Risk factors for surgical complications in the management of ossification of the posterior longitudinal ligament.

BACKGROUND CONTEXT: Ossification of the posterior longitudinal ligament (OPLL) is a progressive, debilitating disease most commonly affecting the cervical spine. When compared to other degenerative pathologies, OPLL procedures carry a significantly higher risk of complications owing to increased case complexity and technical difficulties. Most previous studies have focused on functional outcomes and few have reported on risk factors for postoperative complications in OPLL patients. PURPOSE: To identify clinical and radiological risk factors of surgical complications following treatment for ce

Potential role of the IL17RC gene in the thoracic ossification of the posterior longitudinal ligament.

The thoracic ossification of the posterior longitudinal ligament (T‑OPLL) can cause thoracic spinal stenosis, which results in intractable myelopathy and radiculopathy. Our previous whole‑genome sequencing study first reported rs199772854 in the interleukin 17 receptor C (IL17RC) gene as a potentially pathogenic loci for T‑OPLL. The aim of the present study was to examine the effects of the IL17RC gene rs199772854A site mutation on osteogenesis by establishing a model of osteogenic differentiation. IL17RC gene mutation site and wild‑type site mouse embryonic osteoblast (3T3‑E1) models were con

Ossification of the Posterior Longitudinal Ligament in Cervical Spine: Prevalence, Management, and Prognosis.

Ossification of the posterior longitudinal ligament (OPLL) in the cervical spine and related neurological complications are not uncommon in East Asian countries. The estimated prevalence of cervical OPLL-related hospitalization is 7.7 per 100,000 person-years in Taiwan, and higher incidence rates have been observed in elderly and male patients. Although cervical OPLL is frequently insidious, it can eventually cause myelopathy and predispose patients to spinal cord injury (SCI). There are multiple options for managing cervical OPLL, ranging from observation to many kinds of surgical procedures,

Role of Runx2 polymorphisms in risk and prognosis of ossification of posterior longitudinal ligament.

BACKGROUND: Our study was aimed at finding out if Runx2 SNPs (single-nucleotide polymorphisms) are related to susceptibility to and prognosis of ossification of posterior longitudinal ligament (OPLL). METHODS: We selected 80 OPLL patients and another 80 independent patients without OPLL from September 2013 to November 2014. Serum was collected to detect the genotypes of rs1321075, rs12333172, and rs1406846 on Runx2 with direct sequencing analysis. RESULTS: Differences in clinical characteristics, including age, weight, height, sex ratio, as well as smoking and drinking history, between OPLL

Posterior longitudinal ligament resection or preservation in anterior cervical decompression surgery.

We reviewed the literature to determine differences in clinical outcomes for the removal or preservation of the posterior longitudinal ligament (PLL) in anterior cervical discectomy and fusion (ACDF). The outcomes are surgeon and case-dependent for both practices. A literature review was performed in PubMed from the years 1960 to 2014 to identify studies describing surgeries where the PLL was removed or preserved during ACDF. Searches were performed using Medical Subject Headings (MeSH) and references included in the reviewed articles were also considered. Additionally we searched recent artic

Sensory innervation of the dorsal longitudinal ligament and the meninges in the lumbar spine of the dog.

Although intervertebral disc herniation is a well-known disease in dogs, pain management for this condition has remained a challenge. The goal of the present study is to address the lack of information regarding the innervation of anatomical structures within the canine vertebral canal. Immunolabeling was performed with antibodies against protein gene product 9.5, Tuj-1 (neuron-specific class III β-tubulin), calcitonin gene-related peptide, and neuropeptide Y in combination with the lectin from Lycopersicon esculentum as a marker for blood vessels. Staining was indicative of both sensory and s

Spontaneous stabilisation of postlaminectomy cervical kyphosis by anterior longitudinal ligament ossification.

Cervical laminectomy has been known to cause postlaminectomy kyphosis and instability, often necessitating extensive anterior and posterior procedures with instrumentation for the stabilisation and correction of the deformity. We report a case of spontaneous stabilisation of postlaminectomy cervical kyphosis by anterior longitudinal ossification in the absence of an ossified posterior longitudinal ligament, thus circumventing the need for surgical intervention.

Reconstruction of the medial patellofemoral ligament using a longitudinal patellar tunnel technique.

