Oritavancin as sequential therapy for vancomycin-resistant Enterococcus faecium prosthetic valve endocarditis: a case report.

We report the case of a 91-year-old man with prosthetic valve endocarditis (PVE) caused by vancomycin-resistant Enterococcus faecium (VRE), successfully managed with an initial two-week course of intravenous daptomycin and fosfomycin, followed by six once-weekly outpatient infusions of oritavancin. The diagnosis was established based on positive blood cultures and transoesophageal echocardiography findings. In the challenging epidemiological setting of Greek hospitals-characterized by endemic multidrug-resistant organisms and a high burden of healthcare-associated infections-minimizing hospita

Protein-inspired antibiotics active against vancomycin- and daptomycin-resistant bacteria.

The public health threat posed by a looming 'post-antibiotic' era necessitates new approaches to antibiotic discovery. Drug development has typically avoided exploitation of membrane-binding properties, in contrast to nature's control of biological pathways via modulation of membrane-associated proteins and membrane lipid composition. Here, we describe the rejuvenation of the glycopeptide antibiotic vancomycin via selective targeting of bacterial membranes. Peptide libraries based on positively charged electrostatic effector sequences are ligated to N-terminal lipophilic membrane-insertive ele

In vitro activity of dalbavancin and telavancin against staphylococci and streptococci isolated from patients in Canadian hospitals: results of the CANWARD 2007-2009 study.

Two novel lipoglycopeptides, dalbavancin and telavancin, and relevant comparative agents were tested for in vitro activity against clinical isolates of staphylococci and streptococci collected in the cross-Canada surveillance study, CANWARD, in 2007-2009. The rank order of potency (based on MIC(90) [μg/mL], i.e., the concentration of antimicrobial agent required to inhibit the growth of 90% of isolates tested) of glycopeptides against both Staphylococcus aureus and Staphylococcus epidermidis was dalbavancin (0.06 μg/mL) >telavancin (0.5 μg/mL) > vancomycin (1-2 μg/mL); concurrent susceptibilit

Pharmacokinetics and safety of a single dose of telavancin in pediatric subjects 2-17 years of age.

Antimicrobial resistance increases infection morbidity in both adults and children, necessitating the development of new therapeutic options. Telavancin, an antibiotic approved in the United States for certain bacterial infections in adults, has not been examined in pediatric patients. The objectives of this study were to evaluate the short-term safety and pharmacokinetics (PK) of a single intravenous infusion of telavancin in pediatric patients. Single-dose safety and PK of 10 mg/kg telavancin was investigated in pediatric subjects >12 months to ≤17 years of age with known or suspected bacter

Dalbavancin for the Treatment of Complicated Gram-Positive Skin and Soft Tissue Infections.

New antimicrobial agents have been developed to treat infections caused by methicillin-resistant Staphylococcus aureus and other multidrug-resistant pathogens. Dalbavancin is a novel semisynthetic lipoglycopeptide antibiotic, particularly active against methicillin-resistant Staphylococcus aureus. Due to its unique pharmacological characteristics and longer half-life, it can be administered once-weekly or every 15 days and in outpatient setting. Currently, it is indicated for complicated skin and soft tissue infections, but accumulating evidence points to its off-label efficacy in osteomyeliti

The future approach for the management of acute bacterial skin and skin structure infections.

PURPOSE OF REVIEW: To discuss the new available options for the treatment of acute bacterial skin and skin structure infections (ABSSSIs) and how to implement in the clinical practice innovative approaches for their management. RECENT FINDINGS: The availability of long-acting antibiotics, including dalbavancin and oritavancin, changed the approach to patients with ABSSSI. Direct discharge from the emergency department and early discharge from the hospital should be considered in patients with ABSSSI. Despite limited data about different bactericidal properties, the choice between dalbavancin

Fever for days: A challenging case of dalbavancin-induced fever.

