Experts' views on long-term care in New Zealand.

AIMS: This study reports findings from the first national survey of long-term care opinion leaders in New Zealand. METHODS: Potential respondents were chosen purposively based on their knowledge and experience in long-term care policy and planning. Questions focused on assessing views regarding long-term care quality, financing, services, workforce, organisation, and regulation. The survey was administered via Web-based format during May-June 2007. Seventy-three individuals responded for an overall response rate of 47.7%. RESULTS: Experts support cost-sharing, insurance, and individual savin

Management of hypertension in elderly long-term care residents.

OBJECTIVE: To determine the adequacy of hypertension management in institutionalized elderly patients. METHODS: Retrospective chart review of all patients with a physician-documented diagnosis of hypertension at 15 long-term care facilities in Edmonton, Alberta. RESULTS: Of 2063 long-term care residents, 733 (36%) were diagnosed with hypertension (mean age 84 years), and 566 (77%) of this cohort were receiving antihypertensive medication. The most frequently prescribed antihypertensive drugs were angiotensin-converting enzyme inhibitors (341 patients [60%]). Of the long-term residents prescr

Difficult behaviors in long-term care patients with dementia.

Difficult behaviors are relatively common challenges that can occur throughout the natural progression of dementia, but are particularly common in the mid to late stages of disease. These behaviors can be challenging to manage in nursing and assisted care facilities, and can cause distress to the caregivers and to the patients themselves. Our ability to manage these symptoms can have a profound effect on the patient's quality of life. This article reviews the appropriate assessment of behavioral and psychological symptoms of dementia (BPSD) and the literature supporting various nonpharmacologi

Depressive symptoms in long-term care residents in Taiwan.

AIM: This paper reports a study to investigate the prevalence of and risk factors for depressive symptoms in elders in long-term care facilities in Taiwan. BACKGROUND: Depression has been identified as a major health concern and is very common among frail elders in Western nursing homes. It is under-diagnosed, and may be associated with eating difficulties and subsequent malnutrition, functional ability and sociodemographic factors. There have been no previous studies of these issues in Taiwan. METHODS: Residents of 18 long-term care facilities were recruited. Those able to communicate in th

Dental hygienists as providers in long-term care facilities.

The authors surveyed all practicing dental hygienists (n = 870, response rate = 69%) in Kansas about providing preventive dental care to elderly residents of long-term care (LTC) facilities. Maximum likelihood estimates stepwise logistic regressions were used to predict the likelihood of dental hygienists filling the need for a specialty workforce. Few respondents wanted to work exclusively with elderly patients. Lack of interest in LTC practice was related to the number of children living at home or discomfort with patients who had limited abilities to communicate. Interest in part-time geria

Osteoporosis in long-term care residents with multiple sclerosis.

OBJECTIVE: To assess the fracture risk of long-term care residents with multiple sclerosis (MS) using ultrasound heel-scan technology and identification of risk factors and areas where intervention by a pharmacist might affect patient outcomes. DESIGN: Bilateral-heel scans were performed on all patients who consented to take part in the study. A retrospective review of each subject's medical records was performed to identify known risk factors for osteoporosis. SETTING: A long-term care facility in Omaha, Nebraska. PARTICIPANTS: All patients with a primary diagnosis of MS residing at the fa

Measuring antibiotic use in a long-term care facility.

BACKGROUND: In the long-term care facility setting, there is little information about correlation of antibiotic use with care delivered or with the occurrence of fever or use of Foley catheters. The objectives of this study were to compare various measures of quantitating antibiotic use and to correlate these measures with febrile morbidity and Foley catheter use in a hospital-based, long-term care facility. METHODS: This was a prospective study in which the number of residents with fever (rectal temperature of 100.5 degrees F or greater) or a Foley catheter was documented daily. Antibiotic u

Streptococcus pyogenes outbreak in a long-term care facility.

BACKGROUND: Although outbreaks involving Streptococcus pyogenes have been uncommon among the elderly population, recent reports suggest that this organism is an important nosocomial pathogen among institutionalized older patients and carries significant morbidity and mortality. An outbreak of S. pyogenes, type M12, T12, occurred in a large long-term care institution serving the ill and chronically disabled. The outbreak involved 14 residents of the intermediate care facility and lasted for 4 months. METHODS: A prospective epidemiologic investigation was initiated at the onset of the outbreak.

Geropsychiatry in long-term care: a nurse-centered approach.

