Identifying people with chronic hepatitis B virus who are lost to clinical follow up: A retrospective case finding and re-engagement service improvement exercise.

BACKGROUND: Hepatitis B virus (HBV) infection is an important cause of liver disease-related mortality and morbidity. The World Health Organisation aims to eliminate this as a public health concern by 2030 and as such the key international guidelines recommend that all patients are reviewed regularly to observe for preventable signs of disease progression. This requires life-long engagement with specialist services, but some patients fall out of the care pathway and become lost to follow-up. OBJECTIVES: This study sought to identify and re-engage patients with HBV who were lost to follow up (…

Updating vital status by tracking in the community among patients with epidemic Kaposi sarcoma who are lost to follow-up in sub-Saharan Africa.

BACKGROUND: Throughout most of sub-Saharan Africa (and, indeed, most resource-limited areas), lack of death registries prohibits linkage of cancer diagnoses and precludes the most expeditious approach to determining cancer survival. Instead, estimation of cancer survival often uses clinical records, which have some mortality data but are replete with patients who are lost to follow-up (LTFU), some of which may be caused by undocumented death. The end result is that accurate estimation of cancer survival is rarely performed. A prominent example of a common cancer in Africa for which survival da…

Patient and Provider Reported Reasons for Lost to Follow Up in MDRTB Treatment: A Qualitative Study from a Drug Resistant TB Centre in India.

INTRODUCTION: Multidrug-resistant Tuberculosis (MDR TB) is emerging public health concern globally. Lost to follow-up (LTFU) is one of the key challenge in MDRTB treatment. In 2013, 18% of MDR TB patients were reported LTFU in India. A qualitative study was conducted to obtain better understanding of both patient and provider related factors for LTFU among MDR TB treatment. METHODS: Qualitative semi-structured personal interviews were conducted with 20 MDRTB patients reported as LTFU and 10 treatment providers in seven districts linked to Nagpur Drug resistant TB Centre (DRTBC) during August …

Lost to follow-up of patients who received intravitreal anti-vascular endothelial growth factor therapy to treat four different retina disorders in an individual center in Brazil.

OBJECTIVES: To identify risk factors for loss to follow-up in periodic intravitreal anti-vascular endothelial growth factor injections for t … istance, and payment mode as risk factors. RESULTS: Two hundred and seventy patients (29.93%) were lost to follow-up. Multivariate analysis showed age, monthly income, eye involvement, and type of medical assistance independently associated with loss to follow-up. The odds of loss to follow-up were greater among older patients than those less than 50 years (refere …

Patients from distressed communities are more likely to be symptomatic at endovascular aneurysm repair and have an increased risk of being lost to long-term follow-up.

BACKGROUND: Endovascular aortic aneurysm repair (EVAR) has become the preferred modality to repair abdominal aortic aneurysms (AAAs). Howeve … I, 2.23-5.44; P < .001) and 1 year (aOR, 1.74; 95% CI, 1.43-2.13; P < .001). A higher risk of being lost to follow-up (28.9% vs 26.3%; P < .001) was also observed in group I. CONCLUSIONS: Patients from distressed communities who require EVAR tended to have multiple comorbidities. These patients were also more likely to be treated for symptomatic AAAs, with a highe …

Lost in transition? Loss of follow-up and quality of life in adults after resection of choledochal malformation in childhood.

OBJECTIVES: Choledochal malformation (CM) is a rare disease that can lead to malignancy and potential long-term sequelae despite surgical re … healthy population. CONCLUSIONS: We confirmed that the majority of patients after CM resection are lost to follow-up. Those who answered our questionnaire showed a good HRQOL. Given the high rate of severe long-term complications and the life-long risk of malignancy, we recommend a transition program for all patients.

Expected Years Of Life Lost Due To Alcohol Consumption In Thai Adults: A 16-Year Follow-Up Cohort Of National Health Examination Survey 2004-2019.