Dalbavancin is a novel long acting lipoglycopeptide antibiotic with a favorable safety profile approved for treating Acute Bacterial Skin and Skin Structure Infections (ABSSSI) caused by Gram-positive organisms. Given its long half-life, a two-dose regimen can provide effective systemic therapy for up to six weeks, making it an appealing option to avoid prolonged intravenous antibiotic therapy. Herein, we report a case of a 27-year-old male who developed dalbavancin-induced fever while treating Methicillin-sensitive Staphylococcus aureus (MSSA) bacteremia. Despite being inconsistently reported

Telavancin-associated acute kidney injury
.

Telavancin is a lipoglycopeptide semi-synthetic derivative of vancomycin used for select infections caused by Gram-positive bacteria including Staphylococcus aureus. Human clinical trials suggest that telavancin is potentially nephrotoxic, however there is no histopathologic description of acute kidney injury (AKI) in humans. An animal model has recently characterized the histologic changes associated with telavancin-induced AKI as proximal tubular injury with numerous phagolysosomes. We present a case of a 47-year-old man with methicillin-resistant Staphylococcus aureus bacteremia treated wit

Oritavancin: a potential weapon in the battle against serious Gram-positive pathogens.

Oritavancin is a lipoglycopeptide antibiotic with activity against aerobic and anaerobic Gram-positive bacteria. Oritavancin separates itself from other glycopeptides through its potent in vitro activity against resistant isolates of Staphylococcus aureus, Enterococcus spp. and Streptococcus spp. Oritavancin possesses a long half-life that should allow, at maximum, once-daily dosing. Currently, oritavancin has completed two Phase III trials and one Phase II trial for the treatment of complicated skin and skin structure infections, and two Phase II trials for the treatment of Gram-positive bact

Optimizing Treatment of Infective Endocarditis in Patients Who Inject Drugs.

The treatment of infective endocarditis (IE) in patients who inject drugs (PWID) is complex and challenging. These patients face stigma in the healthcare system, as well as higher mortality, longer length of hospital stay, higher rates of recurrent infections, and lower likelihood of antibiotic completion. Standard treatment for IE involves several weeks of antibiotics. For PWID, these prolonged courses often mean intravenous antibiotics administered in the inpatient setting for at least 4 to 6 weeks, which increases both healthcare costs and the risk of patient-directed discharge against medi

Dalbavancin for the management of osteomyelitis: a major step forward?

Osteomyelitis is one of the most challenging infectious diseases to treat and is associated with a significant burden to healthcare systems. There are limited available therapeutic options to treat osteomyelitis caused by Gram-positive pathogens and these are not without limitations. Dalbavancin is a long-acting lipoglycopeptide, approved for the treatment of acute bacterial skin and skin structure infections caused by designated susceptible strains of Gram-positive microorganisms. It demonstrated promising pharmacokinetic/pharmacodynamic properties and safety data to be considered for the tre

Telavancin (VIBATIV) for the treatment of complicated skin and skin structure infections.

Methicillin-resistant Staphylococcus aureus has emerged as a major causative pathogen in complicated skin and skin structure infections (cSSSIs). Unfortunately, treatment failure with vancomycin has been increasingly reported. Over the past decade, several alternative antimicrobial agents have been studied and approved for the treatment of cSSSIs. One such agent is the lipoglycopeptide telavancin, which was approved by the US FDA 2009. Given its dual mechanism of action, telavancin is characterized by a highly bactericidal activity and low potential for resistance selection. In addition, in cl

Population pharmacokinetics of telavancin in healthy subjects and patients with infections.

A population pharmacokinetic model of telavancin, a lipoglycopeptide antibiotic, was developed and used to identify sources of interindividual variability. Data were obtained from healthy subjects (seven phase 1 studies), patients with complicated skin and skin structure infections (cSSSI; two phase 2 and two phase 3 studies), and patients with hospital-acquired pneumonia (HAP; two phase 3 studies). A two-compartment open model with zero-order input best fit the telavancin data from healthy individuals and patients with cSSSI or HAP. Telavancin clearance was highly correlated with renal functi