Patellar instability is a common clinical problem affecting a young, active population. A large number of procedures have been described to treat patellar instability. We present the clinical results in a case series of 25 medial patellofemoral ligament reconstructions in 21 patients with up to 30 months follow-up (mean: 7.3). Reconstruction was performed using either the gracilis or semitendinosus tendon autograft. The Tegner activity score improved overall from 3 to 4.4 at follow-up and the mean follow-up Kujala score was 87 (range: 55-100). No patella redislocations were observed. Five pati

Removal of posterior longitudinal ligament in anterior decompression for cervical spondylotic myelopathy.

STUDY DESIGN: The clinical and radiologic results of the patients with removed posterior longitudinal ligament (PLL) were compared with those of the patients with preserved PLL in the treatment of cervical spondylotic myelopathy (CSM). OBJECTIVE: To investigate effect of resection of the PLL in anterior decompression for CSM. SUMMARY OF BACKGROUND DATA: Anterior decompression has been proved to be effective in the treatment of CSM, and the pathogenic matters including herniated disc, proliferative osteophyte, and ossification of posterior longitudinal ligament should be definitely removed. Posterior approaches in the management of cervical spondylosis and ossification of the posterior longitudinal ligament.

BACKGROUND: If the cervical lordotic curvature has been well preserved, spondylostenosis or ossification of the posterior longitudinal ligament, with or without instability, may be approached posteriorly in selected older patients (over 65 years of age). Posterior surgical alternatives include the laminectomy with or without fusion, or laminoplasty. However, in younger patients or in geriatric patients with predominantly anterior disease with kyphosis, direct anterior surgical procedures yield better results. METHODS: Laminectomy with medial facetectomy and foraminotomy is classically perform

Corpectomy for multi-level cervical spondylosis and ossification of the posterior longitudinal ligament.

The choice of a surgical approach for multi-level cervical spondylotic myelopathy (CSM) and ossification of the posterior longitudinal ligament (OPLL) is still a controversial issue. While most of the surgeons are still performing decompression by laminectomy, some are doing multi-level anterior decompression. Few neurosurgeons are performing decompression by corpectomy. We have treated 26 patients by median cervical corpectomy during the last 4 years. These patients were followed up for a mean period of 25 months. Twenty one (80%) patients had a good outcome, 2 patients remained unchanged and

A reappraisal of the diagnosis in cervical disc disease: the posterior longitudinal ligament perforated or not.

Herniated nuclear material of the cervical disc often perforates the posterior longitudinal ligament. Of 22 patients who were operated on by an anterior approach, 10 were of this type (the subligamentous type of cervical disc protrusion), while in the remaining 12 patients and 15 discs the protruded disc caused no tear in the ligament (the epiligamentous type). Myelography and CT myelography of these patients were reviewed. On CT myelography a localized and sharply demarcated excavation of the metrizamide ring was commonly found in the subligamentous type. Myelographic lateral view in this gro

Involvement of reduced ENPP1 function in the pathogenesis of ossification of the posterior longitudinal ligament.

Ossification of the posterior longitudinal ligament (OPLL) is an ectopic ossification disorder with incompletely understood pathophysiology. In a previously reported 50-patient OPLL cohort, ectonucleotide pyrophosphatase/phosphodiesterase 1 (ENPP1) exonic heterozygous variants were identified in 7 patients (14%), and plasma inorganic pyrophosphate (PPi), a biochemical marker reflecting ENPP1 function, was lower in patients with OPLL than in cervical osteoarthritis controls regardless of ENPP1 variant status. However, the clinical significance of ENPP1 variants and reduced PPi in OPLL, as well

Analysis of intervertebral disc degeneration in patients with ossification of the posterior longitudinal ligament.

BACKGROUND: As degenerative disease with nerve compression, little is known about the relationship between cervical ossification of the posterior longitudinal ligament (OPLL) and intervertebral disc degeneration (IVDD). This study investigates the effect of ossification of ligaments on IVDD in patients with cervical OPLL. We focus on the ossification-related segment in patients diagnosed with OPLL, which is characterized by ossification that crosses intervertebral space or occurs concurrently in the upper and lower vertebrae. METHODS: A total of 92 ossification-related segments from 40 patien
