Endurance exercise training: conditions of enhanced hemodynamic responses and tolerance to LBNP.

In cross-sectional comparisons, several investigators have reported highly trained endurance athletes to have a prevalence toward orthostatic hypotension and intolerance compared with average fit individuals. These observations have raised concern that regular exercise designed to increase aerobic capacity may impair regulatory mechanisms of blood pressure control and that perhaps certain populations of individuals with a predisposition for fainting exhibit an inability to elevate heart rate, vasoactive hormones, and peripheral resistance during an orthostatic challenge. In longitudinal experi

Hyperventilation in response to progressive reduction in central blood volume to near syncope.

INTRODUCTION: There is little evidence to support the usefulness in monitoring respiration during casualty triage and transport as an early indicator of hemorrhage severity and trauma patient outcome. We, therefore, tested the hypothesis that hyperventilation can be elicited by progressive reductions in central blood volume independent of metabolic stimuli. METHODS: Progressive central hypovolemia was induced in 10 healthy subjects (5 men, 5 women) by applying lower body negative pressure (LBNP). The LBNP protocol consisted of a 5-min controlled rest period (0% LBNP) followed by progressive 5

The effect of oscillatory hemodynamics on the cardiovascular responses to simulated hemorrhage during isocapnia.

During cerebral hypoperfusion induced by lower body negative pressure (LBNP), cerebral tissue oxygenation is protected with oscillatory arterial pressure and cerebral blood flow at low frequencies (0.1 Hz and 0.05 Hz), despite no protection of cerebral blood flow or oxygen delivery. However, hypocapnia induced by LBNP contributes to cerebral blood flow reductions, and may mask potential protective effects of hemodynamic oscillations on cerebral blood flow. We hypothesized that under isocapnic conditions, forced oscillations of arterial pressure and blood flow at 0.1 Hz and 0.05 Hz would attenu

Visual disturbances during prolonged space missions.

PURPOSE OF REVIEW: During prolonged spaceflight, astronauts often experience ocular changes, due to constant head-ward fluid shifts in space as compared with Earth. This article reviews symptoms, likely causes, and potential solutions, such as lower body negative pressure, to counteract space-associated neuroocular syndrome (SANS). RECENT FINDINGS: Low gravity conditions and other aspects of spaceflight affect the eye detrimentally, causing SANS which is characterized by optic disc edema, choroidal thickening, cotton wool spots, and a hyperopic shift. SANS is probably caused by altered hemody

Skin heating during simulated hemorrhage lowers arterial blood pressure but not tolerance following exercise in a cold environment.

Skin heating helps avoid hypothermia in trauma victims but may influence the systolic (SBP) and mean arterial blood pressures (MAP) helping to guide resuscitation. We examined the effect of skin heating on tolerance and arterial blood pressure during lower body negative pressure (LBNP) across four trials. Nine participants completed 15 exercise intervals [60 s 88% peak power output (PPO) and 60 s 10% PPO] in a cold environment (0°C, 70% RH) lowering mean skin temperature (Tsk) before undergoing LBNP to pre syncope where Tsk remained low (Cold Trial: 27.6 ± 1.1°C) or was increased via a water-p

Percutaneous gastrostomy for decompression in patients with advanced gynecologic malignancies.

The objective of this study was to evaluate the use of percutaneous decompression gastrostomy in patients with gynecologic malignancies. Decompression gastrostomy tubes were inserted percutaneously in patients with recurrent gynecologic malignancies and small bowel obstruction or fistulae. The technique was evaluated for successful gastric decompression, acute and long-term complications, and palliation of symptoms. Twenty patients underwent placement of percutaneous gastrostomy tubes for decompression. Tube placement was successful in all patients, and all had significant relief of symptoms.

Decoding Blood Volume Loss: Can Non-Invasive Signals Match the Gold Standard?

The potential of advanced waveform analysis of physiological signals to diagnose varying levels of hypovolemia has not been fully explored. This study extends previous work by comparing the discriminative ability of invasive and non-invasive signals to classify levels of ongoing hypovolemia using a deep learning (DL) framework. Hypovolemia was simulated via a dynamic lower body negative pressure (LBNP) model among healthy volunteers, allowing for real-life-like fluctuations in intravascular blood volume caused by bleeding and resuscitation. The analysis incorporated photoplethysmography (PPG)

Anesthesia blocks the cardiovascular response to baroreceptor unloading in rats.

Baroreflex responsiveness and orthostatic stability in humans can be assessed by a variety of approaches, including exposure to graded levels of lower body negative pressure (LBNP). However, such approaches have limited applicability in animal studies owing to the need to anesthetize or sedate the animal. We recently reported a novel approach for the assessment of baroreceptor responsiveness in the awake rat using LBNP and presented preliminary findings that 3% isoflurane anesthesia completely blocked the normally robust baroreflex. In the present study, we sought to extend these findings by s

Physical fitness and cardiovascular regulation: mechanisms of orthostatic intolerance.

