CT-based three-dimensional kinematic comparison of dart-throwing motion between wrists with malunited distal radius and contralateral normal wrists.

AIM: To compare motion of the capitate, scaphoid, and lunate in wrists with a malunited distal radius and contralateral normal wrists during dart-throwing motion (DTM) by three-dimensional kinematic studies using computed tomography (CT) images. MATERIALS AND METHODS: CT was performed simultaneously on both wrists in six patients with a unilateral distal radius malunion at three stepwise positions simulating DTM. Using volume registration technique, the kinematic variables of helical axis motion of the capitate, scaphoid, and lunate were calculated and compared between both wrists. The helica

[Fracture of the lunate--a rare injury].

Fractures of the normal lunate--isolated or associated with fractures of the scaphoid or radius--are very rare. In the Traumatological Hospital Meidling/Vienna, 12 patients with acute fractures of the lunate bone were treated between 1983 and 1993. Dependent on the fracture-line and the intraosseous vascularity, partial or total avascular necrosis of the bone occurs in rare cases. In dislocated pole fractures, surgical intervention is indicated.

Arthroplasty for advanced Kienböck's disease using a radial bone flap with a vascularised wrapping of pronator quadratus.

We have performed a form of lunate replacement arthroplasty, which included excision of the lunate and insertion of a vascularised radial bone flap wrapped in pronator quadratus, for stage IIIB or stage IV Kienböck's disease, in 41 patients who have been followed up for more than three years. All patients reported an improvement in their symptoms, and 20 of the 41 became free of pain after the operation. Extension and flexion of the wrist were increased by a mean of 9 degrees and 6 degrees , respectively (p < 0.05). The radioscaphoid angle and the carpal height ratio were not significantly cha

A 32-year follow-up of lunate excision for Kienbock's disease: a case report and a review of results from excision and other treatment methods.

Lunate excision alone is seldom utilized in the management of Kienbock's disease due to concerns about progressive carpal collapse following removal of this central carpal bone. We report a 32-year follow-up of a patient who underwent lunate excision only for treatment of Kienbock's disease with a successful outcome. Although lunate excision is thought to be associated with a high failure rate, a review of the literature suggests that success rates following lunate excision are comparable to those reported for other more conventional techniques such as radial shortening, ulnar lengthening, lim

[Management of malacia of the os lunatum by implantation of a dorsal metacarpal vascular bundle].

In the treatment of malacia of the lunate bone, beside the prosthesis and procedures aiming the decrease of load, the revascularization of the bone is the third basic therapeutic principle. One possibility of the later, not yet current in this country, is the implantation of the metacarpal vascular bundle in the lunate bone. As far as we know we were the first to perform this operation in Hungary. In this paper our first cases are described together with a detailed description of the operative technique and we would call at the same time to further common examinations.

A novel arthroscopic technique utilizing bone morphogenetic protein in the treatment of Kienböck disease.

Kienböck disease, first described in 1910, is osteonecrosis of the carpal lunate and has been associated with ulnar minus variance. Numerous joint leveling procedures have been developed for patients with ulnar-negative variance to decrease forces transmitted across the lunate. The basis of operative treatment is the presence or absence of advanced osteoarthritis associated with fragmentation of the carpal lunate and capitate descent. Bone morphogenetic proteins (BMPs) have been utilized successfully as adjunctive treatment in fracture healing and recently in the surgical treatment of a patien

Transulnar styloid palmar scapho-lunate dislocation with median nerve injury.

A patient with a transulnar styloid palmar scapho-lunate dislocation with median nerve injury is described. The dislocation could be reduced by closed manipulation under anaesthesia, and the scapho-lunate ligament was repaired subsequently using a Mytek Micro bone anchor. This case is reported for its rarity and its management. Although closed reduction can be achieved by manipulation, scapho-lunate ligament repair is essential to prevent rotatory instability of the scaphoid with this pattern of injury.

[Kienböck's disease. Diagnosis and therapy].

Kienböck's disease (carpal lunate necrosis or lunatomalacia) is the most common aseptic osteonecrosis of the upper extremity. Other bone necroses in the hand occur less frequently. The name indicates a disease with unclear etiology which eventually always leads to necrosis of the lunate bone. It usually affects the dominant hands of males between 20 and 40 years of age. Treatment methods extend from immobilization to revascularization surgery on the affected bone. There is still no gold standard for the treatment of Kienböck's disease.

The relative contribution of selected carpal bones to global wrist motion during simulated planar and out-of-plane wrist motion.

The relative contribution of the scaphoid, lunate, triquetrum, and capitate to wrist motion was examined in 6 fresh cadaver forearms. A wrist-joint motion simulator was used to dynamically move each wrist through planar and nonplanar motions. During wrist flexion-extension, the motion of the capitate closely followed the motion of the third metacarpal, while the lunate motion was approximately 50% of the total motion; the triquetrum, 65%, and the scaphoid, 90%. Similar differences in motion for these carpal bones occurred during radioulnar deviation and circumduction and dart-throw motions. Th

Joint-preserving management of Kienböck's disease.

Kienböck's disease, an uncommon avascular necrosis of the lunate, remains challenging to manage, particularly in young adults. We report a woman in her 20s with early-stage disease (Lichtman stage II) and positive ulnar variance, but without lunate collapse. She underwent a distal radial osteotomy with autologous cancellous bone grafting, stabilised with a volar locking plate, along with intraosseous lunate decompression. Postoperative recovery was uneventful, with complete union at the osteotomy site, restoration of wrist motion and return to unrestricted activity at 1 year. This case illustr

[Numerical variants and congenital fusions of carpal bones].

