic method appears to be a powerful tool in the neurorehabilitation treatment.
OBJECTIVE: The aim of this case study was to prove the beneficial effects of cognitive training as part of the neurorehabilitation after a lightning strike.
METHODS: Six neuropsychological

Electrical injuries can occur in pregnant women but currently their incidence is not completely known. Notwithstanding, those represent clinical important events such maternal and fetal death, which can be avoided if properly managed. The objective of this paper is to describe the results of electrical injury (high and low voltage), in pregnant women in scientific reports. A systematic search was performed with keywords "electrical injuries", "lightning injuries", "lightning strike", "pregnant women" and "pregnancy", using the databases: MedLine, Scielo, Lilacs, Clinical key, Web of Science, S

Only limited clinical scenarios are grounds for induction of therapeutic hypothermia. Its use in traumatic cardiac arrests, including those from lightning strikes, is not well studied. Nonshockable cardiac arrest rhythms have only recently been included in resuscitation guidelines. We report a case of full neurological recovery with therapeutic hypothermia after a lightning-induced pulseless electrical activity cardiac arrest in an 18-year-old woman. We also review the important pathophysiology of lightning-induced cardiac arrest and neurologic sequelae, elaborate upon the mechanism of therape

BACKGROUND: Despite an increasing number of case reports, lightning strikes in humans remain a rare occurrence. The associated complications range from burns and transient unconsciousness to severe cardiac consequences such as arrhythmias, cardiomyopathies, and cardiogenic shock until death.
CASE SUMMARY: A 26-year-old patient was struck by lightning. After a temporary loss of consciousness, he was admitted to the hospital. Serious injuries were excluded by computed tomography (CT). A transthoracic echocardiography (TTE) showed a normal cardiac function without structural or functional altera

BACKGROUND: Direct lightning strikes are rare, and multiple organ systems can be involved. Prognosis is dependent on the severity of the injury. Severe myocardial injury associated with transient electrocardiogram changes, which have been previously described, is a hazardous complication.
CASE SUMMARY: A 35-year-old man with no known past medical history presented unresponsive following a direct lightning strike while sitting in a portable toilet. High-quality cardiopulmonary resuscitation was started in the field, with return of spontaneous circulation (ROSC) after 1 h. Following ROSC, he re

OBJECTIVES: The purpose of this study was to investigate the effects of lightning strikes on the cardiovascular system.
BACKGROUND: A lightning strike can attack its victims in one of three ways: direct hit, splash or ground strike. The cardiovascular system can be affected directly by mechanical or electrical trauma during a direct hit or can be indirectly affected through effects on the total body with extensive catecholamine release or autonomic stimulation. Reported effects include hypertension, tachycardia, nonspecific electrocardiographic (ECG) changes including prolongation of the corr

HISTORY AND CLINICAL FINDINGS: A group of six hikers were hit by lightning out of the blue sky. The biggest harm was done to a 29-year-old man (size: 190 cm) while walking along a high spruce. He experienced a seizure with consecutive sinus tachycardia and hypertensive dysregulation. One year later he still complained about reduced physical strength. The other five hikers had less severe injuries.
INVESTIGATIONS: Burns were detectable in five of six patients. Elevated creatine kinase and myoglobin were indicative for myolysis. Renal parameters were normal.

Lightning strikes are a common and leading cause of morbidity and mortality. Multiple organ systems can be involved, though the effects of the electrical current on the cardiovascular system are one of the main modes leading to cardiorespiratory arrest in these patients. Cardiac effects of lightning strikes can be transient or persistent, and include benign or life-threatening arrhythmias, inappropriate therapies from cardiac implantable electronic devices, cardiac ischaemia, myocardial contusion, pericardial disease, aortic injury, as well as cardiomyopathy with associated ventricular failure

Electrical injuries in animals occur most often accidentally. They comprise contact to various forms of currents, including alternating, rotary, or direct currents. Depending on various parameters of the current (including the type of circuit, voltage, current and duration of exposure) and conditions of the animal (such as wet or dry hair coat and pathway of current through the body), lesions may be absent or may include early or localized development of rigor mortis, signs of acute circulatory failure, or severe thermoelectrical burns. Such burns may present as external current marks, singed

Electrical injuries in pediatric patients are uncommon but can be life-threatening and require efficient and effective identification and management. Injury severity is based on the characteristics of the electricity, the duration of contact with the electrical source, and the current's pathway through the body. This issue discusses the specific threats posed by high-voltage, low-voltage, and lightning injuries. The various presentations are described, including burns, arrhythmias, respiratory arrest, cardiac arrest, blunt trauma from falls or blast events, rhabdomyolysis, tympanic membrane ru

Electrical injuries can be caused by exposure to current from low-voltage and high-voltage sources as well as lightning strikes, and the circumstances of the exposure will dictate management strategies. Human tissues have varying resistance characteristics and susceptibility to damage, so injuries may be thermal, electrical, and/or mechanical, potentially causing burns, thrombosis, tetany, falls, and blast injury. This issue reviews the types of trauma seen with electrical injury and how body systems can be affected by occult or delayed effects, and the optimal evidence-based resuscitation and

Lightning strikes are frequently encountered and are a major cause of morbidity and mortality. It could impair several organs, but the effects of electric current on the cardiovascular system contribute to the primary cause of cardiorespiratory arrest. These effects can be either transient or persistent, ranging from benign or life-threatening arrhythmias, ischemic injury, myocardial contusion, aortic injury, cardiomyopathy, and ventricular failure. Myocarditis has been an important but not very well-understood cause of cardiac dysfunction. Fulminant myocarditis is defined as patients presenti

A case is reported of a 23 year old white man who was struck by lightning. The patient suffered cardiac and respiratory arrest. Immediately, he received external cardiac massage and artificial breathing, which were successful. He was then admitted to an intensive care unit, where the diagnosis of acute myocardial infarction was made, on the basis of electrocardiographic abnormalities, a significative elevation of cardiac enzymes, and ultimately a thallium-201 scintigraphy. There were no complications of this heart injury, except for a persistent sinus tachycardia, which was treated by the infu
