The purpose of this study was to assess the frequency of irreversible lingual nerve (LN) injury in patients undergoing sagittal ramus osteotomies (SRO) with bicortical screw fixation. A retrospective cohort study of patients treated by a single surgeon was performed (follow-up 2-11 years). The sample consisted of a series of subjects with a bimaxillary dentofacial deformity (DFD). The SRO and bicortical screw fixation techniques were consistent. The primary outcome variable was the prevalence of irreversible LN injury. Two hundred sixty-two subjects undergoing 523 SROs with bicortical screw fi

Mitogen-activated protein kinases (MAPKs) play a pivotal role in the mediation of cellular responses to a variety of signaling molecules. The current study demonstrates phosphorylation of extracellular signal-regulated kinase (ERK) and p38 MAPK in each subdivision of the trigeminal sensory nuclear complex (TSNC) following lingual nerve injury. Immunohistochemical labeling for phosphorylated ERK (p-ERK) or phosphorylated p38 (p-p38) MAPK was performed in histological sections of the brainstem. A transient increase in the immunoreactivity for p-ERK was found in each subdivision of the TSNC follo

Lingual nerve damage subsequent to lower wisdom tooth removal affects a small number of patients, sometimes producing permanent sensory loss or impairment. A number of surgical techniques have been described which are associated with low incidences of this distressing post-operative complication. When a technique is adopted by an individual clinician then a personal audit may be prudent to establish how effective it is in relation to established nerve injury rates. This audit looks at a technique involving the minimal interference of lingual soft tissues during lower wisdom tooth removal in a

OBJECTIVES: To investigate the different changes in nociceptive activity between two animal models of trigeminal neuropathic pain: unilateral external carotid artery ischemic reperfusion and lingual nerve crush in rats.
DESIGN: In this study, changes in nociceptive activity were investigated in unilateral external carotid artery ischemic reperfusion and lingual nerve crush models of trigeminal neuropathic pain in rats. Field excitatory postsynaptic potentials (fEPSPs) evoked by capsaicin application on the tongue of rats were recorded in the trigeminal nucleus caudalis. In addition, immunohis

OBJECTIVES: MR neurography has the ability to detect and depict peripheral nerve injuries. This study evaluated the potential of MR neurography in the diagnosis of post-traumatic trigeminal neuropathy.
METHODS: Forty-one participants prospectively underwent MR neurography of the lingual and inferior alveolar nerves using a 3D TSE STIR black-blood sequence. Two blinded and independent observers recorded the following information for each nerve of interest: presence of injury, nerve thickness, nerve signal intensity, MR neurography Sunderland class, and signal gap. Afterwards, the apparent nerv

INTRODUCTION: Injury to the inferior alveolar and lingual nerves is a recognized complication of surgical extraction of impacted mandibular third molars and may result in temporary or permanent neurosensory disturbances. The identification of patient-, radiographic-, and procedure-related risk factors is essential for optimizing surgical planning and minimizing postoperative complications. This study aimed to evaluate the incidence of postoperative inferior alveolar and lingual nerve injuries following impacted mandibular third molar surgery and to identify the demographic, radiographic, and i

BACKGROUND: Inferior alveolar nerve (IAN) and lingual nerve (LN) injury remain among the most significant complications of mandibular third molar surgery. Earlier studies relied primarily on panoramic predictors such as interruption of the white line (IoWL), often without three dimensional validation. This multicentre retrospective analytical study aimed to identify independent predictors of neurosensory injury and to develop and internally validate a composite risk prediction model. METHODS: Records of 2,850 third molar extractions performed between January 2019 and December 2024 across three

A known neurologic complication of third molar surgery is lingual nerve injury. Certain risk factors can cause lingual nerve injury during the extraction of mandibular third molars. This article will review the existing literature regarding lingual nerve injuries and related risk factors, and it attempts to study lingual nerve vulnerability during extraction of lower third molars, particularly in cases of distoangular impaction. The case report describes exposure of the lingual nerve during the extraction of the lower third molar. Also presented are 4 specific risk factors of lingual nerve inj