We studied three groups of eight men each--high, mid, and low fit (peak O2 consumption 60.0 +/- 0.8, 48.9 +/- 1.0, and 35.7 +/- 0.9 ml.min-1.kg-1)--to determine the mechanism of orthostatic intolerance in endurance athletes. Tolerance was defined by progressive lower body negative pressure (LBNP) to presyncope. Maximal calf vascular conductance (Gmax) was measured. The carotid baroreflex was characterized using both stepwise R-wave-triggered and sustained (2 min) changes in neck chamber pressure. High-fit subjects tended to have lower LBNP tolerance than mid- and low-fit subjects but similar b

Differential effects of central blood volume loading and unloading on pulmonary diffusing capacity in humans.

Central blood volume (CBV) is a key haemodynamic determinant of pulmonary diffusing capacity for carbon monoxide (DLCO) and its components, capillary blood volume (Vc) and membrane diffusing capacity (Dm,CO). However the contribution of CBV to DLCO regulation remains poorly defined. We tested the hypothesis that graded increases and decreases in CBV would elicit differential changes in DLCO, primarily driven by Vc. Eighteen healthy young adults (6 women; age: 25 [3] years; V ̇ O 2 peak : 44.5 [8.5] mL/kg/min) completed supine graded lower body positive (LBPP) and negative pressure (LBNP). D

COMPARISON BETWEEN ACTIVE ABDOMINAL COMPRESSION-DECOMPRESSION CARDIOPULMONARY RESUSCITATION AND STANDARD CARDIOPULMONARY RESUSCITATION IN ASPHYCTIC CARDIAC ARREST RATS WITH MULTIPLE RIB FRACTURES.

Background: Active abdominal compression-decompression cardiopulmonary resuscitation (AACD-CPR) is potentially more effective for cardiac arrest (CA) with multiple rib fractures. However, its effect on survival rates and neurological outcomes remains unknown. This study aimed to assess if AACD-CPR improves survival rates and neurological outcomes in a rat model of asphyctic CA with multiple rib fractures. Methods: Adult male Sprague-Dawley rats were randomized into three groups-AACD group (n = 15), standard cardiopulmonary resuscitation (STD-CPR) group (n = 15), and sham group (n = 10)-after b

Autonomic Afferent Dysregulation in Atrial Fibrillation.

OBJECTIVES: This study sought to evaluate the role of cardiac afferent reflexes in atrial fibrillation (AF). BACKGROUND: Efferent autonomic tone is not associated with atrial remodeling and AF persistence. However, the role of cardiac afferents is unknown. METHODS: Individuals with nonpermanent AF (n = 48) were prospectively studied (23 in the in-AF group and 25 in sinus rhythm [SR]) with 12 matched control subjects. We performed: 1) low-level lower body negative pressure (LBNP), which decreases cardiac volume, offloading predominantly cardiac afferent (volume-sensitive) low-pressure barorec

Vagal function in achalasia of the cardia.

Autonomic nervous function in achalasia of the cardia was assessed by measuring the response of the lower oesophageal sphincter to abdominal compression, the gastric secretory response to insulin-induced hypoglycaemia and the pulse rate variability with deep respiration. Twenty-eight patients with symptomatic achalasia and 24 age and sex-matched control subjects were studied. Rise in intra-abdominal pressure normally causes a rise in lower oesophageal pressure through a vagally-mediated mechanism. Before treatment this response was unimpaired in eight of 10 patients with achalasia. A sub-norma

Directional sensitivity analysis of the cerebral pressure-flow relationship during normothermia and moderate hyperthermia.

Dynamic cerebral autoregulation (dCA) reacts differently when mean arterial pressure (MAP) increases versus decreases (i.e., directional sensitivity). Although heat stress alters dCA, its influence on directional sensitivity remains unclear. This analysis investigated the impact of moderate hyperthermia on the directional sensitivity in the cerebral pressure-flow relationship. Ten healthy participants (7 males; age: 37 ± 12 yr; body mass: 75 ± 9 kg) underwent 6 min of oscillatory lower body negative pressure (OLBNP) to induce large MAP fluctuations at 0.03 and 0.10 Hz under normothermic and mo

Increase in reflex vasoconstriction with indomethacin in patients with orthostatic hypotension and central nervous system involvement.

Since indomethacin may be effective in the treatment of orthostatic hypotension, the ability of this drug to increase reflex vasoconstriction was studied in six patients with orthostatic hypotension and in five normal subjects. Reflex forearm vasoconstriction during lower body negative pressure at 20-40 mm Hg was measured before and after indomethacin 50 mg by mouth. In patients with orthostatic hypotension and central nervous system involvement indomethacin increased recumbent blood pressure, resting forearm vascular resistance, and reflex forearm vasoconstriction during lower body negative p

Beneficial effects of elevating cardiac preload on left-ventricular diastolic function and volume during heat stress: implications toward tolerance during a hemorrhagic insult.

Volume loading normalizes tolerance to a simulated hemorrhagic challenge in heat-stressed individuals, relative to when these individuals are thermoneutral. The mechanism(s) by which this occurs is unknown. This project tested two unique hypotheses; that is, the elevation of central blood volume via volume loading while heat stressed would 1) increase indices of left ventricular diastolic function, and 2) preserve left ventricular end-diastolic volume (LVEDV) during a subsequent simulated hemorrhagic challenge induced by lower-body negative pressure (LBNP). Indices of left ventricular diastoli