High prevalence rates of psychiatric disorders and disruptive behaviors in nursing home residents create the need for structured programs of psychiatric consultation and teaching in the long-term care setting. Over 40 months, 473 residents were evaluated by a psychiatric consultation-liaison clinical nurse specialist; 100 of these cases were reviewed in detail. Apparent depression was the most common reason for referral, and depressive and adjustment disorders were the most common diagnostic conclusions of the consultation team. Behavioral complications of dementia were also frequently diagnos

Longlife insurance: a prototype for funding long-term care.

"Longlife insurance" combines nursing home, home health, and deferred annuity benefits. It costs less than life care, allows the elderly to remain in their own homes, and protects assets. Adverse selection is limited because the plan is attractive to both frail and healthy elders. An analysis of 18,600 respondents in the Social Security Administration's New Beneficiary Survey indicates that 67 percent of all retirees could afford a typical longlife insurance plan. However, less than one-half of all females living alone, 24 percent of minorities, and 8 percent of the disabled could pay privatel

Pulmonary complications of long-term respirator care in infants.

Twelve infantile autopsy cases up to 205 days using a long-term respirator care were examined in this study. These cases could be divided into three subgroups. The first group included the cases treated with long-term respirator care for hyaline membrane disease. In this subgroup, bronchopulmonary dysplasia was found during the course. The second group included the cases treated with long-term respirator care for other causes and were very premature infants. In this group, some cases showed bronchopulmonary dysplasia-like lesions. The third group was similar to the second group except for not

Innovating dementia care; implementing characteristics of green care farms in other long-term care settings.

UNLABELLED: ABSTRACTBackground:People with dementia at green care farms (GCFs) are physically more active, have more social interactions, are involved in a larger variety of activities, and come outdoors more often than those in other long-term dementia care settings. These aspects may positively affect health and well-being. This study explored which and how characteristics of GCFs could be implemented in other long-term dementia care settings, taking into account possible facilitators and barriers. METHODS: Semi-structured interviews were conducted with 23 professionals from GCFs, independe

Evaluation of preventive care for swallowing difficulty through policy changes in Japanese long-term care insurance: analysis of a nationwide claims dataset for long-term care insurance.

BACKGROUND: Swallowing/feeding difficulty is a serious hidden health problem in the older population. Although oral intake services based on the degree of this difficulty have been provided and revised in Japanese public long-term care (LTC) insurance since 2006, their implementation has not been examined. We evaluated the impact of policy changes on these services. METHODS: The nationwide database of LTC service uses in Japan was analysed for three oral intake services: Enhanced Oral Function in daycare facilities whose users were slightly disabled, Maintenance of Oral Ingestion and Transiti

A pulmonary tuberculosis outbreak in a long-term care facility.

In long-term care facilities (LTCFs), the elderly are apt to be infected because those with latent tuberculosis infections (LTBIs) are at an increased risk for reactivation and post-primary TB disease. We report an outbreak of TB in staff and residents in a LTCF. An outbreak investigation was conducted after two TB cases were reported from the LTCF. A tuberculin skin test (TST), bacteriological examination and chest radiograph were administered to all facility staff and residents. An outbreak is defined as at least two epidemiologically linked cases that have identical Mycobacterium tuberculos

The role of disability in explaining long-term care utilization.

OBJECTIVE: In view of aging populations, it is important to improve our understanding of the determination of long-term care (LTC) service use among the middle-aged and elderly population. We examined the likelihood of using 2 levels of LTC-homecare and institutional care-in the Netherlands and focused on the influence of the measured degree of disability. METHODS: We pooled 2 cross-sectional surveys-one that excluded institutionalized and one that was targeted at institutionalized individuals aged 50+. Disability is measured by impairment in (instrumental) activities of daily living (iADL, A

Modular protein supplements and their application to long-term care.

BACKGROUND/AIM: Glioblastomas (GBM) are infiltrative malignant brain tumors which mostly recur within a year's time following surgical resection and chemo-radiation therapy. Studies on glioblastoma cells following radio-chemotherapy, have been demonstrated to induce trans-differentiation, cellular plasticity, activation of DNA damage response and stemness. As glioblastomas are heterogenous tumors that develop treatment resistance and plasticity, we investigated if there exist genome-wide DNA methylation changes in recurrent tumors. MATERIALS AND METHODS: Utilizing genome-wide DNA methylation