We present a novel, patient-specific 3D-printed lingual nerve protector designed to minimize the risk of nerve injury during mandibular third molar extraction, a common cause of lingual nerve damage. Lingual nerve injuries, often resulting from trauma, dental procedures, or maxillofacial surgeries, lead to significant functional impairments. Using computed tomography (CT) data, the custom protector is fabricated through 3D printing with a self-retaining structure to shield the lingual mucosa and nerve. The device effectively separates the lingual nerve from the surgical field without requiring

BACKGROUND: Placement of mandibular endosseous implants can result in damage to the lingual nerve, the inferior alveolar nerve or both nerves. All dentists who place mandibular implants should be aware of the appropriate early management of these injuries, as well as the appropriate time to refer patients with these injuries to a microneurosurgeon.
OVERVIEW: The lingual nerve is less likely to undergo spontaneous regeneration than is the inferior alveolar nerve, which is protected within the inferior alveolar canal. Since the inferior alveolar canal can be seen on most panoramic radiographs a

PURPOSE: The purpose of this study was to assess the effectiveness of ultrasonography in visualizing the lingual nerve, calculating its distance from the lingual cortex at the area of the third molar, and in detecting injuries.
MATERIALS AND METHODS: Using a standardized protocol, lingual nerve dissections were performed in Yorkshire pig cadaver heads. After nerve isolation was confirmed, the nerve was left intact, fully transected, or partially transected. The dissection flap was repositioned and the pig heads were given to 3 blinded evaluators. Using a handheld ultrasound device, the evalua

Fifty-four injection injuries in 52 patients were caused by mandibular block analgesia affecting the lingual nerve (n=42) and/or the inferior alveolar nerve (n=12). All patients were examined with a standardized test of neurosensory functions. The perception of the following stimuli was assessed: feather light touch, pinprick, sharp/dull discrimination, warm, cold, point location, brush stroke direction, 2-point discrimination and pain perception. Gustation was tested for recognition of sweet, salt, sour and bitter. Mandibular block analgesia causes lingual nerve injury more frequently than in

Iatrogenic lingual nerve lesion is a well-known and unfortunate complication after mandibular third molar removal. Occasionally, the nerve injury can cause severe neuropathic pain. Here, the authors present the history of 2 patients with lingual nerve injury due to mandibular third molar removal, and with severe neuropathic pain in the craniomandibular region. Pharmacotherapy and physiotherapy did not reduce the pain, and ultimately, the lingual nerve was surgically explored. Scar tissue and a lingual nerve neuroma were observed and resected in both patients. In the first patient, the gap betwee

The lingual nerve is at risk of damage during mandibular third molar (M3M) surgery. Current trends to avoid lingual retraction and prevent injury to the nerve run counter to the surgical principles of locating and protecting important structures. To our knowledge, the last review of this subject only considered repurposed lingual retractors such as the Howarth. Other studies have reported the use of purpose-built retractors with improved outcomes for lingual nerve injuries. This systematic review aims to assess the current view regarding lingual nerve retraction in M3M surgery. A search of Pub

PURPOSE: Lingual nerve damage is one of the main complications of mandibular third molar removal. In literature, the series published are heterogeneous so we have decided to study the importance of the lingual protection only during germectomies (wisdom teeth without root formation).
METHODS: We conducted a randomized prospective study in 300 germectomies, operated by the same skilled surgeon. These 300 third molar removals have been divided in two groups, one with lingual nerve protection and one without.
RESULTS: Lingual nerve injuries following germectomies, with or without lingual protec

OBJECTIVE: Lingual nerve damage sometimes occurs after the removal of third molars. The use of a lingual retractor has been advocated to protect the lingual nerve. A systematic review of the literature was undertaken to evaluate the incidence of lingual nerve damage after third molar surgery and the effect of a lingual retractor on nerve damage.
STUDY DESIGN: An exhaustive computerized search of several databases and references cited in the various studies was performed. Predetermined inclusion and exclusion criteria were used to identify the 8 published studies acceptable for detailed analys
