BACKGROUND: Routine lab testing of stable inpatients represents low-value care and contributes toward healthcare's significant carbon footprint. If the US healthcare system was its own nation, it would be the 13th largest emitter of greenhouse gases. Integrating the concepts of high-value care and environmental sustainability represents a potentially new approach to reduce unnecessary lab testing, low-value care, and carbon emissions.
OBJECTIVE: To assess if an intervention integrating concepts of environmental sustainability and high-value care can influence resident lab ordering practices.

BACKGROUND: Inappropriate and wasteful use of health care resources is a common problem, constituting 10-34% of health services spending in the western world. Even though diagnostic imaging is vital for identifying correct diagnoses and administrating the right treatment, low-value imaging-in which the diagnostic test confers little to no clinical benefit-is common and contributes to inappropriate and wasteful use of health care resources. There is a lack of knowledge on the types and extent of low-value imaging. Accordingly, the objective of this study was to identify, characterize, and quant

BACKGROUND: Low back pain (LBP) is a leading cause of disability and healthcare utilization worldwide, yet management in primary care often remains inconsistent with clinical guidelines, with underuse of high-value care and persistent use of low-value interventions. The primary objective of this study is to evaluate the effectiveness of PRIME (PRImary care MultilEvel intervention for low back pain) compared with usual care in terms of cost-effectiveness (system level), back-related imaging rates (practitioner level), and self-reported disability (patient level), while concurrently examining im

BACKGROUND: Quality improvement activities targeting low-value care are important to ensure that scarce healthcare resources are used responsibly. However, there has been little systematic research into what diagnostic testing is considered by emergency department (ED) clinicians to be at risk of unwarranted variation or potentially low value.
OBJECTIVES: This study aimed to determine the views of ED clinicians on which diagnostic tests are highest risk for variation and/or low-value care.
METHODS: A voluntary electronic survey was distributed to emergency clinicians across Australia and Aot

INTRODUCTION: The use of opioids for acute low-back pain (LBP) is considered low-value in most clinical scenarios. Understanding trends in low-value opioid utilization can inform interventions to support guideline-concordant care. This study evaluated the utilization of low-value opioids for acute LBP, and variation by rurality, payer, and sex, before and during the COVID-19 pandemic.
METHODS: In this retrospective cohort study, we examined opioid claims from Virginia's All-Payer Claims Database among adults with commercial, Medicare Advantage (MA), or Medicaid insurance during 2019-2021. We

BACKGROUND: Despite increasing healthcare costs, training on cost-consciousness is lacking in graduate medical education (GME). Medical centers must consider how best to incorporate value-based training into their GME curricula.
OBJECTIVE: To incorporate low-value principles into an existing GME simulation exercise and assess incoming interns' recognition of low-value care.
METHODS: Choosing Wisely™ lists were reviewed to identify 4 low-value hazards to be embedded into a simulated hospital room in addition to the 8 patient safety hazards used previously. Interns were given 10 minutes to ind

OBJECTIVES: Low-value esophagogastroduodenoscopies (EGDs) for uncomplicated gastro-oesophageal reflux disease (GERD) can harm patients and raise patient and payer costs. We developed an electronic health record (EHR) 'eMeasure' to detect low-value EGDs.
DESIGN: Retrospective cohort of 518 adult patients diagnosed with GERD who underwent initial EGD between 1 January 2019 and 31 December 2019.
SETTING: Outpatient primary care and gastroenterology clinics at a large, urban, academic health centre.
PARTICIPANTS: Adult primary care patients at the University of California Los Angeles who underw

INTRODUCTION: Robust randomised trial data have shown that routine preoperative (pre-op) testing for cataract surgery patients is inappropriate. While guidelines have discouraged testing since 2002, cataract pre-op testing rates have remained unchanged since the 1990s. Given the challenges of reducing low-value care despite strong consensus around the evidence, innovative approaches are needed to promote high-value care. This trial evaluates the impact of an interdisciplinary electronic health record (EHR) intervention that is informed by behavioural economic theory.