The number of carpal bones may be increased or decreased by the fact of anatomical variants or true congenital anomalies. Numerical increment arises from additional or from split bones. Over twenty accessory carpal bones have been described but the commonest are the os centrale carpi, the os radiale externum, the triangular bone and the styloideum bone. Additional carpal bones usually result from a failure of fusion of their ossification centers. A congenital origin is not clearly established for all these ossicles. The scaphoid and lunate may split into two or three bones and several cases of

[A prospective study of early diagnosis of lunate necrosis by means of MRI].

AIM: Early recognition of lunate necrosis by means of MRI. METHODS: 49 patients with typical symptoms and normal radiographs were examined. Coronary T1 and T2 weighted images were obtained. In addition, an attempt was made to quantify fluid shift and measure of bone marrow oedema by means of extreme T2 weighting (TR 5950 ms, TE 150 ms). RESULTS: Three instances of necrosis were found; in two cases it was possible to demonstrate marrow oedema. Oedema preceding necrosis could not be demonstrated by quantifying a grey scale. CONCLUSIONS: The pattern of pain in the region of the lunate is not t

There Was No Evidence of Osteonecrosis in Lunates of Early-Stage Kienbock's Disease.

BACKGROUD: The etiology and pathogenesis of Kienbock's disease (KD) have not been clearly established. In osteonecrosis of other bones, such as the proximal femur, the interface between necrotic and normal bone is evident in early stages. The purpose of this study was to retrospectively review and analyze magnetic resonance (MR) images of early-stage KD to confirm whether the double-line sign or an abnormal signal intensity band corresponding to the reactive interface is present. METHODS: We retrospectively reviewed 31 cases of 30 patients (average age, 41.2 years) diagnosed with early-stage

Superselective angiography of the wrist in patients with Kienböck's disease.

BACKGROUND: Microvascular problems like increased intraosseous pressure or venous congestion may influence the development of Kienböck's disease. We examined if wrist position modifies the blood flow in the nutrient vessels. METHODS: Retrospective analysis of 17 patients with Kienböck's disease who had a superselective microangiography of the radial, ulnar and interosseous artery in different wrist positions under general anaesthesia. We analysed the data with Fisher's exact and Wilcoxon-test. RESULTS: We found vessels that entered the bone, that ended at the bone edge, and that supplied a v

Metaphyseal Core Decompression of the Distal Radius for Early Lunate Necrosis.

Background: Metaphyseal core decompression of the distal radius (MCD) is clinically effective in early lunate necrosis without changing individual wrist mechanics. Its concept is based on the induction of physiologic mechanisms known as physiologic fracture healing response. However, this biological concept does not yet have its place in the historically developed mechanical concepts about Kienböck's disease and requires more detailed clarifications to understand when a change of individual wrist mechanics might be unnecessary. Methods: Thirteen consecutive cases, Lichtman stage I (n = 1) or I

Subcutaneous extensor tendon rupture caused by Kienböck disease complicated by carpal tunnel syndrome: A case report.

RATIONALE: Subcutaneous extensor tendon rupture caused by Kienböck disease is rare. Only 22 cases have been reported in the English literature since 1986. PATIENT CONCERNS: A 74-year-old male experienced numbness of his right hand for several years and was unable to extend his right middle and ring finger 2 months before consultation. DIAGNOSES: Physical examination revealed no pain in his right wrist, but was unable to extend the middle and ring fingers. He also had numbness of the thumb, index, middle, and radial side of the ring finger. Plain radiography and computed tomography revealed o

Collapse After Carpal Bone Tunneling: An Underreported Complication?

PURPOSE: To report the complication of carpal bone tunnel collapse in scapholunate reconstruction. METHODS: The authors present six cases of carpal collapse or bone necrosis after bone tunneling for ligamentous reconstruction, three in the scaphoid bone and three in the lunate. RESULTS: All six cases were secondary to ligament reconstruction for chronic scapholunate dissociations, none of which had preoperative structural scaphoid or lunate deformities. CONCLUSIONS: Although the incidence of this occurrence is unknown, the consequences could be substantial; therefore, patients should be war

Existing or Worsening Symptoms of Kienböck's Disease after Distal Radius Fracture: Two Case Reports.

We report two rare cases of existing or worsening symptoms due to Kienböck's disease after distal radius fracture (DRF). During examination, radiographs show changes in the lunate bone; there was persistent wrist pain after treatment for DRF. In each case, surgeries were performed: A combined therapy (bone marrow transfusion, bone peg graft, external fixation, and low intensity pulsed ultrasound) for one, as well as carpal coalition for another. The etiology of these case presentations suggest that a compression fracture of the lunate due to a DRF resulted in softening and sclerosis.

Radioscapholunate arthrodesis using low-profile dorsal pi plate.

Radioscapholunate arthrodesis is a desirable method to treat isolated radiolunate arthritis or ulnar translocation of the carpal bones. An intact midcarpal joint is a prerequisite for functional range of motion. Previously, high rates of nonunion were observed with these procedures, as rigid fixation was difficult to obtain with simple Kirschner wires and screws. A successful outcome depends on bringing the scaphoid, lunate, and the radius to an anatomic alignment, and rigid fixation of the arthrodesis. We describe a technique for the arthrodesis of the radioscapholunate joint from the dorsal