AIMS: Evidence of premature death attributable to alcohol, a modifiable risk factor, is crucial for guiding public health policy for alcohol control. The aim of this study was to estimate alcohol-related mortality and potential years of life lost (PYLL) in Thailand in 2004-2019. METHODS: We analysed data of the third National Health Examination Survey in 2004 linked to National Death Registry data of 2004-2019. Causes of death were based on International Classification of Diseases version 10. PYLL was calculated by cause of death, age group and sex. All analyses were weighted to take into acc

Outcomes After Being Lost to Follow-up Differ for Pregnant and Postpartum Women When Compared With the General HIV Treatment Population in Rural South Africa.

BACKGROUND: Undetermined attrition prohibits full understanding of the coverage and effectiveness of HIV programs. Outcomes following loss to follow-up (LTFU) among antiretroviral therapy (ART) patients may differ according to their reasons for ART initiation. SETTING: We compare the true outcomes of adult patients previously identified as LTFU by reason for ART initiation in 8 health facilities in north eastern South Africa. METHODS: Adult HIV patient records were linked to health and demographic surveillance system (HDSS) data from 2014 to 2017. Outcomes of adults categorized as LTFU (>90

Return to care of children and adolescents living with HIV who missed their clinic visits or were lost to follow-up: a continuous quality improvement study in Uganda.

BACKGROUND: While the UNAIDS 95-95-95 targets have been met among adults, those for children and adolescents remain suboptimal. This study aimed to improve the return-to-care rates among children and adolescents living with HIV (CALHIV) who missed clinic appointments at a county-level rural health facility in eastern Uganda. METHODS: Between January 2023 and January 2024, we conducted a continuous quality improvement (CQI) study. A CQI committee was established through entry meetings and training, and quality of care gaps were identified through data reviews. We prioritised one gap for CQI th

Roles of mobile teams in tracing lost to follow-up clients: evidence from the optimization of COVID-19 vaccination uptake and routine immunization in Ekiti State.

BACKGROUND: Evidence from literature has established that tracing lost to follow-up clients is an effective strategy for complementing other mechanisms for infectious disease control like human immunodeficiency virus (HIV), tuberculosis, and other diseases such as Ebola. As a long-standing successful public health method of optimizing acceptance and/or adherence to infectious disease treatment tracing lost to follow-up clients is usually carried out by manually investigating individuals who absconded or are absent from treatments designed to manage and/or promote their health status. This stud…

Lost clips after targeted lymph node biopsy in breast cancer patients: Follow-up of the CLIP-study.

INTRODUCTION: Clipping and selective removal of axillary lymph nodes in breast cancer patients presenting with initially node-positive disease and achieving a nodal downstaging after primary systemic therapy is a less invasive method for axillary staging. An imaging guided localization and successful extirpation of these clipped lymph nodes is not possible in all patients. To date no follow-up data regarding patients with lost clips are available. MATERIAL AND METHODS: The oncological outcome of all participants of the CLIP-study and the results of postoperative axillary imaging in those pati

Lost and Not Found: Determinants and Patterns of Loss to Follow-Up Among Men Who Have Sex With Men in the Dutch HIV Pre-Exposure Prophylaxis Pilot Program.

INTRODUCTION: One of the challenges of a national HIV pre-exposure prophylaxis (PrEP) program is continuation of PrEP use and retention in care. The aim of this study was to explore determinants of loss to follow-up (LTFU) among men who have sex with men (MSM) in the Dutch national PrEP pilot program. METHODS: Data from the national STI surveillance database between July 1, 2019, and December 31, 2022, were used of all 3-monthly PrEP pilot consultations among MSM who had at least 2 consultations. LTFU was defined as having no consultation for ≥7 months, and was further divided into "missed co

Development and internal validation of a clinical risk score for lost to follow-up among drug-resistant tuberculosis patients in a high-burden setting, Ethiopia: a multicenter retrospective study.

OBJECTIVES: Lost to follow-up (LTFU) in drug-resistant tuberculosis (DR-TB) treatment is a major global public health problem, leading to continued transmission, further amplification of drug resistance, and increased morbidity and mortality. Predicting the risk of LTFU is crucial for early intervention, but validated prediction models for DR-TB patients in Ethiopia are lacking. The aim was developing and internal validation of a risk prediction model for LTFU among DR-TB patients in selected treatment-initiating centers in the Amhara region, Ethiopia. METHODS: A multicenter retrospective fol…