Since the auditory brainstem response (ABR) test is a test for the brainstem capacity and hearing function, it is recorded at the discharge from the neonatal intensive care unit (NICU) in our hospital. Although the reference range for the normal full-term newborn infants is well documented, that for the pre-term newborn and/or low birth weight infants is yet to be determined. The latency between wave-I peak and wave-V peak (I-V inter peak latency) on the ABR test was measured on the 254 infants (124 males and 130 females) of 36 to 45 corrected weeks old which was defined as the sum of the gest

PURPOSE: Low-value imaging is associated with wasteful health care spending and patient harm. The routine use of magnetic resonance imaging (MRI) for the work-up of lateral epicondylitis is an example of low-value imaging. As such, our aim was to investigate the use of MRIs ordered for lateral epicondylitis, the characteristics of those undergoing an MRI, and the downstream associations of MRI with other care.
METHODS: We identified patients aged ≥18 years with a diagnosis of lateral epicondylitis between 2010 and 2019 using a Humana claims database. We identified patients with a Current Proc

OBJECTIVE: Much of the pain care in the United States is costly and associated with limited benefits and significant harms, representing a crisis of value. We explore the current factors that lead to low-value pain care within the United States and provide an alternate model for pain care, as well as an implementation example for this model that is expected to produce high-value pain care.
METHODS: From the perspective of aiming for high-value care (defined as care that maximizes clinical benefit while minimizing harm and cost), we describe the current evidence practice gap (EPG) for pain car

The transition toward value-based care in the United States has introduced episode-based payment models that increasingly tie physician reimbursement to longitudinal costs and standardized outcomes. The Centers for Medicare & Medicaid Services (CMS) Ambulatory Specialty Model (ASM), targeting chronic low back pain (cLBP), represents a pivotal extension of this framework into interventional pain management. While intended to reduce low-value utilization, such models risk redefining clinical success in ways that may not align with the heterogeneous and biopsychosocial nature of chronic pain. Thi

Efforts to improve patients' hospital experience can reduce uncertainty and indignity, but they can also create "hospitable harm" when pressure for smoother encounters encourages low-value care, preferential access, or compromises in safety, stewardship, or equity. This Perspective introduces the Experience-Integrity Matrix as a conceptual model for examining such initiatives. One axis asks whether an initiative makes care more reliable and dignified for patients; the other asks whether it aligns with clinical integrity. The four quadrants indicate what leaders should scale, repair, decline, o

BACKGROUND: Gastrointestinal (GI) perforation is a risk factor for mortality in very low birth weight (VLBW) infants. Little data exist regarding pretreatment factors and patient characteristics known to independently correlate with risk of death.
MATERIALS AND METHODS: A retrospective review of all VLBW infants who sustained GI perforation between 2011 and 2018 was conducted. Birth, laboratory, and disease-related factors of infants who died were compared to those who survived.
RESULTS: 42 VLBW infants who sustained GI perforations were identified. Eleven (26.19%) died. There were no signif

IMPORTANCE: Plain radiography offers limited diagnostic value in maxillofacial trauma and is considered low-value care. Yet, when imaging is indicated, plain radiography remains commonly used despite guidelines favoring computed tomography (CT).
OBJECTIVE: To examine national trends in, factors associated with, and consequences of low-value plain radiography use in the initial imaging of adults with maxillofacial trauma.

BACKGROUND: Organization for Economic Cooperation and Development (OECD) estimates suggest that 20% of health care spending is wasteful or even harmful. Previous interventions have had limited success in discouraging low-value care in medical practice.
METHODS: We conducted a nationwide randomized controlled trial among primary care physicians (PCPs) in Switzerland (November 2020-December 2021). We randomly assigned PCPs to one of three intervention groups related to low-value care (vitamin D testing, generic prescribing, or a cost intervention) or a control group. Negative consequences of immobility during hospitalization are widely known and remain undisputed. Evidence of low mobility for general medicine adult inpatients persists. Patients who experience hospital acquired functional decline due to low mobility require costly post-acute care services. The impact of immobility on post-acute care physical function and quality of life is directly at odds with value-based care. New Medicare payment models emphasize value-based care to promote care improvement and better patient outcomes. Quality improvement projects show promise in changing clinical practi

The purpose of this review is to examine the literature regarding episode-based bundled payment models for hand surgery. Health care and productivity costs associated with the surgical management of hand and wrist pathologies represent a substantial burden on the United States health care system. Traditional fee-for-service models fail to incentivize interdisciplinary collaboration and optimization of resources. More recently, the concept of episode-based bundled payments has evolved as a potential solution to rising health care costs by encouraging care coordination, streamlining billing proc
